Name of Operation: SHANA'S CHILD DEVELOPMENT CENTER Facility ID: 26000716 Consultant: SANDRA JOHNSON Operation Type: Center Case Number: Visit Date: 3/24/2025 Number Present: 1 Completed Date: 3/24/2025 Age: From 7 To 7 Total Minutes: 110 Time In: 02:25 PM Time Out: 04:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a routine unannounced visit. The previous Annual Compliance Visit was completed September 24,2024 and prior to today’s visit, the eighteen-month compliance history score was at 100% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued November 2, 2023. The License was based on the program earning seven (7) points in education standards, two (2) points in program standards, and one quality point by choosing to meet an education option the program has combined turner over rate of 20% or less for the administrator, program coordinator, lead teacher, teacher, and group leader over the last 12 months. The information on record with the DCDEE was verified as correct by Samuel Cothran, Operator. Current Inspections are as follows: Sanitation Classification of Superior completed 10/22/2025. Fire Approved for daytime and night care 4/17/2024. Mr. Cothran was present while I observed the indoor and outdoor areas. Upon my arrival no children were present. One child arrived at 3pm. Upon the child’s arrival, the child put his personal items away in individual cubby and participated in hand washing routines. The child then had a snack. After the snack the child was observed participating in outdoor activities. The child was able to ride his bike. The provider stood out in the play area and talked with the child while he rode his bike. They talked about the child’s day at school as well as his birthday that is coming up. The snack served today was apple juice, goldfish, and apple sauce. At the completion of the visit, the Visit Summary was reviewed with Mr. Cothran. The following violation was observed. Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The provider who is scheduled to work with children did not complete ITS-SIDS training every three years. The last time the training was completed was 1/11/2022. .1102(f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violation observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on April 7, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Mail or email the information to: Sandra Johnson Child Care Consultant P.O. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TA: An observation was completed from 3:10p.m. to 3:25 p.m., the child was engaged in riding his bike. He was allowed to pick what activities he did outdoors. The child really enjoyed going outdoors before doing homework or indoor activities. The provider was very engaged in talking with the child about his day. I recommended that the provider keep having engaging conversations with the child. A violation was cited regarding ITS-SIDS Training. Refer to child care rule 10A NCAC 09 .1102(f). We discussed the following strategies: Mark on calendar when trainings must be complete and how often. Keep a dry erase board with all staff and dates of documents and when they must be renewed. Reminders: The DCDEE has a new initiative – Natural Outdoor Learning Initiative. Through this initiative, we hope to: increase knowledge and confidence for everyone in developing and implementing best practices to support the creation, programming, and safe use of naturalized outdoor spaces in child care facilities.
