The previous Annual Compliance Visit was completed June 18, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 95% which is above 75%, required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a three-Star License issued August 27, 2019. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet an education option. The child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Janice Beatty, Administrator. Current Inspections are as follows: Sanitation Classification of Superior completed 6/4/2025. Fire Classification of Approved for daytime and nighttime care completed 12/1/2025. J. Beatty, Administrator, was present while I monitored the indoor and outdoor areas. Upon my arrival there were 7 children present ages, one (1) to seven (7) years, with one staff member J. Beatty. The children were observed participating in free choice activities. Children participated in drawing and writing activities. They were observed playing with puzzles, and blocks. The lunch served today was bologna, mixed fruit, sweet peas, crackers, and milk. A checklist was used to note the requirements I monitored during today’s visit. The Staff and Training Worksheet was received and monitored during today’s visit. Children’s Records were monitored. At the completion of the visit, the Visit Summary was reviewed with J. Beatty. The following violations were observed. Violation Number Comment Rule 315 When combining age groups, the staff/child ratios for the youngest child in the group was not maintained for the entire group. In space #2, the group of children present age range was one (1) to seven (7) years of age. This was corrected when the children one(1) and two years of age were moved to space #1. .0713(a)(1) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #1, there was a bottle of medication, "Cetinize Hydrochloride Oral Solutions". This was corrected by the provider locking away the medication and returning it to the parent at the end of the day. 15A NCAC 18A .2820(d) 1756 Enhanced staff/child ratios and group sizes were not met. In space #2, there were seven children present age 1 to 5, with one teacher. This was corrected when an additional teacher arrived and transitioned the children one (1) and two years of age into space #1. 10A NCAC 09 .2818 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility did not have the required roster on file and available for review. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on June 11, 2026, you must submit a written, dated, and signed statement to me. The statement must be described accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. Children were observed playing with push toys, soft dolls, and books. The teacher was sitting on the floor with the children while they moved about the space and assisted the children. She was talking to the children about colors and different shapes of the toys. The children were engaged and smiling and trying to talk with the teacher. At the completion of the visit, the Visit Summary was reviewed with Ms. Beatty. The following violation was observed. The previous Annual Compliance Visit was completed June 26, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 87% which is above 75%, required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a three-Star License issued August 27, 2019. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet an education option. The child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Janice Beatty, Administrator. Current Inspections are as follows: Sanitation Classification of Superior completed 6/4/2025. Fire Classification of Approved for daytime and nighttime care completed 12/2/2024. Ms. Beatty, Administrator, was present while observed the indoor and outdoor areas. Upon my arrival there were 7 children present. Ms. Beatty stated that she would be closing early due to the AC going out in the facility. Children were observed participating in free choice activities. Children were observed drawing, writing, playing with blocks, soft toys. The lunch served today was going to be peanut butter sandwiches, mixed vegetables, peaches, sliced bread, and milk. A checklist was used to note the requirements I monitored during today’s visit. The Staff and Training Worksheet was received through email on 6/12/2025. Children’s Records were monitored. During today’s visit a volunteer was present and her file was monitored. The TB screening was not on file. At the completion of the visit, the Visit Summary was reviewed with Ms. Beatty. The following violations were observed. Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A volunteer did not have a TB on file prior to first day of volunteering. .0701(a) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child did not have medical exam on file within 30 days of enrollment. GS110-91(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on July 2, 2025, you must submit a written, dated, and signed statement to me. A shelter-in-place or lockdown drill had not been completed since 9/12/2024. This was corrected with the operator conducted a shelter-in-place drill during the visit today. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Refer to child care rule 10A NCAC 09 .0304 (a). We discussed the following strategy. Use a dry erase board or calendar to keep track of all inspections on a dry erase board or calendar. I shared with Ms. Beatty that there was a lot of positive interaction with the children. However there needs to be more conversations instead of directives going on during free choice play. I encouraged the teacher to keep providing positive interaction with the child and to provide more conversation with the children. Self-talk and Parallel talk will both be necessary in providing a language enriching environment. Self-talk offers an opportunity to allow conversation to occur. For example, the teacher can talk out loud about what she/he is doing, seeing, hearing, or feeling. Parallel talk is important for the teacher to engage in conversations by talking about what the child is doing, seeing, hearing, or feeling. Lunch served today was Chicken and cheese tortillas, corn, apple slices, cookie, and milk. A checklist was used to note the requirements I monitored during today’s visit. The Staff and Training Worksheet was received during today’s visit. Children’s Records were monitored. At the completion of the visit, the Visit Summary was reviewed with Ms. Beatty. The following violations were observed. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted in space #2 was dated June 17 - June 21, 2024. This was corrected by the teacher update the activity plan during the visit. GS 110-91(12); .0508(a) 530 Each infant was not held for bottle feeding until he/she can hold his own bottle. In space #1, one infant was placed in his crib with his bottle to fed himself. This was corrected by the teacher removing the infant from the crib and place in a high chair to drink his bottle. 10A NCAC 09 .0902(b) 544 Screen time was offered to children under three years of age. In space #2, four children 2 years of age were allowed to have scree time. This was corrected by the teacher turning the Television off. .0510(f) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #1, there was an 19oz. aerosol can of Lysol spray sitting on a shelf that was not stored in a locked room or cabinet. This was corrected by the teacher locking the aerosol can in a locked closet. .2820(b) 846 Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. In Space #1, there where five different types of diaper cream, three with a child's name and one tub of teething gel. Four of the creams and the teething gel did not have a authorization form in witting by parent or physician. This was corrected by the teacher dis carding the creams and teething gel. 10A NCAC 09 .0803(4) 871 Center staff did not comply with the safe sleep policy. In space #1, One infant was laying in his crib with a bottle and two blankets. This was corrected by the teacher removing the blankets and the bottle from the child's crib. 10A NCAC 09 .0606(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One volunteer did not update the required emergency information annually. This was corrected by the volunteer updating the information today. .0701(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. On staff who is required to complete 10 on-going training hours annually only completed five hours of training. .1103(a) 1756 Enhanced staff/child ratios and group sizes were not met. In space #2, One staff member cared for 13 children ages, 2years to 9 years of age. While I was completing enrollment two other children arrived which then made the space out of compliance with enhanced staff/child ratio. Lunch served today was Chicken and cheese tortillas, corn, apple slices, cookie, and milk. A checklist was used to note the requirements I monitored during today’s visit. The Staff and Training Worksheet was received during today’s visit. Children’s Records were monitored. At the completion of the visit, the Visit Summary was reviewed with Ms. Beatty. Reminders: Emergency Preparedness and Response Drills and Plan (EPR): Ensure each year to go online into your portal and update your EPR plan. Drills are to be completed quarterly and the Ready to Go File and Kit available and ready in case of an emergency. Review the EPR plan with all staff at least annually. QRIS Modernization (Two–Five Star Rated Licenses): The North Carolina Child Care Commission and the NCDHHS Division of Child Development and Early Education (DCDEE) are collaborating to modernize North Carolina's Quality Rating and Improvement System (QRIS) commonly known as the Star Rated License. DCDEE are developing a plan that will be reviewed by the legislature in Spring 2024. During the next few months, we are collecting information from parents, teachers, administrators, operators, and partners to consider in the plan. You are invited to participate in opportunities for Commission members and Division staff to listen and learn about your experiences and challenges within our Star Rated License system.
