The previous Annual Compliance Visit was completed March 03, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 99%. This program currently operates with a Five -Star License issued November 3, 2020. The License was based on the program earning seven points in education standards, seven points in program standards, and one quality point by choosing to meet a programmatic[DM2.1] option for: Has reduced group size by one in each group from the seven point level as required in Rule .2818(c) The information on record with DCDEE was verified as correct by Ms. Mary Green, Lead Teacher and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed December 09, 2025. Fire Classification of Approved completed March 12, 2025. Ms. Green was present and accompanied me while observing the indoor and outdoor areas. In Space #243, the children were sleeping on their cots with quiet music playing. In Space #247, the children were sleeping on their cots, with quiet music playing. After rest time, the children cleaned up their things, completed toileting, and hand-washing routines. A violation was observed for storage of hazardous items and verification for CPR. Snack served was graham crackers and juice. The most recent fire drill was completed January 29, 2026, and the lockdown drill was completed January 16, 2026. The outdoor inspection form was completed January 7, 2026. At the completion of the visit, the Visit Summary was reviewed with Ms. Green. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #243, there were five (5) large refill containers of hand soap stored in an unlocked cabinet, below the sink. This was corrected when the staff member removed the containers and placed them in a locked storage room. .2820(b) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. There was one staff member that did not have verification of completion for the CPR course. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00pm on February 19, 2026, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. On May 08, 2025, one violation was observed for discipline, and one violation was observed for nurture treatment of child. Today’s visit was to verify compliance. All classrooms were complying with discipline requirements and nurture treatment of child. At the completion of the visit, the Visit Summary was reviewed with Ms. Green. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #243, there was one aerosol can of Lysol stored on a shelf, greater than 5 feet. The was corrected when the staff member removed the aerosol can from the shelf and placed in a locked storage. .2820(b) 860 Balloons were accessible to children. In Space #247, the balloons were stored inside clear cubes. The balloons contained air. This was corrected when the staff member let the air out of the balloons and removed the balloons from the classroom. .0604(q) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Storage of Hazardous Items A violation was observed today for Storage of Hazardous Items. Store items that are labeled “keep out of reach of children” and have no other warning can be stored a shelf or in an unlocked cabinet. The shelf or cabinet must be mounted at least five feet above the floor. In one classroom, there was one can of aerosol stored in an unlocked cabinet, five feet above the floor. Aerosol cans must be stored in locked storage. A handout was left today for Hazardous Items Storage for Child Care. It is important to make sure cleaning supplies are stored at least 5 feet from the floor. This will ensure that potentially harmful products will be out of reach for the children. Today we talked about different substitute items that can be used in place of the balloons. There are colorful balls that can be used. In Space #243, violations were observed for discipline and nurture of care. Lunch served was pizza, carrots, apples, and milk. There are allegations of violations of child care requirements, including: • Discipline • Nurture of Care I interviewed the administrator, Brady Davis, to gather more information regarding the allegations and to inform him of the purpose of my visit. The administrator stated that this was a substitute staff member, and the staff member informed him about what had happened verbally. The administrator had the staff member write a statement about what happened. During today’s visit, Mr. Davis provided me with a copy of the statement. The administrator stated that verbally and in writing the staff member confirmed that she hit a child on their hand. I interviewed the Lead Teacher, that was present in the classroom with the substitute, and the teacher stated that she did not hear anything. She was working one on one with a child for testing. She also stated that the child named in the report did not cry or say anything. On the day of the incident, the teacher stated that the substitute named in the report did not inform her of “popping the child” until after the children returned indoors from outdoor play. After the children returned indoors, the substitute named in the report confirmed that she had “popped the child on the hand”. She stated that the teacher wanted to talk to the administrator. The written statement that was completed by the staff member (substitute) named in the report included that she said the child’s name “Stop!” and she “popped her hand”. The staff member stated in the written statement that they both looked up at each other in shock. She then took her aside and apologized and explained that even though she need to follow directions, she shouldn’t have “popped” her hand and she felt bad that she did. Based on the observations and interview information, the allegation for discipline is substantiated. Based on the observations and interview information, the allegation for nurture of care is substantiated. At the completion of the visit, the Visit Summary was reviewed with Ms. Green. Facility Specific TA/Consultation/Reminders : Administrative Action: Due to the substantiated complaint, it may result in administrative action. Nurturing and Positive: A violation was cited today because it is required for staff members to provide a nurturing and positive environment for children. It is best to review the discipline policy and provide examples of a nurturing and positive environment. Staff members need to make sure that if they observe another staff member not providing a nurturing and positive environment, to notify the administrator. If there are concerns, the administrator will need to evaluate the situation to determine the next steps. Refer to child care rule G.S. 110-91(10). Discipline Policy: A violation was cited today for inappropriate discipline. . No child shall be subjected to any form of corporal punishment The employee identified in the allegation last day of employment was May 1, 2025. It is best to ensure this situation does not occur again. It is best to review the discipline policy with all staff, and provide examples of appropriate discipline practices, nurturing and a positive environment. Staff members need to make sure to be aware of all activities within the classroom. This includes constantly looking up to evaluate the children. It may be best practice to modify procedures when one teacher is responsible for one on one activities and one teacher is working with the other group of children. It is best to provide procedures to help eliminate situations that may be harmful to children. Please refer to 10A NCAC 09 .1803 for more requirements regarding this rule. I interviewed the Lead Teacher, that was present in the classroom with the substitute, and the teacher stated that she did not hear anything. She was working one on one with a child for testing. She also stated that the child named in the report did not cry or say anything. On the day of the incident, the teacher stated that the substitute named in the report did not inform her of “popping the child” until after the children returned indoors from outdoor play. After the children returned indoors, the substitute named in the report confirmed that she had “popped the child on the hand”. She stated that the teacher wanted to talk to the administrator. The written statement that was completed by the staff member (substitute) named in the report included that she said the child’s name “Stop!” and she “popped her hand”. The previous Annual Compliance Visit was completed March 12, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 100% which is above 75% as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Five- Star License issued October 10, 2024. The License was based on the program having a Cognia Accreditation. The information on record with DCDEE was verified as correct by Mary Green, Lead Teacher, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. The children were transitioning from breakfast into large group time. After group time the children transitioned to free choice play. Children were observed playing with ABC dinosaur connectors, painting, making small structures with wood blocks, and playing with hand puppets. Lunch served today was cheese dunkers, green beans, carrots, mixed berry juice, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Green. Name of Operation: NEW CENTURY INTERNATIONAL ELEMENTARY Facility ID: 26002170 Consultant: NANCY STRATFORD Operation Type: Center Case Number: Visit Date: 3/3/2025 Number Present: 32 Completed Date: 3/3/2025 Age: From 4 To 5 Total Minutes: 240 Time In: 08:45 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to an annual compliance visit, including compliance with requirements located in Child Care Rule Section .3000 in Spaces #245 & 246, which serve children participating in the NC Pre-K program, and Health and Safety Training. The previous Annual Compliance Visit was completed March 12, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 100% which is above 75% as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Five- Star License issued October 10, 2024. The License was based on the program having a Cognia Accreditation. The information on record with DCDEE was verified as correct by Mary Green, Lead Teacher, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 2/3/2025. Fire Classification of Approved for daytime care only completed 9/16/2024. Ms. Green was present while I observed the indoor and outdoor areas. Upon entering the childcare spaces children were observed washing their hands. The children were transitioning from breakfast into large group time. After group time the children transitioned to free choice play. Children were observed playing with ABC dinosaur connectors, painting, making small structures with wood blocks, and playing with hand puppets. Lunch served today was cheese dunkers, green beans, carrots, mixed berry juice, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Green. The following violations were observed. Violation Number Comment Rule 1867 The depth of the loose surfacing was not based on critical height of the equipment. The outdoor area where the children play the surfacing depth measured 3 inches in depth and based on the height of the equipment the depth has to be maintained at 6 inches. .0605(k)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff had not completed recognizing and Responding to Suspicions of Child Maltreatment training within 90 day. However one staff did completed on 2/25/2025. One staff still has not completed the hire date for this staff is 10/4/2024 .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on March 17, 2025 , you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. Refer to sanitation rule 10A NCAC 09 .605 K (1-4). We discussed the following strategies: Monitor the play areas near the slides, the steps, and any other area where the children may run or jump from the play equipment. Make sure there are not areas of the playground that has holes or dips with no surfacing where the children play. Reminders: The DCDEE has a new initiative – Natural Outdoor Learning Initiative. Through this initiative, we hope to: increase knowledge and confidence for everyone in developing and implementing best practices to support the creation, programming, and safe use of naturalized outdoor spaces in child care facilities.
