Health and Safety training requirements were monitored. The previous Annual Compliance Visit was completed February 25, 2025, and prior to today’s visit, the eighteen-month compliance history score is 97%, which exceeds requirements as outlined in North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five (5) Star License issued October 10, 2024. The License was based on the program issued October 10, 2025. The License was based on the program obtaining the Cognia accreditation. The information on record with DCDEE was verified as correct by Cheryl Frye. Cheryl Frye, Lead Teacher, and Tajhai Black, teacher, were present while observing the indoor area. The outdoor play area was monitored unaccompanied by a staff member. I observed staff and children in circle time on the carpeted floor. C. Frye discussed dental care with children and read the book “Bear’s Loose Tooth”. Children recalled the parts of the book and how to take care of books when C. Frye prompted them. Children danced to the “Brush Your Teeth” and talked about procedures of proper teeth brushing. Staff assisted children in transitioning to free choice activities in interest centers. Children were observed playing with colored magnetic tiles, searched for dinosaur fossils in kinetic sandbox, and played with framed and floor puzzles on the carpet. T. Black assisted a group of children with writing their names with colored markers and drawing individual pictures. Children created towers using unit blocks and dinosaur families and played dress up with community helper uniforms in dramatic play. T. Black and a group of children picked up lunches from the cafeteria. Children completed hand washing and prepared for mealtime. Lunch served was chicken tenders with biscuit, green beans, chilled mixed fruit cup, and milk. Teachers encouraged children to open their own milk cartons and packages and assisted as needed. Following lunch children completing personal care routines and rested on individual cots with clean linen. A checklist was used to note the requirements monitored during the visit. The Staff and Training worksheet on file was updated and all employee files were monitored. The Children’s Record form was completed and three (3) files were monitored. The most current monthly fire drill and was conducted January 23, 2026. A quarterly lockdown emergency drill was completed on February 6, 2026. A monthly playground inspection was completed on January 30, 2026. At the completion of the visit, the Visit Summary was reviewed with C. Frye. The following violations were observed: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In the outdoor play area, I observed an active fire ant mound in the mulch surfacing. The ramp leading to the climbing structure had metal stakes that were not flush with the surface and caused a tripping hazard. 10A NCAC 09 .0601(a) 1054 Documentation of staff's on-going training was not on file and/or was not current. Children were engaged in building structures with unit blocks and vehicles, playing with dramatic play materials such as dress up clothes, pretend foods, and toy shopping carts. Children in small group were observed manipulating Lego blocks and building structures at child-sized tables. Two children were observed writing their names on writing paper with colored markers. Children were encouraged to clean their spaces and transition to the carpet in preparation for mealtime. Children completed personal care routines and hand washing as the substitute teacher cleaned and sanitized the tables. For lunch, children were served chicken strips with whole wheat waffle, corn, chilled strawberries, and milk. Teachers assisted children with preparing for rest time. Children rested on individual cots on clean linen as soft instrumental music played. A checklist was used to note the requirements I monitored today. A completed staff and training worksheet was received and updated during today’s visit. Children’s Record worksheet was completed for three (3) children’s files monitored. Ms. Frye reminded children of activity area rules and guided them to transition to free choice activities in interest centers. Children were engaged in building structures with unit blocks and vehicles, playing with dramatic play materials such as dress up clothes, pretend foods, and toy shopping carts. Children in small group were observed manipulating Lego blocks and building structures at child-sized tables. Two children were observed writing their names on writing paper with colored markers. Children were encouraged to clean their spaces and transition to the carpet in preparation for mealtime. Children completed personal care routines and hand washing as the substitute teacher cleaned and sanitized the tables. For lunch, children were served chicken strips with whole wheat waffle, corn, chilled strawberries, and milk. Teachers assisted children with preparing for rest time. Children rested on individual cots on clean linen as soft instrumental music played. A checklist was used to note the requirements I monitored today. A completed staff and training worksheet was received and updated during today’s visit. Children’s Record worksheet was completed for three (3) children’s files monitored. Name of Operation: LOYD AUMAN ELEMENTARY Facility ID: 26002134 Consultant: YOLANDA PRIDGEN Operation Type: Center Case Number: Visit Date: 9/27/2023 Number Present: 15 Completed Date: 9/27/2023 Age: From 4 To 5 Total Minutes: 255 Time In: 09:15 AM Time Out: 01:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a routine unannounced visit including compliance with requirements located in Child Care Rule Section .3000 and Health and Safety Training. Space #1 of the program serves children participating in the NC Pre-K program. The previous Annual Compliance Visit was completed April 6, 2022, and prior to today’s visit, the eighteen-month compliance history score is 94%, which exceeds requirements as outlined in North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five (5) Star License issued January 31, 2020. The License was based on the program earning seven (7) points in education standards, seven (7) points in program standards, and one (1) quality point by choosing to meet an educational option: Has reduced group size by one in each group from the seven point level as required in Rule .2818 (c). The information on record with DCDEE was verified as correct by Cheryl Frye. Current Inspections are as follows: Sanitation Classification of superior completed September 22, 2022. Documentation of staff contacting Cumberland County Environmental Health Department was on file however an updated inspection was not on file. Fire Classification of approved for day time care only completed September 13, 2023. Cheryl Frye, Lead Teacher, and Michaela Mayer, Teacher, were present and accompanied me while observing the indoor area. I observed the outdoor area unaccompanied. Upon arrival, children were sitting at child-sized tables eating breakfast. Children consumed blueberry muffins, 100% fruit juice, and milk. Teachers monitored and assisted children with hand washing before transitioning to large group activity on the carpet. Ms. Frye and children discussed seasons changing and read the book "Leaf Jumpers” and how different trees have different colors of leaves. Children listened attentively and answered questions from Ms. Frye at the end of the story. I observed children dancing and moving to the Seasons Song using the whiteboard. Ms. Frye guided the children as they transitioned from the carpet morning group time to free choice activities. Ms. Frye reminded children of the centers rules and how to take care of their materials and to ask for help if they need it. Children were engaged in building structures with unit blocks and vehicles, playing with dramatic play materials, and manipulating playdough with tools on activity mats. Children were observed cleaning their spaces and transitioned to prepare for mealtime. Children completed personal care routines and hand washing as Ms. Frye cleaned and sanitized the tables. For lunch, children were served chicken queso with tortilla chips, fiesta corn, fruit cocktail, and milk. Teachers assisted children with preparing for rest time. Children rested on individual cots on clean linen as soft instrumental music played. Sean Brost, PreK Coordinator for Cumberland County Schools, arrived and assisted during the visit. A completed staff and training worksheet was received and updated during today’s visit. A criminal background check qualification letter was not on file for Michaela Mayer It was discussed with Tara Bratcher, principal, and a substitute teacher, Karen Bennett was temporarily assigned to the classroom. Ms. Mayer was reassigned to another classroom until a qualification letter is received. A CBC application for M. Mayer had been submitted in the DCDEE ABCMS system however the status of a qualification letter was pending determination for M. Mayer. At the completion of the visit, the Visit Summary was reviewed with Ms. Frye. The following violations were observed: Violation Number Comment Rule 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The most current sanitation inspection report on file for the facility, conducted September 22, 2022, was not completed within 12 months of the previous inspection. GS 110-91 1041 Prior to employment a Criminal Background Check was not completed. Prior to employment, a Criminal Background Check was not completed for staff member M.M. hired August 23, 2023. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. G.S. 110-90.2(b) & (d) & .2703(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on October 11, 2023, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. I shared that additional information regarding the criminal background check process can be found in NC Child Care Rule .2700 and on our website at https://ncchildcare.ncdhhs.gov/Home/DCDEE-Sections/Criminal-Background-Check-Unit. A current NCID and updated fingerprints from local law enforcement will be required to complete the new online criminal background check process. Please be reminded, if a valid qualification letter must be obtained for M. Mayer by October 11, 2023, prior to returning to work in the licensed classroom space. Staff member M. Mayer must submit by scanning and emailing a current qualification letter to me no later than 5:00pm on October 11, 2023 to yolanda.pridgen@dhhs.nc.gov. Please be reminded, a valid CBC qualification letter is required and must be on file for all staff prior to employment. Sanitation Inspection: The sanitation inspection for the facility expired on September 22, 2023. Contact Environmental Health within two business days at 910-433-3676, to inform them of today’s visit and inform them that a sanitation inspection must be completed before October 11, 2023. Please scan and submit a copy of the sanitation inspection report attached to the compliance letter. July 2023 Child Care Rule Changes: The DCDEE is excited to share information about the July 2023 child care rule changes. The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. These rule changes may or may not impact your facility. The multi-age group staff/child ratio changes support a previous rule change submitted by Early Head Start and the new rule for defining cooperative arrangements provides clarification to a previous change in statute. A new July 2023 New Rule/Rule Amendments Overview module has been added to the Child Care Rules Training on DCDEE Moodle.
