Name of Operation: PRECIOUS LAMBS QUALITY CHILD CARE SERVICE Facility ID: 26000991 Consultant: RONA DAVIS Operation Type: Family CC Home Case Number: Visit Date: 11/18/2025 Number Present: 5 Completed Date: 11/18/2025 Age: From 0 To 5 Total Minutes: 210 Time In: 09:30 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance with applicable child care requirements pertinent to an annual compliance visit. The previous Annual Compliance Visit was completed December 6, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 100%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued March 17, 2020. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an infant capacity restricted to no more than two children under one year of age. The information on record with DCDEE was verified as correct by Cynthia Walker. Current Inspections are as follows: Sanitation Classification of Superior completed October 20, 2025. A copy of the sanitation inspection was received during today’s visit. The information was put in the Regulatory system. Cynthia Walker, Operator, was present and assisted me while I observed the indoor and outdoor areas. There were five (5) children present. Upon my arrival the children were sitting on the carpet having a discussions. Next two children sat at a table and played with colored magnetic windows. Two other children sat on the carpet building with colored blocks. I observed clean-up time, toileting, and hand washing. One child was sitting in a rocker using soft toys. Ms. Walker often picked the child up, talked to her, and ensured that she had soft toys. Later, the children sat at a table and used crayons to draw on a large paper. Lunch consisted of chicken, rice, oranges, green beans wheat bread and milk. At the completion of the visit, the Visit Summary was reviewed with C. Walker. The following violations were observed: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility's ABCMS Roster was not created and available for review. G.S. 110-90.2 & .2703(r) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The operator did not complete health and safety training after five years of completing the initial health and safety training. .1703(d)(2) The violation(s) must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on December 2, 2025, describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. Box 1218 Raeford, NC 28376 Technical Assistance/Reminders ABCMS – CBC Provider Portal Reminder: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff and or household members are noted on the roster. The information should be updated in the ABCMS on an ongoing basis. Should you need assistance contact the Criminal Background Check Unit at (919)814-4401 and someone will assist you. Health and Safety training topics: must be completed within the first year. Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. The “Recognizing and Responding to Child Abuse and Maltreatment” training must be completed within 90 days of hire. Documentation of training must be kept on file for review. Complete the health and safety training record as found on the DCDEE website. After the first year, staff must complete the health and safety training topics at least once every five years. Your Health and Safety Trainings are due to expire on December 2, 2025. Health and Safety Training courses are available in DCDEE Moodle, free of charge. Log into DCDEE Moodle with your NCID, search for and register for the required training. Capacity Change: We discussed changing your facility license capacity to 10. You will need to maintain fire safety, evacuation, and lockdown plan. A carbon monoxide alarm and detective system. Once you meet the requirements email me with a request to increase your facility’s capacity to 10. QRIS Modernization Pathway: During today’s visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation you will be pursuing the Classroom and Instructional Quality Pathway 2.
