The previous Annual Compliance with Rated License Assessment Visit was completed July 9, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 87%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-star license, issued February 27, 2020, earning seven (7) points in education standards, two (2) points in program standards, and one quality (1) point by choosing to meet a programmatic option. The information on record with DCDEE was verified as correct by Stephanie Allen and the owner’s corporation, Tiny Treasures Daycare & Learning Center, LLC status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed March 28, 2025. Fire Classification of approved completed February 6, 2025. Stephanie Allen, administrator, was present and assisted me while I observed the indoor and outdoor areas. Upon my arrival, there were thirty-three (33) children in care. Children in space #1/2 were sitting at a table discussing the 4th of July. There were fifteen children, ages three through five years old and one teacher. According to the floor plan the capacity for this space is fourteen and the staff/child ratio is 1/10 for the youngest child present (three years of age). I observed preparation for the outdoors. Children lined up for toileting and handwashing. In space #3/4, children were observed playing with age-appropriate materials like, cars, people, rings, and soft pillows. I observed diaper changing and handwashing procedures. In space #5, school-aged children were sitting at a table drawing the United States Flag. They used crayons, glue sticks and paper. They counted the stars and stripes. When finished, the children were given a math ditto sheet. Next, they participated in stretching exercises. In space #6, children were sitting with their teacher, walking, crawling, and sleeping in cribs. Positive communication was observed. Teachers spoke to children about what they were doing. Three bottles were stored in the refrigerator without dates. This was corrected during the visit. Today’s lunch was consisted of chicken nuggets, fries, baked beans, fruit cocktails, wheat crackers, and milk. At the completion of the visit, the Visit Summary was reviewed with S. Allen. The following violation was observed: Violation Number Comment Rule 103 The number of children present was not within permit capacity. There were fifteen children in space #1/2. The capacity is fourteen. The previous Annual Compliance with Rated License Assessment Visit was completed July 9, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 96%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-star license, issued February 27, 2020, earning seven (7) points in education standards, two (2) points in program standards, and one quality (1) point by choosing to meet a programmatic option. There was one teacher with five children, one year of age and four children, two years of age. Therefore, the purpose of today’s visit is to verify enhanced staff/child ratios are in compliance. Stephanie Allen, administrator, was present and assisted me while I monitored for enhanced staff/child ratios. Upon my arrival, there were thirty (30) children in care. Children in space #1/2 were sitting at tables participating in group time activities. Children in space #3/4, were using puzzles and musical instruments. Children in space #6, were using soft blocks, books, and stackable rings. According to today’s menu, lunch will be consisted of franks, beans, wheat crackers, oranges, and milk. Later I observed one teacher walking to the front entrance door of the facility leaving one teacher alone in space #3/4 with five children, age one-year old and five children, age two-years old for approximately one-minute. Enhanced staff/child ratio was not verified in compliance during the visit. Therefore, Item # 1756 was cited during the visit. This is a repeat violation from 1/8/25. In addition, a violation was cited for TB test results. At the completion of the visit, the Visit Summary was reviewed with Ms. Stephanie Allen. The following violations were observed. I observed diaper changing, handwashing and sanitation in space #6. Children were observed toileting, eating lunch, and resting on cots. Lunch consisted of riblets, mash potatoes, green beans, pineapples, wheat crackers, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Stephanie Allen. The following violations were observed: Violation Number Comment Rule 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. The facility must receive at least five points in program standards to increase the star rating to five-stars.
