The previous Annual Compliance Visit was completed July 02, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 97%. This program currently operates with a Four-Star License issued December 30, 2019. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Casandra Riley-Burns, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed January 09, 2026. Fire Classification of Approved completed October 07, 2025. Ms. Casandra Riley-Burns was present and assisted me while observing the indoor and outdoor areas. In Space #1, the children transitioned to go outdoors. Outside, the children rode on tricycles and pushed trucks. After the children returned indoors, the children washed their hands. In Space #2, the children were sitting on the carpet talking about swimming. In Space #9, the children were playing on the floor and exploring their space. The caregiver talked to the children and assisted the children in play. Toys and materials were available and open. Toileting and handwashing routines were observed. Lunch served was pork roast, potatoes, pineapples, bread, and milk. The most recent fire drill was completed on May 21, 2026, and the most recent shelter in place and lockdown drill was March 24, 2026. The most current monthly outdoor inspection was completed on May 20, 2026. At the completion of the visit, the Visit Summary was reviewed with Ms. Riley-Burns. The following violation was observed. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In Space #9, the activity plan did not include a date for reference. The previous Annual Compliance Visit was completed July 7, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 97% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). In Space #1, the children were observed in the following activities: pushing trucks, building with the blocks, and stacking Legos. The toys and materials were open and available for use. Toileting and handwashing routines were observed. A violation was observed for ABCMS provider portal requirement. Lunch served was beef ravioli, cheese, corn, apple sauce, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Riley-Burns. The following violation was observed: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the requirements for the ABCMS provider portal. The operator was unable to provide a staff roster. G.S. 110-90.2 & .2703(r) Child Care programs are expect to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today must be corrected immediately. Additionally, by 5:00pm on July 16, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. The previous Annual Compliance Visit was completed July 7, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 97% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued December 30, 2019. The License was based on the program earning seven (7) points in education standards, two (2) points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Cassandra Riley-Burns, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed June 17, 2025. Fire Classification of Approved completed October 31, 2024. Ms. Riley-Burns was present and accompanied me while observing the indoor and outdoor areas. I observed school age children outside on the fenced in playground. The children were running about the space, coloring with chalk, playing with musical instruments, and playing with toys. In Space #1, the children were observed in the following activities: pushing trucks, building with the blocks, and stacking Legos. The toys and materials were open and available for use. Toileting and handwashing routines were observed. A violation was observed for ABCMS provider portal requirement. Lunch served was beef ravioli, cheese, corn, apple sauce, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Riley-Burns. The following violation was observed: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the requirements for the ABCMS provider portal. The operator was unable to provide a staff roster. G.S. 110-90.2 & .2703(r) Child Care programs are expect to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today must be corrected immediately. Additionally, by 5:00pm on July 16, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected.
