The previous annual compliance visit was completed on September 18, 2025, and prior to today’s visit the eighteen-month compliance history score was at 95% which is above 75%, required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued July 21, 2025. The License was based on the program earning five points in education standards, four points in program standards, and one quality point by choosing to meet an education option: the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Country Martin, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 11/6/2025. Fire Classification of daytime care only completed 8/28/2025 Ms. Martin was present while I observed the indoor and outdoor areas. In space #1 the children from space number #5 were being cared for until staff arrived. The children that are normally cared for in this space were being cared for in other spaces due to staff being out sick. The children were observed having breakfast. The children in #2, the children were observed participating in free choice play as they transitioned from breakfast. The children in space #4, were observed crawling and walking about space as they were playing with musical shakers, and handheld toys. While monitoring this space there were three bottles that had a date of 5/7/2026, and three bottles that did not have a date at all. The teacher shared that the bottles were brought today by the parents. The teacher dated the bottles for today’s date. The children that were in space #1, transitioned to space #5, and were observed participating in coloring and writing activities. The children in space #6, were observed participating in a review of the week’s lessons. Breakfast served was Chex cereal, peaches, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Martin. The following violation was observed: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In space #4, there where three bottles that were dated for 5/7/2026, and three bottles did not have a date at all. This was corrected by the teacher labeling the bottles with the correct date. 15A NCAC 18A .2804(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. TA: A violation was cited during today’s visit for not labeling bottles and sippy cups. Refer to child care rule 15A NCAC 18A .2804(d): We discussed the following strategies. Ensure that parents are aware of the labeling of bottles and sippy cups. Ensure that teachers are checking the items when parents return them to the facility each day. Rated License Assessment: On 2/12/2026 – During the routine unannounced visit. The New Pathways to the stars (New QRIS) was discussed with Ms. Martin. After discussion, a review of the following documents Family and Community Engagement, Continuous Quality Improvement and Professional Development Plan, Continuous Quality Improvement Plan facility, Staff Information and Education sheet, and Star Level Assessment form. Ms. Martin decided that facility would be completing Pathway #2 Classroom and Instructional Quality. Documents for the chosen pathway were left with Ms. Martin. An application for Rated License Assessment was received on 3/18/2026, from Ms. Martin, Administrator. The facility chose the Classroom and Instructional Quality Pathway. The following documents were reviewed and monitored for compliance today. Application for Assessment for a Rated License for Centers – The facility will meet Enhanced Staff/Child Ratios, Family and Community Engagement Standards Foundational Practices and meeting three additional options: C-1, EL-2, and EO-1. Facility Continuous Quality Improvement (CQI) Plan – Goal/Plan was established; reason for goal, timeframe to achieve goal, and steps to achieve goal were documented and available for review today. Individual Continuous Quality Improvement (CQI) Professional Development (PD) Plan (Child Care Center) – Individual PD plans were on file and available for review today. Goals were identified for each employee, time frame to achieve the goal, and steps to achieve the goal were documented on each plan. Staff Information and Education (Lead Teacher and other Educators) – LT star level is four (4), Other Educator Star level is four (4), and the final Star Level Education Awarded is four (4). The curriculum used is Early Learning Experience for children 0-5 years of age, Funnydaffe for School-age children. Formulative assessments being used for children 0-5 years of age is Early Learning Experience. Coaching, training, and formulative assessment will be monitored annually. During the facility next annual compliance visit. Based on the information provided, the facility may be eligible for a four (4) Star License with enhanced staff/child ratio. The completed packet will be submitted for review and processing. A new license will be mailed directly to you and must be posted upon receipt. You must mail the old license to me once the new one is received, as it remains the property of the State of North Carolina Reminders: Measles Cases: Measles is on the rise globally and in North Carolina. The following violations were observed: Violation Number Comment Rule 530 Each infant was not held for bottle feeding until he/she can hold his own bottle. In space #4, there was and infant child laying on his back drinking a bottle of milk. The previous Annual Compliance Visit was completed September 23, 2024 and prior to today’s visit, the eighteen-month compliance history score was at 90% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued April 13, 2022. The License was based on the program earning five (5) points in education standards, four (4) points in program standards, and one quality point by choosing to meet an education option: the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with the DCDEE was verified as correct by Country Martin, Administrator, and the owner corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 6/7/2024. Fire Approved for daytime and night care 8/28/2024. Ms. Martin was present while I observed the indoor and outdoor areas. Upon entering the facility children were observed playing with art materials, blocks, and dress-up clothes, Children were also observed participating in outdoor activities. In space number 4 there were two teachers with eleven children 0 to 1 year of age. Another teacher entered the space about 2 minutes after I did. Lunch served today was sausage, eggs, waffles, green beans, peaches, and milk. Observation was completed from 10:30am to 10:45am for the children in space #2. The children were observed participating in outdoor activities. They were seen playing with push toys, trucks, and bubbles, The teachers walked about the space while talking with the children and playing with the bubbles that were blown from a bubble machine that also played children songs. The children really enjoyed the bubbles. They ran about the space chasing the bubbles, laughing, and smiling while talking to the teachers. Observation was completed from 10:45 a.m. to 11a.m., in space #4. The children were observed walking and crawling about the space playing with soft toys, grasping toys, soft block, and musical shaking toys. Both teachers were holding a child and talking to the children while another teacher was cleaning up. While also assisting the children that were moving freely within the space. At the completion of the visit, the Visit Summary was reviewed with Ms. Martin. The following violation was observed. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. In space #4, there were two teachers with eleven children 0 to 1 year of age. This was corrected when the third teacher entered the space about 2 minutes after I did. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violation observed and documented today may impact the compliance history score. The violation was corrected during today’s visit. However, a follow-up visit is required within two weeks of today’s visit. TA: A violation was cited for Enhanced Staff Child Ratio. Refer to child care rule 10A NCAC 09 .2818. We discussed several strategies. Make sure staff understand the Staff Child Ratio Charts that are posted in the classroom. Make sure staff know who to contact when they have more children in the classroom then they should. Based on today’s classrooms observation, I discussed with Ms. Martin the positive interactions and various learning opportunities provided to the children throughout the visit. I encouraged Ms. Martin to share with the teachers to be flexible in the posted schedule based on the children’s interest and attention span. I encouraged Ms. Martin to share with the teachers to continue the positive interactions and conversations between them and the children and to keep encouraging conversation with the children. The previous Annual Compliance Visit was completed September 23, 2024 and prior to today’s visit, the eighteen-month compliance history score was at 90% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued April 13, 2022. The License was based on the program earning five (5) points in education standards, four (4) points in program standards, and one quality point by choosing to meet an education option: the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with the DCDEE was verified as correct by Country Martin, Administrator, and the owner corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 6/7/2024. Fire Approved for daytime and night care 8/28/2024. Ms. Martin was present while I observed the indoor and outdoor areas. Upon entering the facility children were observed playing with art materials, blocks, and dress-up clothes, Children were also observed participating in outdoor activities. In space number 4 there were two teachers with eleven children 0 to 1 year of age. Another teacher entered the space about 2 minutes after I did. Lunch served today was sausage, eggs, waffles, green beans, peaches, and milk. Observation was completed from 10:30am to 10:45am for the children in space #2. The children were observed participating in outdoor activities. They were seen playing with push toys, trucks, and bubbles, The teachers walked about the space while talking with the children and playing with the bubbles that were blown from a bubble machine that also played children songs. The children really enjoyed the bubbles. They ran about the space chasing the bubbles, laughing, and smiling while talking to the teachers. Observation was completed from 10:45 a.m. to 11a.m., in space #4. The children were observed walking and crawling about the space playing with soft toys, grasping toys, soft block, and musical shaking toys. Both teachers were holding a child and talking to the children while another teacher was cleaning up. While also assisting the children that were moving freely within the space. At the completion of the visit, the Visit Summary was reviewed with Ms. Martin. The following violation was observed. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. In space #4, there were two teachers with eleven children 0 to 1 year of age. This was corrected when the third teacher entered the space about 2 minutes after I did. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. However, a follow-up visit is required within two weeks of today’s visit. TA: A violation was cited for Enhanced Staff Child Ratio. Refer to child care rule 10A NCAC 09 .2818. We discussed several strategies. Make sure staff understand the Staff Child Ratio Charts that are posted in the classroom. Make sure staff know who to contact when they have more children in the classroom then they should. Based on today’s classrooms observation, I discussed with Ms. Martin the positive interactions and various learning opportunities provided to the children throughout the visit. I encouraged Ms. Martin to share with the teachers to be flexible in the posted schedule based on the children’s interest and attention span. I encouraged Ms. Martin to share with the teachers to continue the positive interactions and conversations between them and the children and to keep encouraging conversation with the children. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. The previous annual compliance visit was completed on October 2, 2022, and prior to today’s visit the eighteen-month compliance history score was at 98% which is above the 75%, required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued April 13, 2022. The License was based on the program earning five points in education standards, four points in program standards, and one quality point by choosing to meet an education option: the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Country Martin, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 8/31/2023. Fire Classification of daytime care only completed 8/29/2023. Ms. Martin was present while I observed the indoor and outdoor areas. In space #1, two children were observed sitting at a table playing uno. In space #2, five children, two years of age were observed dancing and watching music and movement songs on a television that hung on the wall in the classroom. In space #3, the children were sitting on a small carpet drawing the letters of the alphabet on a dry erase board using markers. In space #4, two infant children were observed laying in their cribs, one child was asleep with a pillow in the crib and one child had a blanket in his crib. Lunch served today was sliced hot dogs, hot dog buns, pears, tomato soup, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Martin. The following violations were observed: Violation Number Comment Rule 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. The bottles that were prepared did not have the date the bottles were prepared. This was corrected by the teacher adding the dates to the bottles. 15A NCAC 18A .2804(d) 544 Screen time was offered to children under three years of age. In space #2, there were five children two years of age watching videos on tv screen, while dancing to music and movement songs. This was corrected by the teacher covering up the tv screen while the children listen and danced to the songs. .0510(f) 871 Center staff did not comply with the safe sleep policy. In space #4, an infant was asleep with in a crib with a pillow. In another crib were a child was laying, there was a blanket in the crib with the child. This was corrected when the teacher removed all items out of the cribs. 10A NCAC 09 .0606(a) 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. In space #4, The safe sleep policy was not posted. This was corrected by the operator posting the safe sleep policy on the parent board in space #4. This was corrected when the teacher removed the items from the crib. .0606(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. All items were corrected during today's visit. No compliance letter is due. TA: A violation was cited during today’s visit for not following the safe sleep policy: Refer to Child Care Rule 10A NCAC 09 .0606(a). We discussed the following strategies: Ensure that the safe sleep policy has been reviewed with all safe. Monitor items that are being brought from home and ensure that parents aren’t putting items in their child’s crib. Reminders: Do you have children who bite? Do children struggle to focus during circle time? Do challenging behaviors cause difficult transitions? Do you spend all day managing behavior? Are you at your wits' end? The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. These rule changes may or may not impact your facility. The multi-age group staff/child ratio changes support a previous rule change submitted by Early Head Start and the new rule for defining cooperative arrangements provides clarification to a previous change in statute. A new July 2023 New Rule/Rule Amendments Overview module has been added to the Child Care Rules Training on DCDEE Moodle. If you are unfamiliar with the Child Care Rule Rollout within Moodle, learn how to navigate Moodle.
