The previous Annual Compliance Visit was completed February 25, 2025, and prior to today’s visit, the eighteen-month compliance history score was 96%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License issued October 10, 2025. The program earned the license based on Accreditation of Head Start. The information on record with DCDEE was verified as correct by Amy Emauel, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 12/16/2025. Fire Classification of Approved Daytime Only completed 8/27/2025. Ms. Emanuel was present while observing the indoor and outdoor areas. Upon my arrival, the children in space A, B, C, D, and E, were observed play in free choice play. In space A, two children were also observed finishing their breakfast. While monitoring space B, there was expired medication found in a cabinet. In space C, there were three packages of wipes that had the statement on the package “keep out of reach of children”. The packages was stored in a drawer that was not at least five feet from the floor. The children were observed playing with stacking blocks, wooden blocks, puzzles, reading books and dancing to music with their teachers. Breakfast served today was cheerios, fresh mixed berries, and milk. At the completion of the visit, the Visit Summary was reviewed with Emanuel. The following violations were observed. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space C, there were three packages of wipes in a drawer that was not at least 5 ft. from the floor. The packages had keep out of reach of children. This was corrected when the administrator put the packages in a cabinet that was 5 ft. high and made them inaccessible to children. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space B, there was expired medication in a cabinet that belong to one of the children in the space. This was corrected when the Administrator removed it from the classroom and gave to the health tech to contact parent to pick up expired medication. .0803(12) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The previous Annual Compliance Visit was completed March 6, 2024, Prior to today’s visit, the eighteen-month compliance history score was at 97%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License issued January 25, 2023. Yolanda Pridgen ,Child Care Consultant, was present and assisted with today’s visit. There is a concern that a child was not adequately supervised. There is a concern that infants were not separated from older children as required. Ms. Acevedo was present while we observed the indoor and outdoor areas. Upon our arrival children were observed playing outdoors. The children were observed riding bikes, playing with blocks, and playing on the large play structures. The allegation regarding supervision was discussed with Ms. Acevedo. Staff assigned to the classroom where the concern was addressed in the report were interviewed. During interviews, staff stated that no children have ever been left alone in any space. Video was requested however, the administrator stated it was no longer available because it was retained for only three days. Based on information gathered and reviewed, this allegation regarding supervision was unsubstantiated. The allegation regarding grouping of children was discussed with Ms. Acevedo. Staff assigned to the classroom where the concern was addressed in the report were interviewed. During interviews, staff stated a four-year-old child received care on one occasion in Space #A, where infant age children were also present and receiving care. Upon our arrival children were observed playing outdoors. The children were observed riding bikes, playing with blocks, and playing on the large play structures. The allegation regarding supervision was discussed with Ms. Acevedo. Staff assigned to the classroom where the concern was addressed in the report were interviewed. During interviews, staff stated that no children have ever been left alone in any space. Video was requested however, the administrator stated it was no longer available because it was retained for only three days. In space b, children 1year of age, were observed from 9:10a.m. to 9:25a.m., the children were observed having breakfast, while some children were playing in free choice play. One teacher was changing the children’s diapers before getting ready for outdoor play. While the other teacher was assisting the last child who was finishing up breakfast and still talking and singing song “The Itsy Bitsy Spinder” with the other children. In space E, an observation was conducted from 9:25 a.m. to 9:45a.m., the children were observed playing in free choice activities. One of the teachers was doing a small painting activity with four to five children at a time. The children were painting a large pumpkin for the fall. They used the fall colors, like red, yellow, green, orange, and brown. The teacher allowed the children to choose other colors like purple, white, and pink. The children really seem to enjoy it. The other teacher moved about the space and assisted other children who were playing in the block area. The children were building a home and playing with small people, and cars. The teacher talked with them about why they were rebuilding their home. The children in space A, were observed in the outdoor space environment. The teachers sat with the infants while they crawled about the space and sat in the teachers’ laps, or close to them. The children did not have any toys outdoors that were age appropriate for this group of children during the observation. However, the teacher stated that they did have toys for the children, however, the toys were not made available for the children to play with. One new staff file was monitored during today’s visit. The teacher talked with them about why they were rebuilding their home. The children in space A, were observed in the outdoor space environment. The teachers sat with the infants while they crawled about the space and sat in the teachers’ laps, or close to them. The children did not have any toys outdoors that were age appropriate for this group of children during the observation. However, the teacher stated that they did have toys for the children, however, the toys were not made available for the children to play with. One new staff file was monitored during today’s visit. Please see attached staff and training worksheet. At the completion of the visit, the Visit Summary was reviewed with Ms. Emanuel. Facility Specific TA for Observation Based on today’s classroom observation, I discussed with Ms. Emanuel the positive interactions and various learning opportunities provided to the children throughout the visit. However, there was one space that may need a little help with activities that need to be provided for the children when participating in outdoor play. The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. These rule changes may or may not impact your facility. The multi-age group staff/child ratio changes support a previous rule change submitted by Early Head Start and the new rule for defining cooperative arrangements provides clarification to a previous change in statute.
