The previous Annual Compliance Visit was completed December 12, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 95%. This program currently operates with a Three- Star License issued October 30, 2021. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Tahshia Johnson, administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed December 19, 2025. Fire Classification of Approved completed December 15, 2025. Ms. Johnson was present and assisted me while observing the indoor areas. In Space #1, the children were playing on the carpet with books, musical toys, and blocks. In Space #2, the children were sitting on the carpet listening to their teacher read a book. A group of children also played with puzzles and pushed small cars. In Space #3, the children were observed in the following activities: painting, drawing on the chalk board, pretending to cook food in the dramatic play area, and building with geometric shapes. In Space #4, the children were using musical shakers, bells, and one child was sitting on the carpet reading books. The lunch served was macaroni and cheese, pasta, broccoli, sliced apples, and milk. The most recent fire drill was completed on June 12, 2026, and the most recent 3-month shelter-in-place drill was completed March 06, 2026. The EPR plan was last updated on September 23, 2026. The most recent playground check was completed for June 12, 2026. Ten (10) new staff files were reviewed. The administrator completed the staff and training worksheet prior to my visit. The information was reviewed for accuracy. In addition, the Criminal Background Checks were reviewed for all staff. At the completion of the visit, the Visit Summary was reviewed with Ms. Johnson. No violations were observed. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. There are books that can be printed out and utilized in your facility to help manage children’s behaviors. For example, Tucker Turtle Takes Time to Tuck and Think. There are some resources and training that are free of charge. The steps to access the resources are located on the main page, select For Teachers and Caregivers, and then you will see the option for the book to print. Once you print the book, the staff can utilize sheet protectors and use a binder. It may be best to print a cover page and add on the front of the binder. For additional accessories, provide a soft turtle to use as a prop. The staff members can place these items in the cozy area as a way for the children to transition when he or she is having a difficult moment. There were twelve books that contained pages and ripped book covers located in the classroom. This was corrected when the administrator removed the books not in good repair. In addition, the teacher can block out time to discuss how to take care of the books. This can include how to turn the pages, and being gentle when handling the books. The lunch served was turkey and ham, corn, baked beans, apples, cornbread, and milk. The most recent fire drill was completed on May 16, 2025, and the most recent shelter in place drill was completed April 17, 2025. The most current monthly outdoor inspection was completed in May 2025. At the completion of the visit, the Visit Summary was reviewed with Ms. McCrowie. The following violation was observed: Violation Number Comment Rule 505 Drinking water was not freely available to children of all ages. There were two groups of children outdoors and did not access to drinking water. This was corrected when the administrator stated that the staff members will bring the container of water from the classroom to outdoors, and drinking water will be free available at all times. .0901(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today was corrected during today’s visit. A compliance letter is not required. Facility Specific TA/Consultation/Reminders : Drinking Water: A violation was observed today for drinking water not accessible to children. During outdoor play, drinking water was not available. It is required to have drinking water available at all times. Refer to Child Care Rules: .0901(e), for more information on this rule requirement. Technical Assistance: A violation was not cited but a reminder was provided to ensure that books are not ripped or torn. There was one book that was ripped in Space #2. The staff members will need to evaluate the learning space to ensure that all toys, materials, books, and equipment are all in good repair. In addition, in Space #1, in the drawer there was a large zip block bag with extra toys, make sure that staff members are using a replacement for storage, and avoiding plastic in toddler/preschool classrooms. Observation/Technical Assistance (TA): An observation was observed in the toddler/preschool classroom and school age classroom. In both classrooms, both staff members provided opportunities for learning and development. The staff members talked to the children and encouraged the children to conversate. In the toddler/preschool classroom, I observed some children not engaging in activities. I suggest providing an alternative activity when the child does not want to participate. Periods of Waiting for Young Children: There is a way to create waiting to be exciting and fun if the period of waiting, is not too long. Limited waiting can help children learn self-control and regulating behaviors. Remember, long periods of waiting can lead to challenging behaviors. Some suggestions that can be used for short periods of waiting include using a timer (the children will know when the timer goes off, that means we transition to the next activity). The staff member can play “Simon Says” or ask how many “circles (use different shapes)” or “purple things (name different colors)”, etc. can you find? In addition, involve children in the transitions, for example bringing markers, or napkins, etc. More importantly, have a plan for children waiting their turn. In Space #3, the children put on their jackets and the teacher counted the children before going outdoors. Lunch served was turkey with cheese, tortilla wrap, broccoli, pears, and milk. During today’s visit for the Administrative Action (AA) Three Month Written Warning was reviewed. The facility was issued the administrative action dated November 05, 2024. The action was received on November 15, 2024. The action was placed near the entrance of the facility. During today’s visit, the stipulations were reviewed. At the completion of the visit, the Visit Summary was reviewed with Ms. Johnson. The following violation was observed: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. In Space #2, there were two places on the flooring that contained uneven parts. The flooring was lifting up from the base of the flooring. On one part in front of the sink, seven (7) inches of flooring was not in good repair and one part in front of the changing table, five (5) inches of flooring was not in good repair. Additionally, by 5:00pm on December 30, 2024, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. In Space #2, the children were listening to music and playing their musical instruments. Space#7, the children were sitting on the carpet singing songs. Lunch served was turkey ham, green beans, bread, mixed fruit, and milk. There is a concern that inappropriate discipline was used. The concern was discussed with Ms. Johnson. Interviews were conducted with the administrator, and one staff that had a conference with the parent and the child. The staff member is employed as an Office Assistance. Regarding the concerns that inappropriate discipline was used, observations and interviews were conducted. Classroom observations were observed. The staff member stated that she had a conference with the parent, and she interviewed the child. When asked what happened, in reference to a hit, the child stated, “she didn’t hit me, she thumped me”. The child used their fingers and motioned a thump action. The child used their thumb and middle finger and pressed together. The child stated that he was thumped in the back of his head. The staff member stated that the child was able to touch the back of his head where he was thumped. The administrator stated that the teacher involved denied the allegation. The staff member involved was not interviewed because their last day of employment was September 23, 2024. In addition, the discipline policy was reviewed and the camera footage. Based off the camera footage on September 20, 2024, the time started around 4:25 pm, it was observed that the child came out of the bathroom holding his head and crying. It appeared that the teacher did not respond to their child’s cry or try to assist the child with their injury. The child leaned against the wall then walked away when the child’s parent came in the room. When the child’s parent came into the room, the child was talking to the parent, and it appeared the child was explaining about the incident that occurred. The teacher walked up to the child and rubbed their head, then proceeded to walk away. The parent turned to the teacher and talked to the teacher and conversation occurred between the two, then the parent took their child out of the classroom. The video did not have sound, but the video had a clear picture of what may have occurred based off of the body language of the child prior to entering the bathroom to leaving the bathroom. Before the child ran into the bathroom, the child was walking and running around the classroom and playing with the other children. The child was not crying prior to walking into the bathroom. It was less than ten seconds, when the child went from playing to crying and holding his head. The camera footage does not show footage inside of the bathroom. Based on the observations and interview information, the allegation that inappropriate discipline was used is substantiated. At the completion of the visit, the Visit Summary was reviewed with Vesca McCrowie, staff member. The following violation was observed: Violation Number Comment Rule 872 Appropriate discipline practices were not followed. It was confirmed by the staff member that she talked to the child involved with the parent present, and the child stated that the teacher did not hit him, she "thumped" him in the head. The child also showed the motion of how the child was "thumped" in the head, by using his fingers to show the motion. In addition, the camera footage was observed and the child was running and playing prior to going in to the bathroom, then in less than ten seconds, the child came out of the bathroom, holding his head and crying. This was corrected when the administrator removed the teacher from the classroom on September 23, 2024, and the the last day of employment was on September 23, 2024. .1803 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Due to the substantiated complaint, it may result in more stringent administrative action. Currently, the facility has a written warning. If a more stringent action is warranted, you will be notified by telephone and/or email. Discipline Policy: A violation was cited today for inappropriate discipline. Although an action has already been put in place, and the employee identified in the allegation last day of employment was September 23, 2024, it is best to ensure this situation does not occur again. It is best to review the discipline policy with all staff, and provide examples of appropriate discipline practices, nurturing and positive environment. Staff members need to make sure that if they observe another staff member not providing positive discipline, nurturing and positive environment, to notify the administrator. The administrator will need to evaluate the situation to determine the next steps. It may be best practice to modify procedures when teachers are helping children conduct toileting and or handwashing in areas that are not accessible for the camera view and/or an area secluded from other staff and children. It may be best to ensure that another staff member is present anytime, a child may need assistance in an area secluded from other staff or children. It is best to provide procedures to help eliminate situations that may be harmful to children.
