The previous Annual Compliance Visit was completed, February 10, 2026, and prior to today’s visit, the eighteen-month compliance history score was at 97%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License March 12, 2020. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet a programmatic option with staff benefits package and infrastructure of parent involvement. The information on record with DCDEE was verified by Brenda Mathis, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 10/6/2025. Fire Classification of Approved Daytime Only 5/7/2025. B. Mathis, accompany me while I observed the indoor and outdoor areas. Children were observed transitioning from breakfast to bathroom routines and putting on their jackets to go outdoors. Some children were having breakfast, and some were observed participating in large group time and participating in outdoors play. The children outdoors was observed climbing on the large play equipment, playing tag and playing on other small play structures. In space #5, the children were observed playing in the block area with Magna tiles, small people, and foam blocks. The children built two large blocks with the magna tiles and then used the foam blocks to build a house for the people they were playing with. Breakfast served today was cooked apples, biscuits and milk. On 4/29/2026, the following ERS summary was received via email. On 4/30/2026, Ms. Mathis was contacted via telephone to share the scores and to schedule a visit to review the ERS summary. The following ERS assessment was conducted on 4/20/2026. ECERS-3 (NC Pre-K space #5) with a score of 4.77. A copy of the ERS assessment was given to Ms. Mathis during today’s visit. At the completion of the visit the visit summary was reviewed with Ms. Mathis. The following violation was observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facility has not received a fire inspection within 12 months. They were observed playing on the large play equipment, playing with large legos, puzzles, and bubbles. Breakfast served today was sausage, hashbrown, and milk. The allegations that inappropriate discipline occurred were discussed with Brenda Mathis, Administrator. The incident was reported to DCDEE by B. Mathis as a self-report. During the visit, I interviewed one teacher who has worked with the teacher in question in space #4. It was stated during the interview that she could not confirm that inappropriate discipline occurred with any staff members that she had worked during her time at the facility. An additional teacher who works with the staff member in question was not present during the visit due to medical leave. B. Mathis stated that she interviewed the additional staff member on the same day the incident occurred, and it was confirmed that no inappropriate discipline was used in space #4, by any staff member. I also interviewed the teacher in question via telephone and based on the interview the teacher stated that she did not remember if she did or did not use inappropriate discipline with any child. She stated that she did grab one child by the hand to assist the child with putting a puzzle back on the shelf. Based on documentation received, information gathered, and observations made during today’s visit, the allegations of violation of child care requirements regarding inappropriate discipline was substantiated. At the completion of the visit, the Visit Summary was reviewed with Ms. Mathis. The following violation was cited. Violation Number Comment Rule 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. Based on information received it was confirmed that a child was handled in a rough way. This was corrected by no staff being observed using inappropriate discipline during today's visit. .1803(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. During today’s visit it was discussed with B. Mathis the plan of action moving forward regarding the facility’s discipline and behavior policy. Ms. Mathis stated that she sent all staff the facility’s policy on Discipline and Behavior Management and information on “What is Child abuse and Neglect” from the Recognizing and Responding to Suspicions of Child Maltreatment website. B. Mathis stated a staff meeting will be conducted on 2/24/2026 to discuss the information sent to them via email. The administrator stated staff will be required to sign an attestation receipt of the discipline and behavior management information discussed during the meeting. B. Mathis stated she will conduct the training “Nurturing the Brain” regarding handling stress and stressful situations in the classroom. B. The previous Annual Compliance Visit was completed, March 5, 2024 and prior to today’s visit, the eighteen-month compliance history score was at 99%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License March 12, 2020. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet a programmatic option with staff benefits package and infrastructure of parent involvement. The information on record with DCDEE was verified by Brenda Mathis, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 2/23/2024. The operator stated that she has tried to contact Health Department and it was stated that they are short staffed but would be out soon. Fire Classification of Approved Daytime Only 2/11/2025. B. Mathis, accompany me while I observed the indoor and outdoor areas. The children in space #1a, #1b, #2, and 3, were observed participating in free choice activities. They were observed playing with blocks, books, stacking toys, soft toys, and animals. In space #4, and #5, the children were observed participating in outdoors play. The children moved about the outdoors space freely. They were seen building things with large stackable blocks, playing in small play structures, and playing on the large play equipment. The lunch served today was chicken, potatoes, mixed vegetables, peaches, and milk. At the completion of the visit the visit summary was reviewed with Ms. Mathis. The following violations were observed. Violation Number Comment Rule 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The sanitation inspection is out of 12 month compliance. The facility has not have an inspection since 2/23/2024. GS 110-91 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One child's file did not have a signed policy for Prevention of Shaken Baby Syndrome and Abusive Head Trauma. This was corrected during the visit. The parent was contacted and came in to sigh the policy. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on March 13, 2025 you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. The previous Annual Compliance Visit was completed, March 5, 2024 and prior to today’s visit, the eighteen-month compliance history score was at 85%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License March 12, 2020. The License was based on the program earning seven points in education standards, five points in program standards, and one quality point by choosing to meet a programmatic option with staff benefits package and infrastructure of parent involvement. There is a concern that inappropriate discipline was used. B. Mathis, Administrator, accompany me while I observed the indoor and outdoor areas. Upon my arrival there were seventy-five children present. The children in space #1a, #1b, #2, and #3 were observed participating in free choice play. The children in space #4, #5, and #6 were preparing for outdoor play. The children were observed playing with soft toys, stacking blocks, dolls, and reading books. Lunch served today was chicken nuggets, applesauce, carrots, rolls, and milk. During my observation I observed two contract workers Katrina Brittain and Avery Morgan from Cumberland County schools pull several children from their classrooms to work with the children on speech and developmental activities. The allegations were discussed regarding inappropriate discipline was used. I interviewed the administrator and nine staff members. It was stated that a staff member who had recently quit, “stated that children were being popped”. Based on the information gathered, interviews conducted, and my observations during today’s visit, there wasn’t sufficient information to confirm the allegations of violation of child care requirements so the allegations that inappropriate discipline was used is unsubstantiated. One new staff and one volunteer file was monitored during today’s visit. See staff and training worksheet attached. At the completion of the visit the visit summary was reviewed with Ms. Mathis. The following violations were observed. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance).Prior to working with children outside of the classroom Avery Morgan who is a contract workers with Cumberland County Schools did not have a Criminal Background Check completed. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. Two contract workers form Cumberland County Schools Katrina Brittain and Avery Morgan were working with several children individually in a sperate space form the classrooms were the children attend. The statement must describe accurately and in detail how and when the violations were corrected.
