The previous Temporary Time Period visit was completed on June 24, 2025, and prior to today’s visit, the eighteen-month compliance history score was 98%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued August 04, 2025. The license was based on the program earning five (5) points in education standards, two (2) points in program standards, and one (1) quality point by choosing to meet the programmatic option of using an age/developmentally appropriate curriculum. The information on record with DCDEE was verified as correct by Viranda DeShields and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Inspections are as follows: Sanitation Classification of Superior was completed on April 03, 2025. Fire Classification of Approved for Daytime Care Only was completed on January 12, 2026. V. DeShields was present while I observed the indoor and outdoor areas. Upon my arrival, nine (9) children and three(3) teachers were present. Children had access to blocks, fine motor, dramatic play, creative art, and literacy materials. In Space #1, children were observed playing with dolls in the dramatic play area, reading books in the library area, and stacking linking logs in a carpeted area. The children cleaned up the materials and transitioned to a carpeted area for a group activity. The teachers and children read and chanted to “We’re Going on a Bear Hunt”. In Space #2, children were observed participating in free choice activities. A diaper change was observed, and all diapering procedures were followed. No children were present in Space #3. However, the space was monitored for general health and safety requirements. The outdoor learning environment was monitored for playground safety requirements. Due to the cold weather, children were not observed participating in outdoor play. Routine care and handwashing practices were observed. The parents/guardians provided lunch for their children. A Nutrition Opt Out Form was on file for each enrolled child. However, the facility keeps the kitchen stocked with supplemental food in case a child arrives without all meals and drinks for the day. Five staff files were reviewed. One staff member did not complete the required Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete all Health and Safety training topics within five years of the previous Health and Safety training. In the free course, you learn risk factors and warning signs for mental health and addiction concerns, strategies to help someone in both crisis and noncrisis situations, and where to turn for help. Offered by UNC School of Social Work and the NCDHHS Division of Child Development and Early Education. Education requirements were also reviewed. Based on today’s conversation, the facility is strongly thinking about pursuing the Classroom and Instructional Quality Pathway. The administrator stated that the program is already using the Funny Daffer Curriculum. She will need some assistance with choosing an approved formative assessment tool. A rated license assessment was conducted. The previous visit was completed on May 13, 2025, and prior to today’s visit, the eighteen-month compliance history score was 99% as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Temporary License issued February 3, 2025 to August 3, 2025. The information on record with DCDEE was verified as correct by Viranda DeShields and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Mary Beth Datka, Licensing Supervisor, accompanied me during today’s visit. Current Inspections are as follows: Sanitation Inspection with a classification of Superior, with six (6) demerits completed April 3, 2025. Fire Inspection, approved for daytime only, dated January 23, 2025. V. DeShields, Administrator, was present. We greeted V. Deshields, and I explained the purpose of the visit and requested to wash our hands prior to conducting a walk-through of the indoor and outdoor areas. There was no enrollment record for Space #1. In Space #2, infants sat with Y. Jones, Teacher, on the carpeted floor and in a bouncy seat, while soft music played in the background. Infants had access to soft books, blocks, and dolls. Infant sleep charts were monitored. Documentation on file was completed June 2, 2025. Based on attendance records observed, two infants were present June 3-23, 2025. No completed sleep charts for June 3-23, 2025, were available for review. Bottle feeding occurred during the visit. Bottles were dated and labeled for individual children. Two diapering routines were observed. All steps in the diapering procedure were implemented. Y. Jones and the infants in care transitioned outdoors for physical activity. Upon their return indoors, they washed their hands and participated in free play activities. Children’s and staff files were monitored on May 13, 2025. No new staff have been employed since the visit was completed on May 13, 2025. Required center program records were monitored today. The most current fire drill was completed on May 28, 2025, at 8:00 a.m. The most current lockdown drill was completed on May 22, 2025. The most current outdoor inspection checklist was completed on May 28, 2025. At the completion of the visit, the Visit Summary was reviewed with V. DeShields. During outdoor play, children learn about their environment, science, and nature. Infants’ and young children’s participation in physical activity is critical to their overall health, development of motor skills, social skills, and maintenance of healthy weight. Daily physical activity promotes young children’s gross motor development and provides numerous health benefits, including improved fitness and cardiovascular health, healthy bone development, improved sleep, and improved mood and sense of well-being. Toddlers and preschoolers generally accumulate moderate to vigorous physical activity over the course of the day in very short bursts (15–30 minutes). Children may be able to learn better during or immediately after these types of short bursts of physical activity, due to improved attention and focus. These outdoor times can be curtailed somewhat during adverse weather conditions in which children may still play safely outdoors for shorter periods, but the time of indoor activity should increase so the total amount of vigorous activity remains the same. Rated License Assessment: An application for assessment for a voluntary 2-5 star rated license was received today. The facility did not request the ERS as part of the rated license assessment. Education Standards: Administrator – The Administrator is qualified as a Level III administrator, 7 years EC admin experience, and BSAC. The administrator earned seven (7) points. Program Coordinator – The PC meets minimum requirements (NCECAC + NCECC + BSAC) and has 24 SA SH completed and over 900 hours licensed school-age experience. The PC earned seven (7) points. Lead Teacher – The lead teacher employed to work in Space #2 does not have a WORKS account. I recommended the teacher set up her WORKS account, upload a copy of her high school diploma, and check the position to be evaluated for qualification. Ensure that the lead teacher scheduled to begin work on July 7, 2025, has a WORKS account created with information required to be evaluated for the position of lead teacher. Program Standards: Two (2) points The facility meets requirements in Rule .2806, and enhanced ratios. The facility has earned 2 points in program standards. Quality Point: One (1) point The facility meets the following quality point programmatic option: Using a curriculum as defined in Rule .0102(10). The curriculum used is Funny Daffer Curriculum. Once points have been assigned for education standards, the total points will be assigned for the rated license assessment. At the end of the temporary time period, the rated license assessment packet will be completed and submitted to Raleigh for the issuance of a star rated license. The facility file will be transferred to the child care consultant in the assigned territory once the new license has been issued. You will need to submit this document to your consultant in preparation for your annual compliance visit. You will need to have an annual fire inspection. It is your responsibility to contact the local fire inspector to request this inspection. An annual sanitation inspection will be due by April 2, 2026, however more frequent visits may be conducted. An annual fire inspection will be due by January 22, 2026. The fire inspection must be submitted to your consultant within 7 days. I also reminded you that the water must be tested for lead every three years. On-going Training: Training hours will be monitored for compliance during your annual compliance visit. Training hours are required to be recorded on the on-going training log with certificates attached to the log for verification. Health & Safety Training: All staff must complete Health & Safety training topics within one year of hire. A training log is available on the DCDEE website. This training information will be reviewed by a DCDEE consultant during a monitoring visit. Annually you must review the EPR plan online and print the review form for your records. You must review the EPR plan and the EMC plan with all staff when it is created, and on an annual basis. I recommend doing this at the same time everyone completes an annual health questionnaire and emergency information. This would also be a good time to review the center’s Emergency Medical Care Plan and to conduct annual staff evaluations and staff development plans. A rated license assessment was conducted. The previous visit was completed on May 13, 2025, and prior to today’s visit, the eighteen-month compliance history score was 99% as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Temporary License issued February 3, 2025 to August 3, 2025. The information on record with DCDEE was verified as correct by Viranda DeShields and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Mary Beth Datka, Licensing Supervisor, accompanied me during today’s visit. Current Inspections are as follows: Sanitation Inspection with a classification of Superior, with six (6) demerits completed April 3, 2025. Fire Inspection, approved for daytime only, dated January 23, 2025. V. DeShields, Administrator, was present. We greeted V. Deshields, and I explained the purpose of the visit and requested to wash our hands prior to conducting a walk-through of the indoor and outdoor areas. There was no enrollment record for Space #1. In Space #2, infants sat with Y. Jones, Teacher, on the carpeted floor and in a bouncy seat, while soft music played in the background. Infants had access to soft books, blocks, and dolls. Infant sleep charts were monitored. Documentation on file was completed June 2, 2025. Based on attendance records observed, two infants were present June 3-23, 2025. No completed sleep charts for June 3-23, 2025, were available for review. Bottle feeding occurred during the visit. Bottles were dated and labeled for individual children. Two diapering routines were observed. All steps in the diapering procedure were implemented. Y. Jones and the infants in care transitioned outdoors for physical activity. Upon their return indoors, they washed their hands and participated in free play activities. Children’s and staff files were monitored on May 13, 2025. No new staff have been employed since the visit was completed on May 13, 2025. Required center program records were monitored today. The most current fire drill was completed on May 28, 2025, at 8:00 a.m. The most current lockdown drill was completed on May 22, 2025. The most current outdoor inspection checklist was completed on May 28, 2025. At the completion of the visit, the Visit Summary was reviewed with V. DeShields. The following violation was observed and corrected during the visit: Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Space #2, the caregiver did not document compliance with visually checking on sleeping infants aged 12 months or younger from June 3-23, 2025. This was corrected when the operator documented compliance with visually checking on sleeping infants in care on June 24, 2025. .0606(g) No compliance letter is due at this time. As a reminder, Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The previous visit was completed on March 5, 2025, and prior to today’s visit, the eighteen-month compliance history score was 100% as required by North Carolina General Statute 110-90(4)(c-d). I explained the purpose of today’s visit and requested to conduct an unaccompanied walk-through of the indoor and outdoor areas. Permission was granted. In Space #1, a child, aged four years old, was present and engaged in free play activities. The child manipulated a counting game with numbers and numerical illustrations, counted by rote in Spanish, and hosted a pretend party with people figures, animals, and dolls. The party was held in the block area using the doll house with furnishings and pretend food. V. DeShields was the honorary guest. V. Deshield participated in parallel play and encouraged conversation and problem solving by asking open-ended questions. V. DeShields followed the child’s lead. In Space #2, two infants were present and slept in their assigned cribs. Bottles with formula were labeled, dated, and stored appropriately. Sleep charts were completed and maintained on file for thirty days. Soft music played in the background while the children slept. After rest time, the infants participated in tummy time activities. Soft dolls, musical toys, and books were accessible to the nonmobile infants. A diapering routine and infant feeding were observed. There is no enrollment record for Space #3. Staff and children’s files were monitored today. In addition, required center program records were also monitored. The most recent fire drill was completed on April 28, 2025. The most recent lock down drill was completed on February 27, 2025. The most recent outdoor inspection was completed on April 29, 2025. Attendance records were current/accurate and maintained for each group of children. An infant, aged eleven months, arrived for care at 10:30 a.m. and was signed in by the parent. Meals are not prepared on site. Lunch is prepared at home and brought to the facility by the parent(s). A nutrition opt out form is on file. Additionally, current infant feeding plans were posted on the information board in Space #2. At the completion of the visit, the Visit Summary was reviewed with V. DeShields. The following violation was observed during the visit: Violation Number Comment Rule 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. The teacher present in Space #2 and counted in staff/child ratio has not completed ITS-SIDS training. .01102 (f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today may impact the compliance history score. The violation observed and documented today must be corrected immediately. Additionally, by 5:00 p.m. on May 27, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violation was corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance Specific to Today’s Visit: Health and Safety Training Requirements: In centers that are licensed to care for infants, the child care administrator and any child care provider scheduled to work in the infant room shall complete ITS-SIDS training. ITS-SIDS training shall be completed within two months of an individual assuming responsibilities in the infant room and every three years thereafter. Completion of ITS-SIDS training shall be included once every three years. At all times, one child care provider who has completed ITS-SIDS training shall be present in the infant room while children are in care. A certificate of each staff member's completion of this course shall be maintained in the staff member's file in the center. During today’s visit, the Administrator stated, “The teacher working in the infant room has not completed ITS-SIDS Training. She is registered to complete the training in May 2025.” Please be reminded: Safe sleep practices help reduce the risk of sudden unexpected infant deaths (SUIDs). Knowledge of injury prevention measures in child care is essential to control known risks. Most SIDS deaths in child care occur on the first day of care or within the first week; unaccustomed prone (tummy) sleeping increases the risk of SIDS eighteen times. Additionally, shaken baby syndrome/abusive head trauma is completely preventable. Pre-service training and frequent refresher training can prevent deaths. Injuries and Sudden Infant Death Syndrome (SIDS) have occurred when children have been left to sleep in car seats or infant seats when the straps have entrapped body parts, or the children have turned the seats over while in them. Sleeping in a seated position can restrict breathing and cause oxygen desaturation in young infants. Sleeping should occur in equipment manufactured for this activity. When children are awake, restricting them to a seat may limit social interactions. These social interactions are essential for children to gain language skills, develop self-esteem, and build relationships. Strategies for achieving compliance with Item #1831 were discussed and due to staff absences today, the administrator was not able to work in the infant room with the teacher. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. DeShields did not request to have the ERS completed as part of the rated license assessment. Education Standards: The staff information sheet for rated license was completed. I stated that education must be current in WORKS by the fifth month (July) of the temporary time period to be eligible for points assigned for education standards. Quality Point: V. DeShields and I reviewed and discussed eligibility requirements for the quality point option. The total points for the Child Care Star Rated License will be determined at the end of the six (6) months Temporary Time Period. As a reminder, to earn a three-star license and receive subsidy funds, the facility must earn at least a total of seven (7) points. Emergency Preparedness and Response (EPR): V. DeShields completed the Emergency Preparedness and Response in Child Care training on January 2, 2024. V. DeShields shall go online and complete the EPR Plan on the template supplied by the Division by June 3, 2025. Emergency drills are being conducted and documented on the emergency drill log. V. DeShields shall review the EPR Plan annually or when information in the plan changes to ensure all information is current. All staff shall review the center's EPR Plan during orientation and/or on an annual basis with V. DeShield. Documentation of the review shall be maintained on file. During today’s visit, V. Deshields stated that she is currently working on the EPR plan for this facility and the EPR plan will be completed on the template supplied by the Division by June 3, 2025. Indicator Month: Discussions occurred regarding the indictor month for annual compliance monitoring visits at a licensed child care facility. I informed V. DeShields that when a facility becomes licensed, it is assigned an “annual compliance” month. The annual compliance month is the month in which the facility’s Temporary License was issued (February). I stated the annual compliance month does not change if the facility is issued a 2 – 5 Star License, a Provisional License, or a Probationary License. Child care consultants use the annual compliance month to determine when to schedule each facility’s annual compliance visit. Additionally, representatives of the DCDEE may visit your program at any time and must complete at least one full monitoring visit at least annually. To best support you and the children in care, our goal is to visit programs more frequently. On-going Training: The staff and training worksheet was updated during today’s visit to capture new staff hired from the last visit. As a reminder: After the first year of employment, the child care administrator and any staff who work with children shall participate in on-going training activities annually.
