The previous Annual Compliance Visit was completed January 27, 2025, prior to today’s visit, the eighteen-month compliance history score was at 93%, as required by North Carolina General Statute 110-90(4) (c-d). This program currently operates with a five-Star License issued May 23, 2023. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet an Educational Options: Completing 20 additional annual on-going training hours for full time lead teachers and teachers and staff working part-time. The information on record with DCDEE was verified as correct by Lora Fryar and the owner’s corporation status was verified on the Secretary of State’s website as Active - Not current. Current Inspections are as follows: Sanitation Classification of Superior completed June 17, 2025. Fire Classification of Approved Daytime Only completed May 1, 2025. Upon my arrival I was greeted by Christiane Mason, Lead Teacher. Ms. Fryar arrived to the facility at 9:00 a.m.; Three children were present upon entering the facility. The children were observed participating in breakfast. After breakfast the children transitioned to handwashing routines, and free choice play. The breakfast served today was pancakes, mixed fruit, and milk. During my observation from 9:05a.m. to 9:20 a.m., the children were observed playing in free choice play. The children were seen playing with puzzles, blocks, and manipulative toys. The teacher sat with the children and did an activity with letter recognition and sounds. The children enjoyed making different types of sounds. At the completion of the visit, the Visit Summary was reviewed with Ms. Fryar. The following violation was observed Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Facility did not have the ABCMS Roster available for review. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00pm on August 6, 2025 you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. Referring to General Statue rule 110 -90.2 & rule 2703(r) the following strategies were discussed. Ensure that you have created the portal of AMBCS and print the roster once completed. Keep on file. When staff leave or are let go remove from the roster in the portal the reprint. When new staff are hired add them to the roster in portal and reprint. Ensure you are updating and printing and available for review. During my observation the teacher showed reflected emotional connection with children by using respectful language, by addressing children by their name, and encouraging them to work and play together. The teacher was engaged with the children by asking questions of the children, standing close to them, and expressing the same emotions and reflection the child gave them. ABCMS – CBC Provider Portal During today’s visit a violation was cited for not having the ABCMS Roster available for my review. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024.Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The information should be updated in ABCMS on an ongoing basis as staff members are hired and when employment is terminated. This satisfies the requirement to notify the Division of new child providers working who were hired or moved into the child care facility within five business days. Your program had/didn’t have a copy of the publication. (If not – add this: A copy was left with you.) Additionally, the publication is available on-line, free of charge, at. https://ncchildcare.ncdhhs.gov/Portals/0/documents/pdf/N/NC_Foundations.pdf Training on use of the publication is available through our Partnership for Children, so please encourage Lead Teachers to visit their website ( https://ccpfc.org/ ) and complete the training. After our discussion, I did leave an NCFELD book for the facility to use. The Cumberland County Partnership for Children is sponsoring the “Let’s Get Connected” The School Aged Connection. The in-person event focuses on creating a safe and inclusive community for sharing and learning new concepts. This learning event is for directors and school-age staff only (newly hired and/or seasoned Group Leaders). Training topics to be covered are growth and development, classroom management, learning environments, building relationships, safety and wellness, positive behaviors, activity planning, and sanitation. This program currently operates with a five-Star License issued May 23, 2023. Upon Mrs. Fryar arriving at the facility she stated that the power company stated that there was a power outage. They stated that the power would be back on shortly. The power returned on at about 10:30a.m. The children were observed playing with soft toys, blocks, reading books, and stacking blocks. Lunch served today will be hot dogs, green beans, peaches, crackers, and milk. The Verification of Required Information for Operator and Additional Caregiver and the Operator’s Statement of Responsibility forms were updated during today’s visit. Your signature on these forms will serve as verification that the information provided was accurate and complete. At the completion of the visit, the Visit Summary was reviewed with Ms. Fryar. The following violations were observed. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. While monitoring of the outdoor space there were several piles of animal waste located in the play area where the children play. This was corrected by the animal waste was removed from the play area during the visit. 10A NCAC 09 .0604(p) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child did not have a immunization on file enrollment date 2/14/2023. 10A NCAC 09 .0302(d)(2) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was not updated annually an on file for review. .0607(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. When talking to her about the infant being cared for in an unapproved space. Ms. Fryar stated that the child was not enrolled, and that Ms. Lewis was taking care of the child in the home. When monitoring the outdoor space, there was a gentleman standing in front of the premises smoking and once seen he moved to the other side of the home. The gentlemen is contracted with health care agency to take care Ms. Fryar grandson. The gentlemen do not have access to the child care space at any time. Lunch served today was pizza, corn, applesauce, and milk. During my observation from 9:30a.m. To 9:50 a.m., the children were observed playing in free choice play. The children were seen playing with soft toys, puzzles, blocks, and musical toys. One of the teacher’s talked with children about why it is important to be supervised at a pool. They also had a contest of who drew the best picture about their summer. At the completion of the visit, the Visit Summary was reviewed with Ms. Fryar. GS 110-91(1)&(4-5) 820 A person(s) was smoking in a vehicle used to transport children or on the premises of the child care center. Upon walking outdoors there was a gentlemen standing outside on the sidewalk of the home smoking. The gentlemen is contracted with health care agency to take care Ms. Fryar grandson. The gentlemen do not have access to the child care space at any time. Lunch served today was pizza, corn, applesauce, and milk. During my observation from 9:30a.m. To 9:50 a.m., the children were observed playing in free choice play. The children were seen playing with soft toys, puzzles, blocks, and musical toys. One of the teacher’s talked with children about why it is important to be supervised at a pool. They also had a contest of who drew the best picture about their summer. At the completion of the visit, the Visit Summary was reviewed with Ms. Fryar. The following violations were observed: Violation Number Comment Rule 209 Children used space that was not approved. One infant child was being cared for in an unapproved space. GS 110-91(1)&(4-5) 820 A person(s) was smoking in a vehicle used to transport children or on the premises of the child care center. Upon walking outdoors there was a gentlemen standing outside on the sidewalk of the home smoking. This was corrected when the operator informed the gentlemen that when he is present at the facility that he can not smoke on the premise of the facility. .0604(h) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. During the monitoring of the outdoor space there was a pile of animal waste located in the area where the children play. This was corrected when one of the teacher went to remove the animal waste from the play area. 10A NCAC 09 .0604(p) 1756 Enhanced staff/child ratios and group sizes were not met. During the visit an infant was brought into the child care space form an unapproved space and grouped with children in space #1, who were 2years of age and older. This was corrected by the provider taking the infant back into the unapproved space until the parent was contacted to pick the infant up. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00pm on August 21, 2024 you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. The in-person event focuses on creating a safe and inclusive community for sharing and learning new concepts. This learning event is for directors and school-age staff only (newly hired and/or seasoned Group Leaders). The previous Annual Compliance Visit was completed February 8, 2024, prior to today’s visit, the eighteen-month compliance history score was at 99%, as required by North Carolina General Statute 110-90(4) (c-d). This program currently operates with a five-Star License issued May 23, 2023. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet an Educational Options: Completing 20 additional annual on-going training hours for full time lead teachers and teachers and staff working part-time. The information on record with DCDEE was verified as correct by Lora Fryar and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed May 15, 2024. Fire Classification of Approved Daytime Only completed May 3, 2024. Upon my arrival there were 10 children present with two teachers. Ms. Fryar was contacted, she stated that she was on her way. Ms. Lewis, another teacher and the daughter of Ms. Fryar who live in the home entered the child care space carrying an infant. When Ms. Lewis brought the infant from the home facility then became out of compliance with grouping of children. I asked who the child was and was the child one of Ms. Fryars foster children. Ms. Lewis stated no. I asked was the child enrolled in the facility Ms. Lewis stated yes. Ms. Fryar arrived at the facility.
