The previous Annual Compliance Visit was completed September 10, 2025, and prior to today’s visit, the eighteen-month compliance history score was 98%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued April 5, 2019. The License was based on the program earning five (5) points in education standards, two (2) points in program standards, and one quality point by choosing to meet an education option of a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher, group leader over the last 12 months. The information on record with DCDEE was verified as correct by Wendy Mathis and the owner’s corporation status, Ponderosa Daycare, LLC, was verified as current-active. Current Inspections are as follows: Sanitation Classification of superior completed May 7, 2025. Fire Classification of approved completed March 14, 2025. Wendy Mathis, Administrator, was present and assisted me while observing the indoor and outdoor areas. Upon my arrival, there were no children in care or enrolled at the facility. At the completion of the visit, the Visit Summary was reviewed with W. Mathis. There was one violation observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A current fire inspection was not on file. The last fire inspection was conducted on 3/15/25. 10A NCAC 09 .0304(a) Facility Specific TA/Consultation/Reminders: We discussed letting me know 24 hours after enrolling children in care. Contact your Environmental Health Specialist regarding having no children in care. QRIS Modernization Pathway: During today’s visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation you will be pursuing the Classroom and Instructional Quality Pathway (2). You plan to submit your application for a rated license on June 15, 2026. Fire Inspection: A violation was cited for not having a current fire inspection. You stated that you are scheduled for a fire inspection tomorrow. Remember to email me a copy no later than one week after the inspection is conducted. Refer to child care rule .0304(a). At the completion of the visit, the Visit Summary was reviewed with W. Mathis. The following violations were observed: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child's file did not have a signed parent's acknowledgement form regarding receiving the Summary of the NC Child Care Law. GS 110-102 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. One child's file did not have a signed parent's acknowledgement form regarding receiving the program's discipline policy. .1804(c) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The information should be updated in ABCMS on an ongoing basis as staff members are hired and when employment is terminated. This satisfies the requirement to notify the Division of new child providers working who were hired or moved into the child care facility within five business days. The previous Annual Compliance Visit was completed September 26, 2023, and prior to today’s visit, the eighteen-month compliance history score was 97%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued April 5, 2019. Snack consisted of cookies, raisins and apple juice. After snack children put together a United States puzzle. Ms. Mathis asked questions about the floor puzzle and if they wanted help. Once the puzzle was completed, they put away the puzzle and prepared for outdoors. They took a basketball and football with them. Once outdoors, they played basketball and used positive communication. The children washed their hands upon returning indoors and participated in an art activity (drawing). At the completion of the visit, the Visit Summary was reviewed with W. Mathis. The following violation was observed. When planning activities for your school age program, take time to look and listen to your students. Reflect on what they are interested in and let them work with you to transform interest into learning experiences.
