The previous Annual Compliance Visit was completed February 12, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 96%. This program currently operates with a Four-Star License issued January 30, 2020. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an educational option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Erika Brown-Campbell, Administrator. Current Inspections are as follows: Sanitation Classification of Approved completed October 12, 2024. Fire Classification of Approved completed January 24, 2024. Ms. Brown-Campbell was present and assisted me while observing the indoor areas. The outdoor area was not monitored due to the weather restrictions. In Space #1, two children were sleeping in their cribs and two children were on the floor exploring their space. The caregiver held the child and talked to the children. The children were playing with ball sized shakers. In Space #4, the children were sitting on the carpet and the teacher was talking to the children and reading a book. In Space #3, the children were playing in the following activities: shapes, blocks, books, manipulatives, pretend play with baby dolls, food, and clothes. A violation was cited today for the electrical outlets. Toileting and handwashing routines were observed. Lunch served was hotdogs, mixed vegetables, wheat bread, peaches, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Latonya Cobb, Staff member. The following violation was observed: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Space #3, there was one electrical outlet that was not covered with a safety plug. This was corrected when the staff member covered the outlet with a safety plug. 10A NCAC 09 .0604(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. In Space #1, three children were on the floor playing with musical shakers and two children were sitting in their infant seats. Toys were open and available for the children. A violation was cited for hand soap. In Space #5, the children were playing outdoors. Outside the children were playing with sand and trucks. Lunch served was beef and cheeses noodles, peas, pears, milk. On June 12, 2024, a violation was cited for enhanced staff/child ratios. The violation was corrected during the visit; however, the visit required a follow-up visit. The facility is in compliance with the enhanced staff/child ratios. At the completion of the visit, the Visit Summary was reviewed with Ms. Wright. The following violation was observed: Violation Number Comment Rule 606 Running water, soap and individual sanitary towels, or other approved hand-drying devices were not supplied at each lavatory. In Space #1, in the soap bottle, it contained hand sanitizer and not hand soap. This was corrected when the administrator added liquid soap in the soap dispenser. 15A NCAC 18A .2818(b) & (d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A compliance letter is not required. Facility Specific TA/Consultation/Reminders: Hand Soap: Technical assistance was provided today for your hand soap. After washing my hands, I noticed the consistency contained hand sanitizer instead of soap. The administrator stated that a staff member refilled the bottles and did not add the correct substance. Hand sanitizer is allowed when water and soap are not available during emergencies. In the classroom, hand sanitizer should not replace hand soap. Toileting and handwashing routines were observed. Additional violations were cited for attendance and staff file requirements. Lunch served was peanut butter sandwich, green beans, apple sauce, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Cobb. The following violations were observed: Violation Number Comment Rule 606 Running water, soap and individual sanitary towels, or other approved hand-drying devices were not supplied at each lavatory. The soap that was used in Space #1, contained more water substance. This was corrected during consultant visit on June 20, 2024. The administrator added soap in the soap dispenser. 15A NCAC 18A .2818(b) & (d) 807 A safe indoor and outdoor environment was not provided for the children. In Space #3, there were two sharp edges parallel to the door handle to the main entrance to the classroom. The sharp edges were present due to the missing blinds for the window of the door. The previous Annual Compliance Visit was completed February 14, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 96%. This program currently operates with a Four-Star License issued January 30, 2020. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an educational option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Angela Wright, Administrator. Current Inspections are as follows: Sanitation Classification of Approved completed March 27, 2023. Fire Classification of Approved completed January 30, 2023. Ms. Wright was present and accompanied me while observing the indoor areas. In Space #1, two infants were sleeping in their cribs and the caregiver was holding one infant. In Space #2, the children cleaned up their toys and transition outdoors to play. Outside the children pushed cars. In Space #5, a group of children were sitting on the floor building with blocks and a group of children were sitting at the table playing with manipulatives. Lunch served was meatballs, noodles, mixed vegetables, peaches, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Wright. The following violations were observed: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. There was one staff member, (B.C.) that did not have a medical report completed prior to employment. The staff member's date of hire was July 24, 2023. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. There was one staff member, (B.C.) that did not have a TB test completed prior to employment. The staff member's date of hire was July 24, 2023. .0701(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on August 21, 2023, you must submit a Mail or email a written, dated, and signed statement to me by date (14 days from today)., The statement must describe accurately and in detail, describing how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information.
