The previous Annual Compliance Visit was completed August 13, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 91%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued December 13, 2019. The License was based on the program earning four (4) points in education standards, two (2) points in program standards, and one quality point by choosing to meet a programmatic option by using staff benefits and infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Barsheem Chapman and the owner’s corporation, Chapmans Management Company, status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed February 26, 2024. Fire Classification of approved completed October 9, 2025. The inspection was not completed on a form provided by the Division. Upon my arrival, I was greeted by Betsy Murphy, Teacher. She accompanied me while observing the indoor and outdoor areas. Barsheem Chapman, Administrator/Owner, arrived during the visit. There were thirteen children in care. Children in spaces #1 were sitting at the table playing a game of color and shape Bingo. The teacher gave instructions on how to play the game. She called out colored shapes like, “white circle and brown oval”. She reminded a child that she had Bingo and asked her to remove the covers and name the colored shape. In space #3, children were sitting at a table playing with alphabet blocks and assorted animals. A teacher asked a child about what he was making, and he responded, “A Dinosaur”. The teacher told the child that he loves dinosaurs. I observed children in space #4, singing, “Wheels on the bus” and “The Itsy Bitsy Spider”. I observed hand washing, sanitation, and lunch. Today’s lunch consisted of chicken tenders, green beans, mixed fruit, and milk. At the completion of the visit, the Visit Summary was reviewed with B. Chapman. The following violations were observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The operator did not submit the original fire inspection to DCDEE within one week. The inspection visit documentation was not on a form provided by the Division. 10A NCAC 09 .0304(a) 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. Food Substitution was not recorded on the menus prior to lunch being served. Menus located in the foyer and all classrooms were notated with a meat and vegetable substitution. 10A NCAC 09 .0901(b) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was a powerstrip stored on a teacher's desk without outlet covers in space #1. The powerstrip was removed from the classroom. 10A NCAC 09 .0604(c) The violations must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on February 26, 2025, describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. You stated that you are currently working on NAEYC Accreditation and are interested in changing a bathroom to a classroom for infants. Observation Suggestions: To stimulate emotional development the activities would give the children the opportunity to learn about different feelings and how to express them in an appropriate manner. For example, in the dramatic play center have a play telephone and encourage the children to talk on the phone. In the book/reading center you could have pictures of people showing different emotions such as smiling, laughing, frowning, and crying. Encourage the children to talk about the people and their own experiences. Ask open-ended questions, "What makes you laugh?" “What makes you sad?” To stimulate Intellectual activities that encourage children to be creative and to use their imaginations. In the discovery center you could have items that make different sounds: an alarm clock, bells. Have an eye chart for children. Talk about the reasons some people need glasses. Self-directed art projects also promote intellectual stimulus. Play pretend games with the children. “My Oh My!”: The teacher calls out “My Oh My” I see something that you don’t see, and the color is _____.” “Symbol Reading”: Have the children sit in a circle on the floor. Tell them they are going to read something and do the action. The first child draws out a card which has a picture of an action. The child “reads” the card and the performs the action. For example, the card might show a child walking on tiptoes. Or the card could show a child touching his toes. Physical development are activities that promote small muscle and large muscle development, such as crawling, dancing, running outside, hopping on one foot, stringing large beads, and using paint brushes. To meet the requirement of physical development with the theme communication, when outside, you could play "Stop Light" by using your hands as signals instead of your voice. Start by using your voice and demonstrating the game; then add the hand signals. You can then play the game using only your hand signals. Children were given a drink of water. A child in space #5 was observed sitting on their teacher’s lap. Next, the child sat and crawled on the floor. The teacher used circles and shapes and asked the child, “Are you going to help me?” I observed toileting, hand washing, sanitation, and lunch. Lunch consisted of pepperoni pizza, corn, green beans, and milk. At the completion of the visit, the Visit Summary was reviewed with B. Chapman. The following violations were observed: Violation Number Comment Rule 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. A first aid kit and fire extinguisher was not in a vehicle that transports children. The administrator placed a first aid kit and fire extinguisher in the vehicle that transport children. 10A NCAC 09 .1003(c) 1314 Emergency information did not name childs health care professional. Two children's application did not include the name and contact number of child's health care provider .0802(c)(2) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. A signed acknowledgement form of receipt of the facility's discipline policy was not on file for one child. .1804(c) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. A parent's signed acknowledgement form was not on file for one child. .0608(b)(1-6) The violations must be corrected immediately. Shaken Baby Syndrome and Abusive Head Trauma Policy: A violation was cited for one child not having a parent signed acknowledgement form on file. SBS/AHT is the name given to a form of physical child abuse that occurs when an infant or small child is violently shaken and/or there is trauma to the head. Shaking may last only a few seconds but can result in severe injury or even death. According to NC Child Care Rule 10A NCAC 09.0608, each child care facility licensed to care for children up to five years of age shall develop and adopt a policy to prevent SBS/AHT. This policy must be provided to parents and staff working at the facility and an acknowledgement signed by both parents and staff verifying receipt of the policy. AHT can occur in children up to 5 years of age. Shaken baby syndrome can lead to serious conditions including brain damage, problems with memory and attention, blindness, or hearing loss; intellectual, speech or learning disabilities; and developmental delays. This program currently operates with a Three-Star License issued December 13, 2019. The License was based on the program earning four (4) points in education standards, two (2) points in program standards, and one quality point by choosing to meet a programmatic option by using staff benefits and infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Barsheem Chapman and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Barsheem Chapman, Administrator/Owner, was present and assisted me while observing the indoor and outdoor areas. Upon my arrival, there were twenty-three children in care. Children in spaces #1, #2, #3, and #4 were observed outdoors sitting, running, digging in the sand, riding bikes, bouncing balls, going up the stairs and down the slide, and clean-up time. Children were given a drink of water. A child in space #5 was observed sitting on their teacher’s lap. Next, the child sat and crawled on the floor. The teacher used circles and shapes and asked the child, “Are you going to help me?” I observed toileting, hand washing, sanitation, and lunch. Lunch consisted of pepperoni pizza, corn, green beans, and milk. At the completion of the visit, the Visit Summary was reviewed with B. Chapman. The following violations were observed: Violation Number Comment Rule 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. The allegations were discussed with the administrator. The administrator and three teachers were interviewed, and information was gathered regarding the report. The Parent handbook, discipline policy, children and staff files were reviewed. Teachers and children mentioned in the report were observed interacting positively with each other. Children in the classrooms appeared comfortable interacting with their teachers and participating in activities. During interviews, it was stated that there were concerns for two siblings regarding their behaviors, language, and eating habits. It was recommended to the children’s parent to have both children evaluated by a physician. The children have been sent home because of behaviors (hurting other children, or to prevent them from hurting themselves and others when having tantrums). Steps are used for positive discipline. Teachers describe how discipline is used when warranted. Children are redirected, some behaviors are ignored (some tantrums), and last resort children are placed in time-out according to their age (example, 3-year-olds for 3 minutes). There are discussions with children regarding unacceptable behaviors. Teachers speak with children about behaviors that are unsafe and may hurt themselves and others. Eye contact is used, and children are given praise and stickers for good behavior. During observations, interviews, and information gathered, it was determined that I could not confirm any allegations were not incompliance. There were twenty-four children in care. Children participated in group activities, played with blocks, duplos, animals, and more. Positive communication was observed. For example, a child was stacking blocks and a teacher asked, “What’s going to happen when it gets to tall? Be careful.” Outdoors, children were running, riding bicycles, sitting at picnic benches in a shaded area. Children were offered and given a drink of water outdoors and indoors. I observed toileting, handwashing, diaper changing, sanitation, lunch and rest time. A current menu was not posted. Lunch consisted of corn dogs, carrots, mixed fruit, and milk. The allegations were discussed with the administrator. The administrator and three teachers were interviewed, and information was gathered regarding the report. The Parent handbook, discipline policy, children and staff files were reviewed. Teachers and children mentioned in the report were observed interacting positively with each other. Children in the classrooms appeared comfortable interacting with their teachers and participating in activities. During interviews, it was stated that there were concerns for two siblings regarding their behaviors, language, and eating habits. It was recommended to the children’s parent to have both children evaluated by a physician. The children have been sent home because of behaviors (hurting other children, or to prevent them from hurting themselves and others when having tantrums). Steps are used for positive discipline. Teachers describe how discipline is used when warranted. Children are redirected, some behaviors are ignored (some tantrums), and last resort children are placed in time-out according to their age (example, 3-year-olds for 3 minutes). There are discussions with children regarding unacceptable behaviors. Teachers speak with children about behaviors that are unsafe and may hurt themselves and others. Eye contact is used, and children are given praise and stickers for good behavior. During observations, interviews, and information gathered, it was determined that I could not confirm any allegations were not incompliance. Therefore, based on the information gathered and observations made, the allegation of violations of child care requirements pertaining to staff not interacting with children in a nurturing and caring manner is unsubstantiated. There were fifteen children in care. A. Williams opened the door for me and left seven children in space #1, by themselves, with the door closed. In space #3, I observed one teacher and seven children, ages one through two years old. Children participated in various activities like letter recognition, using large trucks, building with Legos, and playing with a stuffed animal and rattle. I observed toileting, handwashing, sanitation, lunch and rest time. Lunch consisted of corn dogs, baked beans, peaches, and milk. The allegation was discussed with the owner B. Chapman. Interviews, and information was gathered regarding the report. Attendance records, meal count, monthly menus, allergies list, sign-in/sign-out information (Brightwheel). During a phone interview and at the facility, it was determined that I could not confirm any allegations were out of compliance without names and dates. Based on the information gathered and observations made, the allegation of violations of child care requirements pertaining to a child being served food items with a known allergy is unsubstantiated. Based on the information gathered and observations made, the allegation of violations of child care requirements pertaining to accurate attendance records are not being taken is unsubstantiated. Based on the information gathered and observations made, the allegation of violations of child care requirements pertaining to the menu not being followed is unsubstantiated. Additional violations were cited regarding supervision, staff/child ratios, not having a qualified administrator, and new staff paperwork. Due to technical issues with Regulatory and printing a typed hand written visit summary, the complaint visit outcomes were reviewed and a hand written visit summary was signed and a copy was given to B. Chapman. Upon completion of the Regulatory Visit Summary, it will be emailed and mailed to you. The following violations were observed and documented. WORKS was checked, and you currently qualify as an Administrator I. Adequate Supervision: During today's visit a violation was cited for children not being adequately supervised. To ensure a health and safety environment refer to child care rule reference 10A NCAC .09 1801(a)(1-5). We discussed several strategies to help maintain compliance with adequate supervision regarding leaving children in the classroom with the door closed when answering the door. If the teacher must open the front entrance for someone, she should take the children with her or leave the door open so that she is able to hear them. You stated that you were short of staff and a new employee started employment during the visit. The violation was corrected during the visit. However, I will return sometime within two weeks to verify the violation remain corrected. Staff/Child Ratios: A violation was documented for failure to maintain compliance with staff/child ratio requirements. Violations of this nature directly impact the safety of the children while in your care. In addition, these types of violations have the greatest negative impact on your compliance history. During the visit, we discussed that repeated violations of this nature during consecutive visits may lead to an administrative action against your license. This violation was corrected during the visit. A teacher took three children, one-year of age to space #4. However, a follow-up visit will be conducted in the near future to monitor compliance with child care requirements. Medical Statement: A violation was cited for a new staff member (F.R.) having a medical statement older than one year after the first day of employment (2/1/24). During a phone interview and at the facility, it was determined that I could not confirm any allegations were out of compliance without names and dates. The eighteen-month compliance history score was 90%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued December 13, 2019. The License was based on the program earning four (4) points in education standards, two (2) points in program standards, and one (1) quality point by choosing to meet a programmatic option by using staff benefits and infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Barsheem Chapman and the owner’s corporation status was verified on the Secretary of State’s website as current-active. The facility was issued a WW dated June 30, 2023. Upon my arrival, I was greeted by Arethea Williams, Teacher. Barsheem Chapman, Owner, arrived during the visit. There were eighteen children in care. Children in space #1, were sitting at tables participating in a group activity using individual dry erase boards and markers. The teacher demonstrated how to make a circle. Each child drew their own circle. The teacher said phrases like, “I want my circle to smile” and the children drew a mouth. In space #5, children were walking, crawling and laying on their back. The teacher talked to the children about what they were doing. In space #3, there were nine children ages one through three years old, with one teacher and there was one child, three years of age, grouped with four children, one year of age. The children were sitting on the carpet preparing for a group activity. According to today’s menu lunch will be consisted of fish sticks, wheat bread, peas/carrots, pears and milk. At the completion of the visit, the visit summary was reviewed with B. Chapman. The following violation was observed. Children in space #1, were sitting at tables drawing pictures using paper, colored pencils and markers. I observed toileting, handwashing, and preparation for outdoors. Once outdoors, they ran, rode scooters, used a football and basketball, and dug in the sand. Three children in space #5, were sitting outdoors on a soft blanket playing with developmentally age appropriate materials like, a soft ball, animals, and dolls. They were also observed walking and crawling. Children in space #3, were observed on the playground, running, riding scooters and sitting on a picnic bench drinking water. I also observed, diaper changing, hand washing, sanitation, lunch, and rest time. Today’s lunch consisted of chicken nuggets, mixed vegetables, bread, peaches, and milk. At the completion of the visit, the visit summary was reviewed with B. Chapman. The following violations were observed.
