The previous Annual Compliance Visit was completed September 12, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 90% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Notice of Compliance issued February 18, 2026. The information on record was verified as correct by Tonya Bolton, Assistant Administrator. Current Inspections are as follows: Sanitation Classification of Superior completed 10/13/2025. Fire Approved for daytime care only 12/15/2025. Ms. Thomason and Ms. Bolton were present while I was observing the indoor and outdoor areas. Upon entering the facility children were observed in their classrooms participating in small group activities. Today’s lunch was chicken or beef tacos, corn, rice, fruit, and milk. An observation was completed from 9:20a.m. to 9:40 a.m. In space # 104 and #105 preschool children were observed participating in small group activities and free choice play. The teachers assisted the children as needed. One teacher sat with the children and was working on a math activity with the children. The teacher sang a song with the children to assist them while coloring. The song was to help them color in the lines. In both spaces the children were very engaged. They worked well together. At the completion of the visit, the Visit Summary was reviewed with Ms. Bolton. The following violation was observed. Violation Number Comment Rule 1867 The depth of the loose surfacing was not based on critical height of the equipment. The depth of the loose wood mulch surfacing was measured at 2inches in depth. It does not meet the 6inches required based on the height of the large play structure in the outdoor play environment. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Once the children transitioned, they were observed participating in toileting routines, large group, small group, and writing activities. Today’s lunch was Pizza, carrots, and fruit cups, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Bolton. The following violations were observed. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The facility did not have a Thermometer for the refrigerator. This was corrected by the facilities supervisor providing a thermometer to the administrator and the administrator place it in the refrigerator and the temperature being read a 45 degrees. 15A NCAC 18A .2806(j)(2) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member hire date of 11/6/2024, did not have a medical report prior to employment. This was corrected by the staff member submitting and medical report on 11/24/2024. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Both documents can be found on the DCDEE Website underneath the provider and professional development tabs. An observation for space #101, was completed from 10:05a.m. to 10:20a.m., the children were observed participating in free choice play. The children were observed building a castle with magnetic block. Three children were observed drawing, writing and doing a word to match the picture worksheet. The teacher moved about the space assisting the children. She would ask the children questions about what they were building. She shared with them other things within the classroom that they could use to help with building their castle. An observation for space # 105, was completed from 10:35a.m. to 10:50a.m., the children were observed participating in a math activity with the teacher assisting them. The teacher was teaching the children how to add by 5 fives. They used cube blocks and themselves as a counting source. She taught them that if you do not have enough of one thing you can use other items to help you create the number you need. During the observation of enrollment. There were two children present how were 6 years of age and had not started public or private school yet. This upon the parents’ request. The permit restriction shows that the facility can only serve children aged 3 to 5 years old. At the completion of the visit, the Visit Summary was reviewed with Ms. T. Thomason.
