The previous Annual Compliance Visit was completed April 24, 2025, prior to today’s visit, the eighteen-month compliance history score was at 94% which at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four (4) -Star License issued February 16, 2022. The License was based on the program earning four (4) points in education standards, five (5) points in program standards, and one (1) quality point by choosing to meet an education option in which the child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct. The owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed March 13, 2026 Fire Classification of satisfactory completed September 2, 2025 Oradia Bastardo, staff member, accompanied me while observing the indoor and outdoor areas.One teacher M. Fuertes assisted one child with hand washing routines after playing with play dough. Staff encouraged children to clean up their areas and prepare for outdoor time. Outdoors, children climbed on stationary equipment. Upon return indoors, teachers assisted children with toileting, and hand washing until lunch were served. Lunch, prepared and served by the cook, consisted of cheese pizza, corn, sliced apples, oranges, and milk. At the completion of the visit, the Visit Summary was reviewed with M.Blanche. The following violation(s) were observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Both violations were corrected, the owner M. In Space #INF, toddlers were playing on the floor with soft blocks, musical activity boxes, and exploring the classroom area. I observed the teacher assist one child with hand washing routines. Children and staff in space #PS were observed singing and dancing to children’s music and transitioned to free choice centers. Staff read books to individual children while another teacher assisted children with coloring pictures with colored pencils on white art paper. Staff encouraged children to clean up their areas and prepare for outdoor time. Outdoors, children played on toy rockers, climbed on stationary equipment, and rode tricycles. Upon return indoors, teachers assisted children with toileting, diapering, and hand washing and listened to story time on the carpet, Lunch, prepared and served by the cook, consisted of ground beef, black beans, rice, sliced grapes, and milk. Following lunch children washed their hands and rested on individual mats with clean linen. Soft instrumental music played as children slept. A checklist was used to note the requirements monitored during the visit. The Staff and Training Worksheet was received on April 17, 2025 and updated during today’s visit and employee files were reviewed. A Children’s Record form was completed and two files were monitored. The most recent fire drill and a shelter-in-place emergency drill were completed on April 4, 2025. The monthly outdoor inspection checklist was completed on April 2, 2025. At the completion of the visit, the Visit Summary was reviewed with M. Blanche. The following violations were observed: Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. In the outdoor play area, two climbing structures with slides and a two-seater rocker measured more than 18 inches high were not installed over protective surfacing that was at least 6 inches in depth. .0605(j) 721 All equipment and furnishings were not in good repair. In the outdoor play area, a child-sized resin table was broken and exposed sharp jagged edges. The violation was corrected when the staff member removed the table and placed it outside of the fenced play area. Upon return indoors, teachers assisted children with toileting, diapering, and hand washing and listened to story time on the carpet, Lunch, prepared and served by the cook, consisted of ground beef, black beans, rice, sliced grapes, and milk. Following lunch children washed their hands and rested on individual mats with clean linen. Soft instrumental music played as children slept. A checklist was used to note the requirements monitored during the visit. The Staff and Training Worksheet was received on April 17, 2025 and updated during today’s visit and employee files were reviewed. A Children’s Record form was completed and two files were monitored. The most recent fire drill and a shelter-in-place emergency drill were completed on April 4, 2025. The monthly outdoor inspection checklist was completed on April 2, 2025. In space INF, a two year old child received care in the same space as an infant. The violation was corrected when the administrator moved the two year old child to space PS. 10A NCAC 09 .0713(a)(5) 428 A current activity plan was not posted for each group of children for reference. The most current activity plan posted in space SA was dated November 4 to November 8, 2024. The violation was corrected when the teacher placed the updated activity plan for November 12 to November 15, 2024 on the parent information board. GS 110-91(12); .0508(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The most current health questionnaire on file for staff member K.T. rehired November 4, 2024 was completed on April 11, 2023. The violation was corrected when the administrator had the staff member complete the document during the visit. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The most current emergency information form on file for staff member K.T. rehired November 4, 2024 was completed on April 11, 2023. The violation was corrected when the administrator had the staff member complete the document during the visit. .0701(a) 1739 All records required were not available for review by a representative of the Division. An employee file was not available for review for staff member K.T., who was rehired on November 4, 2024. The violation was corrected when the administrator obtained the file for another licensed facility where the employee is currently employed. .2318(1-8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation observed and documented today was corrected immediately therefore a compliance letter is not required. Facility Specific Technical Assistant /Consultation/Reminders: Resource: Observation Suggestion: I observed staff members having conversations with children before rest time and when they were awakened from nap. Teachers were attentive to children who were crying and having a hard time transitioning from one activity to another. When school-aged children arrived, staff were observed greeting them and asking about their day. I encourage staff to continue with make materials readily available for children. I encourage staff to continue planning activities with the interests of the children in mind and having meaningful conversations with children individually and within each age group of children. Grouping of Children: The violation for grouping of children was cited today in space #INF. As a reminder, children under one year of age must be kept separate from children two years and older. I suggest teachers communicate with administrators and other staff members requesting assistance when children arrive at the facility. Also, all staff should become familiar with staff/child ratios and grouping of children for each age group and classroom space. It is required that programs maintain required group sizes for centers as defined in 10A NCAC 09 .0713. The previous Annual Compliance Visit was completed May 9, 2023, prior to today’s visit, the eighteen-month compliance history score was at 96% which at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four (4) -Star License issued February 16, 2022. The License was based on the program earning four (4) points in education standards, five (5) points in program standards, and one (1) quality point by choosing to meet an education option in which the child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Margarita Blanche, Administrator and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed April 16, 2024. Fire Classification of satisfactory completed August 9, 2023. Upon my arrival, Santa Ortega, Lead Teacher greeted me at the door. Ms. Blanche arrived later during the visit. I observed the indoor and outdoor areas. In Space #3, infants were exploring their space while the teacher completed diaper changing and handwashing routines. The teacher described her and the children’s actions as she changed children diapers. In space #2, children were observed participating in free choice activities, including dramatic play, and manipulatives. Children moved freely about the space. After free play, the children went outdoors to engaged in gross motor activities. (running, bouncing balls, chasing each other). Bathroom, and handwashing routines were also observed. Lunch served today was hamburger/bun, lettuce and tomatoes, grapes, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Blanche. No violations were observed.
