The previous Initial Assessment Visit was completed December 10, 2024, and prior to today’s visit, the eighteen-month compliance history score was 94%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star Family Child Care Home license issued June 13, 2025. The license was based on the program earning six (6) points in education standards, two (2) points in program standards, and one (1) quality point by choosing to meet programmatic option: uses an age/developmentally appropriate curriculum. The information on record with DCDEE was verified as correct, and the owner’s status was listed as administrative-dissolved on the Secretary of State’s website. Inspections are as follows: Sanitation Classification of Superior was completed on November 12, 2024. Angela Stevens, Child Care Consultant, accompanied me during today’s visit. Toni Anthony, Operator, was present while we observed the indoor and outdoor areas. Upon our arrival, three (3) children were present. Children had access to blocks, fine motor, dramatic play, and literacy materials. The children were observed stacking blocks, completing puzzles, and playing with soft toys. One child was observed napping in a playpen. The operator read a book that a child brought to her, titled ‘I’m Ready to Read with Minnie’. The outdoor learning environment was monitored for health and safety requirements, and was observed in compliance with applicable child care rules. Routine care and handwashing practices were observed. Lunch consisted of shredded turkey, rice, broccoli, pears, and milk. The operator’s file was reviewed, and all documentation was observed in compliance with applicable child care rules. One child’s record was reviewed, and all documentation was observed in compliance with applicable child care rules. Program files were reviewed. The most recent fire drill was completed on November 17, 2025. The most recent quarterly drill was completed on October 03, 2025. The most recent outdoor inspection was completed on October 20, 2025. The EPR plan was most recently updated on November 30, 2025. The program does not currently provide transportation to enrolled children. At the completion of the visit, the Visit Summary was reviewed with T. Anthony. The following violations were observed: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Anthony assisted me during the visit. While monitoring the indoor space, I observed an unzipped red Steve Madden Tote with a child’s personal belongings stored inside a plastic bag sitting on top of a bench accessible to children in care. When prompted, T. Anthony removed the plastic bag with the child’s personal belongings from the tote and placed the child’s personal belongings inside their assigned cubby and discarded the plastic bag. Additionally, I observed a mylar Princess Birthday balloon tied to a vase of fresh flowers sitting on the countertop in the kitchen. This was corrected when T. Anthony removed the balloon from the primary care space and took it upstairs in the FCCH. At 9:35 a.m., a child, aged five months, arrived at the facility. The parent signed the child in for care and requested to express/pump human milk prior to leaving the facility. Accommodations were made for the parent to pump human milk. T. Anthony engaged in pleasant talking with children throughout the observation during both free play/tummy time activities and routines. The talk was personalized, with frequent one-on-one communications. Prior to mealtime, a diapering routine was observed. Lunch prepared and served was spaghetti with pasta sauce and ground beef, sweet corn, diced peaches, and milk. A walk-through of the indoor and outdoor areas was completed. T. Anthony’s personnel file, and children’s and program records were monitored today. The most current playground inspection checklist on file was dated May 6, 2025. The most current fire drill documented on file was May 1, 2025, at 10:00 a.m. At the completion of the visit, the Visit Summary was reviewed with T. Anthony.
