Name of Operation: ONE OF A KIND HOME DAY CARE Facility ID: 26002444 Consultant: SANDRA JOHNSON Operation Type: Family CC Home Case Number: Visit Date: 12/2/2025 Number Present: 1 Completed Date: 12/2/2025 Age: From 2 To 2 Total Minutes: 175 Time In: 02:35 PM Time Out: 05:30 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a routine unannounced visit. The previous Annual Compliance Visit was completed April 23, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 100% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a three-Star License issued June 25, 2021. The License was based on the program earning four (4) points in education standards, two (4) points in program standards, and one quality point by choosing to meet a programmatic option: with serves no more than two infants under one year of age. The information on record with the DCDEE was verified as correct by Brenda Small, Operator. Current Inspections are as follows: Sanitation Classification of Superior completed 7/23/2025. Ms. Small was present while observing the indoor and outdoor areas. Upon my arrival there was one child present. The child was resting in a portable crib. Snack served today will be wheat crackers and bananas, and juice. An observation was completed from 3p.m. to 3:20p.m. The child was awakened by the provider and assisted her with getting up. The child participated in bathroom routines and was served snacks. After having snack, the child washed her hands and then participated in a drawing and writing activities. The operator sat with her and gave her the words she needed or was trying to say. At the completion of the visit, the Visit Summary was reviewed with Ms. Small. No violations were observed. Violation Number Comment Rule 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The operator did not complete ITS-SIDS every three years. ITS-SIDS training expired 12/7/2024. .1703(a)(4) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The operator had not completed Recognizing and Responding to of Child Maltreatment within the five years of completion. The last time was completed was 9/18/2019. .1703(d)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on December16, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TA: During the observation the child was engaged in writing and drawing activities while the operator sat with her and talked about the activities. . She talked about the child siblings and described the children while she drew her pictures. The child was smiling, laughing and having conversations with the child. The operator gave her different words to say to help her in describing what she wanted to draw and write. I encouraged the operator to keep having good interaction with the children, talking, playing, and singing with them. This will help with the children’s vocabulary, social and emotional skills. During today’s visit the following information was reviewed and forms left for the Pathways to the Stars. (New QRIS), for pathway #2, Education Standards, CQI Plan, Family and Community Engagement Standards. Ms. Small stated that she has already begun to work on the requirements for pathway #2 and that she is working with Kathy Hoffler, Family Child Care Consultant, with the Partnership for Children, who has already begun to work with her. We tried to review her education in Works Unit and were unable to complete. I will review and email her by end of week. The plan is for Ms. Small to have completed all requirements submitted in March for April of 2026 and to submit all documents for a five-star license. During today’s visit, I discussed access and use of the NCFELD book in the development of activity planning as required by NC Child Care Rule 10A NCAC .0508 Activity Schedules and Plans. Your program has a copy of the publication.
