The previous Annual Compliance Visit was completed September 9, 2025, prior to today’s visit, the eighteen-month compliance history score was at 96% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued November 13, 2023. The License was based on the program earning 7 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option by having staff benefits and infrastructure of parent involvement. The information on record with DCDEE was verified as correct. Inspections are as follows: Sanitation Classification of Superior completed September 17, 2025. Fire Classification of Satisfaction completed January 9,2026. Mammie Summers, and Hillary Summers, were present while I observed the indoor and outdoor areas. Upon my arrival, I observed children participating in free choice activities indoors. During free play, the children participated in dramatic play. During group time, a variety of musical instruments were used with the assistance of H. Summers. Lunch documented on the menu for today is pizza, corn, fruit cocktail and milk. At the completion of the visit, the Visit Summary was reviewed with M. Summers. The following violation(s )were observed. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. M.andH.Summers first aid training certification on file expired February 4,2026. .1102(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00 pm on March 31,2026 you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. TA: Planned routines and experiences allow children to be successful with little support. I discussed with M. Summers the results from the classroom observations conducted during today’s visit. H. Summers, engaged in conversation with children. M. Summers assisted the children with sharing during morning activities. Children were engaged in all classroom activities and were responsive to the teachers as they asked questions. There were ample materials in each interest center for children to use at their choice. The facility uses redirection to assist with keeping the learning environment safe. Discussion: NCDHHS Responds to Increase in Measles Cases: Measles is on the rise globally and in North Carolina. As a highly contagious, airborne disease, even one case is a major priority for public health. NCDHHS is taking action by launching new outreach resources for child care providers, families, and community stakeholder to help curb the spread. Child care providers can share information about vaccinations with the families served and be informed with the most current information. Capacity: We discussed staff/child ratios for a center located in a residence with a licensed capacity of 13 to 15 children. Ages of Children, Ratios, Staff/Children Maximum Group Sizes are as follows: 0 to 12 Months, 1/5, 10 12 to 24 Months, 1/6, 12 2 to 13 Years, 1/10, 15 3 to 13 Years, 1/15, 15. At this time M.Summers will like to keep her capacity at 12. Files A violation regarding staff files was observed and documented today. Record keeping is one of the most time-consuming tasks associated with administration. It is an important part of licensure. Staff records consist of various documentation such as employment application, medical report, TB test results, Annual Health questionnaire, Emergency contact information, CRC qualification letter, documentation of orientation, in service training hours, CPR and First Aid, and playground safety. I discussed with the administrator to monitor due date for first aid certifications. Also monitor all files every month. The previous Annual Compliance with rated license assessment Visit was completed November 13,2023, prior to today’s visit, the eighteen-month compliance history score was at 99% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued November 13, 2023. The License was based on the program earning 7 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option by having staff benefits and infrastructure of parent involvement. Upon my arrival, there were seven children in care. I observed children participating in free choice activities indoors. Children cleaned-up and were given rhythm sticks and shakers. Music was played and children danced with their musical instruments. Next, the children sat at tables and participated in a group activity singing songs as they took turn toileting and getting diapers changed. They sang songs like, “Monkeys Jumping on the Bed”. They listen and pointed to their body parts (head, shoulders. Knees etc.). Outdoors, children ran, walked, pushed and rode bikes and Little Tyke cars. Once indoors, the children washed their hands and the tables were properly sanitized. According to the menu, lunch will consist of peanut butter, Ritz crackers, broccoli, pineapples, and milk. At the completion of the visit, the Visit Summary was reviewed with M. Summers. The following violations were observed: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child's file did not include a parent's signed acknowledgement form regarding receiving the Summary of the NC Child Care Law. GS 110-102 1314 Emergency information did not name childs health care professional. One child's application did not include the name of a health care professional. .0802(c)(2) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. One child's file did not include a signed parent's acknowledgement form regarding the facility's smoking and tobacco restriction. .0604(j) You must correct the above violations immediately. Lunch served were Vienna sausage, wheat crackers, bananas, peas, and milk. A check list was used to note the requirements I monitored today. At the completion of the visit, the visit summary was reviewed with M. Summers. The following violation(s) were observed.
