The previous Annual Compliance visit was completed May 06, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 98%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three Star License issued December 1, 2023. The License was based on the program earning five points in education standards, two points in program standards, and one quality point by choosing to meet a programmatic option for Infant capacity restricted to no more than two children under one year of age. The information on record with DCDEE was verified as correct by Ms. Nyree Wooten. Current Inspections are as follows: Sanitation Classification of Superior completed March 27, 2025. A violation was observed for the sanitation inspection. Ms. Wooten was present and accompanied me while observing the indoor and outdoor areas. Upon arrival, there were four children present. The caregiver talked to the children and helped the children with routine tasks. The children played with toys and the caregiver talked to the children about what they were playing with. After playing in centers, the children played outdoors. Outside the children explored the area. The following activities were observed: slides, outdoor toys, running, and playing with outdoor learning equipment. After the learning activities, the children washed their hands in preparation for lunch. The children transitioned to the area for lunch. Lunch served was chicken nuggets, macaroni and cheese, peas, apple slices, and milk. After lunch, the children completed toileting and handwashing routines and transitioned to their cot. There were violations observed for Criminal Background Check. The most recent fire drill was completed on April 06, 2026, and the most recent lockdown drill was completed April 06, 2026. The most current monthly outdoor inspection was completed on April 06, 2026. At the completion of the visit, the Visit Summary was reviewed with Ms. Wooten. The following violations were observed. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Prior to the expiration date of the qualification letter, the operator Nyree Wooten did not complete requirements to complete a criminal background check. The qualification letter expired on September 21, 2025. Today you stated that you attended the EPR training on August 10, 2024, the EPR Plan will be required to be completed by December 10, 2024. During today's visit, I was able to review that you had completed most of your plan online. Refer to Child Care Rules 10A NCAC 09 .1714, for more information regarding this rule requirement. Classroom Observations/TA Consultation: I observed some children not engaging in activities. I suggest providing an alternative activity when the child does not want to participate. In addition, you can visit http://csefel.vanderbilt.edu/ for resources to help manage behaviors and provide strategies to help your non-verbal children. This website is the Center on the Social and Emotional Foundations for Early Learning (CSEFEL) that focuses on promoting the social emotional development and school readiness of young children birth to age 5. There are books that can be printed out and utilized in your facility to help manage children’s behaviors. For example, Tucker Turtle Takes Time to Tuck and Think. There are some resources and training that are free of charge. The steps to access the resources are located on the main page, select For Teachers and Caregivers, and then you will see the option for the book to print. Once you print the book, the staff can utilize sheet protectors and use a binder. It may be best to print a cover page and add on the front of the binder. For additional accessories, provide a soft turtle to use as a prop. The staff members can place these items in the cozy area as a way for the children to transition when he or she is having a difficult moment. A rated license assessment was conducted. The previous visit was completed August 23, 2023, and prior to today’s visit, the eighteen-month compliance history score was 99% as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a temporary license effective, May 31, 2023 to November 30, 2023. The information on record with DCDEE was verified as correct by Ny’Ree Wooten. We discussed the importance of reviewing all children’s files to ensure accuracy with completion of all required areas of the child's application. The administrator should not sign off on the document until all areas of the form have been addressed. Additionally, documentation of receipt of operational policies shall be on file the first day of attendance for each child enrolled in the FCCH and a medical report on file within thirty days of enrollment. A child enrolled June 5, 2023, did not have a medical report on file on or before July 5, 2023. As a reminder, the medical report on file shall be dated/completed within the last twelve months. Rated License Assessment: The FCCH must transition to a 3-5 Star License to continue to be eligible to receive subsidized care after six (6) months. Current Inspection is as follows: Sanitation Classification of Superior with three demerits completed June 16, 2023. N. Wooten, Operator/Teacher, was present and assisted me during the visit. N. Wooten accompanied me during the walk-through of the indoor and outdoor areas. Preschool-age children participated in free play activities inside and outside. The theme for this week is “Zoo Animals.” During circle time, the children and N. Wooten sung “Sally the Camel” and “Five Little Monkeys Jumping on the Bed.” They practiced singing the alphabet and counting by rote to ten. N. Wooten engaged the children in conversation and asked open-ended questions. After circle time, children transitioned outside with N. Wooten. The children slid on the Little Tykes slide, threw balls, and played in the Little Tykes Playhouse/Farmers Market. Upon their return indoors, N. Wooten assisted the children with toileting and hand washing routines. The children transitioned to activity areas set up in the primary care space. They manipulated magnetic tiles and pop-its, colored at the art table, looked at books, and built with Lego blocks. Children cleaned up and washed their hands prior to lunch being served. Lunch served was spaghetti noodles with ground beef and pasta sauce, sliced bread, green beans, applesauce, and milk. The children’s files and N. Wooten’s personnel file were monitored during todays’ visit. In addition, required program records were also monitored. At the completion of the visit, the Visit Summary was reviewed with N. Wooten.
