The previous annual compliance visit was completed July 7, 2025, and prior to today’s visit, the eighteen-month compliance history score was 96%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License issued September 30, 2025. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet an education option with The child care administrator has at least 10 years of documented early childhood work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Jamilah Ashton, Assistant, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed November 18, 2025. Fire Classification of Approved for daytime care only completed April 1, 2025. Sandra Johnson, Child Care Consultant assisted me during today's visit. NC Pre-K was not monitored during the annual compliance visit on 7/7/2025, due to children already being dismissed for the school year. NC Pre-K requirements were monitored for space #3 during today’s visit. Ms. Jamilah was present while we observed the indoor and outdoor areas. upon our arrival children were observed having breakfast. The breakfast served today was grits, apple slices, and milk. Observation was completed in space ,# 1 children were engaged in classroom activities and responsive to the teachers. There were ample materials in each interest center for children to use at their choice. The teachers were always aware of their surroundings. Diaper changing routines were also observed during the visit. To assist with handwashing the teacher sang a song “over under” with the children. Observation was completed in space, # 2 Diapering and toileting routines were observed before transitioning to outdoor play. During outdoor play, the children slid down the slide, teachers were in reach to assist children if needed. Observation was completed in space #4, children were observed sitting at a table participating in an art activity with their teacher. The teacher used white finger paint so the children could use their hand to paint a melted snowman.. She asked the children, “Did they see any snow ? Did they build a snowman”. Next, the teacher assisted the children with handwashing. Observation was completed in space #5, the teacher discussed the agenda for the remaining of the day with children on the carpet. The teacher let the children know the expectations were in preparation for lunch. Another teacher assisted children with the rest room. Hand washing and toileting routines were observed. At the completion of the visit ,the visit summary was reviewed with Ms. Ashton. The following violations was observed Violation Number Comment Rule 1769 The health assessment did not include a dental screening. Three children files were monitored during today's visit, one child did not have a dental screening on file for review. .3005 (a)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today must be corrected immediately. Additionally, by 5:00 p.m. on February 11, 2026, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violation was corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. Provide a balance of adult and child-directed activities where children can make guided decisions based on teacher’s clearly identified expectations (example, where to sit at circle time). Plan routines and experiences that allow children to be successful with little support. Pay attention to children and notice their cues and assist with difficulties and need assistance. The children present during today’s visit will seek support and interact with staff. A violation was cited, for dental screening not being on file within 30 day of entering the facility. Refer to NC Child Care Rule 10A NCAC 09 .3005(a)(5). We discussed the following strategies: Ensure that you are using the checklist from the website that lists the requirements for children record. Ensure that you are giving the parents a due date for the return of all required documents. QRIS: During today’s visit the following information was reviewed and forms left for the Pathways to the Stars. (New QRIS), Education Standards for all positions of each staff member, Staff Information and Education Worksheet, the QRIS Pathways #2, Program Assessment document, and Classroom and instructional Quality for Centers, and Application for Assessment for a Rated License, and Rated License Assessment Request Review. Ms. Jamilah and Ms. Crosby and l reviewed all documents. She stated that she is looking to complete pathway #1 for the facility. I shared with Ms. Jamilah and Ms. Crosby that if she would like help with Pathway 2, she could reach out to me or apply with the Partnership for Children and request additional assistants. We discussed ensuring that all staff have submitted information for their Works account. I shared with her that she can reach out to me through email or via phone with any additional questions. I also shared with her that she can also reach out to NCRLAP. Reminders : The DCDEE has a new initiative – Natural Outdoor Learning Initiative. Through this initiative, we hope to increase knowledge and confidence for everyone in developing and implementing best practices to support the creation, programming, and safe use of naturalized outdoor spaces in child care facilities. During the observation one child was seen walking about space trying all the different toys, such as musical toys, soft books, and stacking blocks. One teacher was observed changing a child’s diaper before allowing the child to transition to free choice play. Both teachers were observed sitting on a carpet with all four children while one teacher read a story to the children. In space #3, an observation was conducted from 9:10am to 9:30am. During the observation some children were participating in bathroom routines while others have already transitioned to large group time. During the transition children were participating in music and movement activities while allowing all the children to finish bathroom routines. Once all children were on the large carpet the teacher then had the children sit on carpet while doing a review of the Letter, color, number, and shape for the week. They then sang Happy Birthday to a child whose birthday was today. They also greeted each other with a song, after the song the children then transitioned to free choice play. Four staff files were reviewed during the visit, see attached staff and training worksheet. The previous staff and training worksheet from the last annual compliance visit was monitored for any updates. Updates were needed and were verified during today’s visit. The previous annual compliance visit was completed 7/18/2023, and prior to today’s visit, the eighteen-month compliance history score was at 98%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License issued May 15, 2024. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet an education option with The child care administrator has at least 10 years of documented early childhood work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Angela Crosby and the owner’s corporation status was verified on the Secretary of State’s website as current-active. The children were observed playing with musical shakers, softy books, and stacking blocks. In space #2, the children in line getting ready for outdoors play. In space #3, the children were participating in bathroom routines and preparing for a field trip. In spaces #4, and #5, the children were participating in outdoor play. They were seen playing with balls, hula hoops, and drawing on art boards that hung from the fence. They also played on the large play equipment. Lunch served today was chicken, fires, mandarin oranges, bread, and milk. Transportation was monitored during the visit. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff has not renewed first aid training with in the required time. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff have not renewed CPR training on or before the expiration date. .1102(d) 1881 Over-the-counter medication was given which exceeded the amount and frequency of the dosage on the manufacturer's label. In space number #1, there was a 8oz bottle of liquid pain reliver, one .18oz tub of baby orajel, and one 13oz jar of Vaseline store in the classroom with no permission slip and instruction on how it should be administered. This was corrected by the administrator removing the items from classroom and discarding the items. .0803(4)(c ) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. They also sang songs like “Old McDonald had a Farm”. The teacher was encouraging the children to talk and to repeat words after them. The children played together very well. The interaction with the teacher and children was great. The children were smiling and laughing with the teacher as they played and talked about what they were doing. I encourage the teachers to keep having conversation with the children as it would enhance their vocabulary. I conducted an observation from 9::40a.m. to 10a.m.: in space #5, the children were engaged in free choice activities. The teachers walked about the space and talked with the children about what they were doing. They shared complements to the children as they observed the children’s artwork and as they played with one another. The teachers were encouraging to the children when in play, trying to get them to build things with blocks and making things with sand toys while playing in the sand table. The interaction with the teacher and children was great. The children were smiling and laughing with the teacher as they played and talked about what they were doing.
