Including health and safety training. The previous Annual Compliance Visit was completed November 21, 2024, and prior to today’s visit, the eighteen-month compliance history score was 88% which is above the 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued September 12, 2021. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet a programmatic option the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program. The information on record with DCDEE was verified as correct by Deborah Evans, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as Current-Active. Sanitation Classification of Superior completed 8/11/2025. Fire Classification of Approved for daytime and night care 8/11/2025. Ms. Smith, Lead Techer, greeted me at door. Deanna Evans entered the space and assisted me with the visit until Ms. Evans arrived. Children were observed participating in free choice play, bathroom routines, and large group time. Lunch served today was fish, sweet peas, apple sauce, sliced bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Evans. The following violation was observed: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In space #3/4, there was medicine emergency medicine stored five feet high with an expiration date of 2/28/2025. This was corrected by the administrator removing the medication from the classroom and contacting the parent provide a prescription with medicine before the child returns. 10A NCAC 09 .0803(1)(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Ensure all staff are monitoring the dates of the medication for the children that are in their care During today’s visit the following information was reviewed and forms left for Pathways #1 & Pathway #2 to the Stars. (New QRIS), Education Standards for all positions of each staff member, Staff Information and Education Worksheet, CQI Plan, Family and Community Engagement Standards, Self Study, the QRIS Pathways #1, and #2, Program Assessment document, and Classroom and instructional Quality for Centers, and Application for Assessment for a Rated License, and Rated License Assessment Request Review. Ms. Evans and l reviewed all documents. She stated that she is looking to complete pathway #2 for the facility. I shared with Ms. Evans that if she would like help with Pathway #2, she could reach out to me or apply with the Partnership for Children and request additional assistants. We discussed ensuring that all staff have submitted information for their Works account. I shared with her that she can reach out to me through email or via phone with any additional questions. The previous Annual Compliance Visit was completed November 21, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 92% which is above the 75%, as required by North Carolina General Statute 110-90(4)(c-d). Lunch served today was cheeseburgers, baked beans, mandarin oranges, sliced bread, and milk. In space # 3/4, an observation was completed from 9:05a.m. to 9:20a.m., The children were observed participating in large group time. The teacher reviewed colors, five senses, they sang songs, and discussed the small group activity they would be participating in today. The Children were engaged and enjoying what they were doing. The teacher allowed the children to make their own choices about what they wanted to do during the different activities. The children were moving about space so freely. In space# 2, an observation was completed from 9:25 a.m. to 9:40a.m., The children were observed participating in group time. They were repeating the teacher while she said a letter of the alphabet, while she blew bubbles. So, as the teacher blew the bubbles the children chased the bubbles and would crawl under the table, and then would repeat the letter at some point as the teacher keep asking them. This activity did not work for this age group. The staff and training worksheet were monitored for any update with CPR & First Aid, IT-SIDS training, and CBC. All the information was current from the previous annual compliance visit. Two new staff files were monitored. See attached staff and training worksheet. At the completion of the visit, the Visit Summary was reviewed with Ms. Evans. The following violations were observed: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One new staff did not complete First Aid training within 90 days. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One new staff did not complete CPR training within 90 days of hire. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on June 5, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. I conducted classroom observation in space #3/4 & 2., The in space #3/4, the teacher was engaged with the children and sharing all kinds of information about the art activity as they talked about what laundry day is and what happens during Landry day and washing of clothes. The children asked questions and shared what they already knew. In space #2, the teacher needs assistance with planning activities for the age group of children she cares for. The children were not engaged and were crawling under the table and just not interested in the activity after a while. I shared with the teacher in space #3/4, how great of a job she was doing with the children and to keep having the positive interaction that she was having with the children. I Shared with the administrator that the teacher in space #2, the teachers need some training and assistants on classroom management and instruction with the age group she work with. The previous Annual Compliance Visit was completed February 15, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 98% which is above the 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four - Star License issued September 12, 2021. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet a programmatic option education option in which the child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification Superior of completed 9/9/2023 Fire Classification of Approved for daytime and night care completed 8/23/2024. Deborah Evans, Administrator, was present while I observed the indoor and outdoor areas. The children were observed playing in their designated play areas. At the completion of the visit, the Visit Summary was reviewed with Ms. Evans. The following violations were observed. Children in Space #2 were participating in free choice and engaged in playing with large Lego blocks. The teacher sat on the carpet with them, singing and asking open-ended questions. She also comforted a crying child. In space #3, one teacher and six preschool-aged children were observed in morning group time discussing letters, colors, and shapes. The children helped set up the table for the next activity by bringing buckets of crayons and construction paper. The children colored a pineapple using colored crayons. Handwashing and toileting routines were also observed. At the completion of the visit, the Visit Summary was reviewed with D. McMillan Evans. The following violation was observed.
