Name of Operation: HEAVENLY HAVEN CHILD DEVELOPMENT CENTER II Facility ID: 26002143 Consultant: RONA DAVIS Operation Type: Center Case Number: Visit Date: 1/7/2026 Number Present: 32 Completed Date: 1/7/2026 Age: From 1 To 5 Total Minutes: 330 Time In: 09:00 AM Time Out: 02:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements and the NC Pre-K program using Child Care Rules Section .3000 pertinent to an Annual Compliance visit. The previous Annual Compliance Visit was completed January 14, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 91%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued August 25, 2020. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by having 75% of teachers with an AAS or higher in ECE/CD. The information in Regulatory was verified correct by Kimberley Baker and the owner’s corporation status, Heavenly Haven Child Development Center, Inc, was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed July 15, 2025. Fire Classification of approved completed October 17, 2025. I was greeted by Marcella Parker, Administrator. Kimberly Baker was present during the visit. She assisted me as I observed the indoor and outdoor areas. There were thirty-two (32) children in care. Children in space #6, were sitting on the floor with their teacher playing with materials like large cardboard blocks, animals, and cars. Next, they cleaned up and prepared for outdoors. In space #3, children were participating in a group activity, listening to their teacher read a book. Parents and children were greeted by teachers upon entering the classroom. Children in space #4, were sitting at tables discussing classroom helpers. Each child had individual draw-erase boards. They were given instructions to draw a picture of the classroom helper they would like to be. Children in space #1 were observed playing outdoors. They were running and riding bikes. They were observed playing tag. I observed toileting, hand washing, sanitation, lunch, and rest time. Today’s lunch consisted of chicken nuggets, macaroni/cheese, peaches, and milk. Ten percent of children and staff files were checked during the visit. NC Pre-K children files were included in a random selection. NCPre-K Administrator: Donna Pyles (off-site) NC Pre-K (space #3) Teacher: Patricia Dilworth (present) TA: K Karsh (present) NC Pre-K (space #4) Teacher (substitute): Jovita Lendor (present) TA: Sylvia Taylor (present) Transportation was monitored during the visit: Ford E350 Passenger Van – JBX-XXXX Registration/Inspection expires - 2/28/26 Insurance – 9/15/25 – 3/15/26 At the completion of the visit, the Visit Summary was reviewed with K. Baker. The following violations were observed: Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One new employee did not have documentation on file that verified the completion of six weeks of orientation. .1101(a) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. One employee did not have documentation on file that verified the completion of their first two weeks of orientation. .1101(a)(b) 1123 All vehicles used to transport children were not free of hazards. There was exposed foam on the arm of the first-row seat of a vehicle that transports children. The exposed foam was covered with duct tape. 10A NCAC 09 .1002(a) The violations must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on January 21, 2026 describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation, the facility will pursue the Classroom and Instructional Quality pathway 2. We discussed: A violation cited for one staff member not having documentation on file to verify the completion of their first 2 weeks and 6 weeks of orientation. Remember new staff must complete orientation and it must be documented within the required time of their first day of employment. Refer to child care rule .1101(a)(b) A violation cited for a vehicle that transported children having exposed foam on the arm of the first-row passenger seat. Vehicles that transport children shall always be in good repair. This was corrected during the visit. The exposed foam was covered with duct tape. Refer to child care rule .1002(a). Reminders: The vehicle that transports children registration expires 2/28/26. Required information for four staff/members Criminal Background Check qualifying letter expires in March 2026. Submit required information as soon as possible so that staff members can have an updated letter before the current letter expires. School age children in space #4, were observed sitting at a table participating in making airplanes. The used colored construction paper, crayons, and pencils. One child explained to another child how to make an airplane. One school aged child was drawing, and another was writing, using line paper. In space #3, children were rolling a ball and using tablets. One child was very upset. The teacher quickly went to the child and calmed him down by talking to him and finding out why he was upset. The child calmed down and continued playing. In space #2, children were sitting at a table participating in an art activity, making a school bus. They used pre-cut shapes (bus, circles, triangles) and glue. Another child was observed sitting on the carpet playing with dinosaurs and zoo animals. In space #1, children were observed outdoors. They used large sidewalk chalk and rode tricycles. They used yellow, green, and blue chalk. Next, they lined up to go indoors. Once indoors, they washed their hands. I observed, toileting, hand washing, sanitation, lunch, and rest time. Lunch consisted of chicken nuggets, broccoli, mandarin oranges, and milk. At the completion of the visit, the Visit Summary was reviewed with K. Baker. The following violations were observed: Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Documentation on a cumulative log or activity plan was not available for review for four children observed using tablets in space #3. A screen time log was completed during the visit. .0510(d)(2)(A-C) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. There were numerous areas of peeling paint and exposed sheetrock on all four top half of the walls in space #2. 15A NCAC 18A .2825(a) 721 All equipment and furnishings were not in good repair. For example, when children are waiting; play a game, ask questions, or sing/chant songs. Let the children share how their school day went. When children are waiting for long periods of time during transitions, typically some may have to be redirected, or positive discipline is needed. Provide a balance of adult and child-directed activities where children can make guided decisions based on the teacher’s clearly identified expectations (example, where to sit at a table). Give at least a five-minute warning before telling children to clean-up. It gives them the opportunity to complete a task. Facility Specific TA/Consultation/Reminders : Furnishings: In space #1, a violation was cited for a child’s size couch and chair in the block area, not being in good repair. There were several rips and exposed foam on the furnishings. According to child care rule G.S. 110-91(6): .0601(b), all equipment and furnishings shall be in good repair. Review violations with staff identifying safety hazards of equipment and furnishings. Peeling Paint: Also, refer to child care rule15A NCAC 18A .2821(a) regarding paint peeling on the wall. Walls shall be clean and in good repair. In space #2, there were peeling paint and exposed sheetrock along every part of the top half of the walls (all four sides). The teacher stated that she is waiting on the walls to be painted but does not have a date. Review violations with staff on how to identify and report indoor and outdoor safety concerns during the next staff meeting provide a maintenance request for identifying safety hazards in the indoor and outdoor environments. Screen Time Documentation: A violation was cited for not having documentation for using Screen Time. Screen time shall be documented on a screen time log or the classroom’s activity plan. There were four (4) children in space #3, sitting at the table using tablets. The teacher stated she did not know about documenting children’s screen time. A screen time log was given to the teacher during today’s visit. Refer to child care rule 10A NCAC 09 .0510(d)(2)(A-C). A Screen Time log was given to the teacher and the form was completed during the visit. ABCMS – CBC Provider Portal Reminder: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The information should be updated in ABCMS on an ongoing basis as staff members are hired and when employment is terminated. This satisfies the requirement to notify the Division of new child providers working who were hired or moved into the child care facility within five business days. NCFELDS: During today’s visit, I discussed with teachers if they had access to NCFELDS and provided an extra copy to each classroom. In 2011, the North Carolina Early Childhood Advisory Council (ECAC) launched and funded an important project of revising the Infant-Toddler Foundations and Preschool Foundations to create the North Carolina Foundations for Early Learning and Development (NCFELDS)—a single document that describes children’s development and learning from birth to age five that would serve as a resource for all programs in the state. Foundations describes Goals for all children’s development and learning no matter what program they may be served in, what language they speak, what disabilities they may have, or what family circumstances they are growing up in. Teachers and caregivers can turn to Foundations to learn about child development because the document provides age-appropriate Goals and Developmental Indicators for each age level—infant, toddler, and preschooler. Foundations are also intended to be a guide for teaching–not a curriculum or checklist that is used to assess children’s development and learning, but a resource to define the skills and abilities we want to support in the learning experiences we provide for children. The Goals for children can be used by teachers, caregivers, and other professionals who support and promote children’s development and learning. Three books were given to teachers for classroom use. Reminder: Reminder - Emergency Preparedness and Response Drills and Plan (EPR): Ensure each year to go online into your portal and update your EPR plan. Drills are to be completed quarterly and the Ready to Go File and Kit are available and ready in case of an emergency. Review the EPR plan with all staff at least annually. Cots were observed at least eighteen inches apart. Children were observed eating snack and playing with developmentally age appropriate materials like, puzzles, sorting blocks, dinosaurs, and activity center’s materials. Snack consisted of Teddy Grahams and water. Ten percent of children and staff files were checked during the visit. NC Pre-K children files were included in a random selection. The previous Annual Compliance Visit was completed February 15, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 94%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued August 25, 2020. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by having 75% of teachers with an AAS or higher in ECE/CD. The information on record with DCDEE was verified as correct by Kimberly Baker, administrator, and the owner’s corporation status, Heavenly Haven Child Development Center, Inc, was verified on the Secretary of State’s website as current-active. There are concerns that a child was left outside unsupervised. Ms. Baker was present and assisted me while I observed the indoor and outdoor areas. There were thirty-six (36) children in care. Just before monitoring staff/child ratios, In space #6, a child was sent out of the classroom and into the hallway without supervision for approximately one-minute. Children were observed, participating in group activities and indoor play. Children used developmentally age-appropriate materials like, dinosaurs, large Duplos, cars and trucks. Children discussed the letter, number, and shape of the week. They discussed the weather and classroom rules. A teacher read a story about Hanukkah to her group of children. She explained how and when candles are lit. Today’s lunch consisted of chicken patty, baked beans, pineapples, bread, and milk. The allegations were discussed with Ms. Baker. I interviewed the administrator, and the staff member mentioned in the allegation. The administrator and teacher stated that the incident did happen on November 22, 2024 and a child in space #6 was left outdoors for less than one minute. The teacher counted the children as they lined up to go indoors. Once the door was closed and children were indoors, the teacher counted and realized that one child was missing. She immediately opened the door and the child came in. The teacher was suspended for five days. Based on the information gathered and observations made, the allegation of violations of child care requirements pertaining to a child being left on the playground unsupervised is substantiated. At the completion of the visit, the Visit Summary was reviewed with K. Baker. I was greeted by Carrie Watlington, teacher. She assisted me as I observed the indoor and outdoor areas. Kimberly Baker, Administrator, arrived during the visit. There were twenty-eight (28) children in care. In space #4, children were observed sitting and running outdoors. Once indoors, children toileted, washed their hands, and participated in creating pictures using paper, markers, and green paint. Children were also observed getting a drink of water. Children in space #2, were observed sitting on the carpet listening to a story. The teacher asked the children questions during the story like, “What do you think was wrong? And Why do you think they took the dog to the park?” The teacher repeated the children’s response and gave affirmation. Children in space #1, were observed outdoors sitting in the shade. They reviewed colors, and sung songs (ABC’s, Ten Little Monkeys, and Head Shoulder, Knees, and Toes). The teacher asked children to show her their body parts (nose, head, ears, etc.). I observed, positive communication, diaper changing, toileting, handwashing, sanitation, lunch, and rest time. Today’s lunch consisted of fish sticks, green beans, bread, fruit cocktails, and milk. At the completion of the visit, the Visit Summary was reviewed with K. Baker. This program currently operates with a Four-Star License issued August 25, 2020. I was greeted by Carrie Watlington, teacher. She assisted me as I observed the indoor and outdoor areas. Kimberly Baker, Administrator, arrived during the visit. There were twenty-eight (28) children in care. In space #4, children were observed sitting and running outdoors. Once indoors, children toileted, washed their hands, and participated in creating pictures using paper, markers, and green paint. Children were also observed getting a drink of water. Children in space #2, were observed sitting on the carpet listening to a story. The teacher asked the children questions during the story like, “What do you think was wrong? And Why do you think they took the dog to the park?” The teacher repeated the children’s response and gave affirmation. Children in space #1, were observed outdoors sitting in the shade. They reviewed colors, and sung songs (ABC’s, Ten Little Monkeys, and Head Shoulder, Knees, and Toes). The teacher asked children to show her their body parts (nose, head, ears, etc.). I observed, positive communication, diaper changing, toileting, handwashing, sanitation, lunch, and rest time. Today’s lunch consisted of fish sticks, green beans, bread, fruit cocktails, and milk. At the completion of the visit, the Visit Summary was reviewed with K. Baker. Name of Operation: HEAVENLY HAVEN CHILD DEVELOPMENT CENTER II Facility ID: 26002143 Consultant: JENNIFER STRICKLAND Operation Type: Center Case Number: Visit Date: 10/24/2023 Number Present: 37 Completed Date: 10/24/2023 Age: From 1 To 11 Total Minutes: 240 Time In: 09:00 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a Routine Unannounced Visit. The previous Annual Compliance Visit was completed April 25, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 93%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four (4)-Star License issued August 25, 2020. The License was based on the program earning seven (7) points in education standards, two (2) points in program standards, and one (1) quality point by choosing to meet a programmatic option by meeting enhanced policies approved and infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Kimberly Baker, Administrator, and the owner’s corporation status, Heavenly Haven Child Development Center, Inc, was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed July 10, 2023. Fire Classification of approved completed October 21, 2022. K. Baker, Administrator, was present and assisted me while observing the indoor and outdoor areas. In space #6, I observed the children sitting on the floor with their teacher, listening to a story. The teacher discussed the pictures and what was read. Do you see apples? Do you like apples? The other teacher changed the children’s diapers while describing her actions. In space #1, Children transitioned outdoors to participate in gross motor activities. Items stored in plastic (cups and napkins), grocery bag with bread and art supplies, other art supplies, and a plastic bag with books were less than five feet above the floor accessible to children under three years of age. Children in space # 5 were on the carpet listening to the teacher read a book. When finished, the teacher asked them, “Do you want to sing a song? Children responded: bumble bee! The teacher replied, Bumble bee, you want to sing? I am bringing home a bumble bee. Ok, let’s sing! Children sang with the teacher and mimicked the song gestures. Parents and children were greeted and welcomed in the classroom during drop-off. In space #3, children were observed playing board games (Trouble, Go Fish, and in the block area) with the teachers. Teachers reminded children of the game's rules when necessary to keep the game going. Children were very involved in the activities. Lunch served was beef lasagna, Corn, fruit cocktail, and milk. The facility's program records were reviewed for compliance. At the completion of the visit, the Visit summary was reviewed with K. Baker. The following violations were observed: Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Violations observed and documented today may impact the compliance history score. The violation was corrected during the visit. No compliance letter is due. Facility Specific TA/Consultation/Reminders: Choking Hazards A violation was cited during today’s visit for items that were potential choking hazards accessible to children under three years of age. I recommended K. Baker to talk to the staff about removing all materials accessible to children that could be a choking hazard in space #1. Additional information regarding plastic bags, toys, toy parts small enough to be swallowed, and materials that can be torn apart can be found in NC Child Care rule 10A NCAC. This violation was corrected during the visit. Fire Inspection: As a reminder, don't forget to forward a copy of the center's new fire inspections within seven days of completion to your Child Care Consultant. Observation Suggestion: I encourage teachers to continue to facilitate children’s learning. Although children can learn a lot independently and during peer play, they learn the most when their play and learning are facilitated by an adult. This is particularly important during center time. Continue to move around to different centers, talk to the children talk to the children about what they are doing, and push the learning opportunities provided by the center. It is important to note that all teachers can enhance these learning moments without being overly directive or interfering with children’s independence. The pamphlet contains information regarding common nonpoisonous plants that are safe for indoors and outdoors in child care programs.
