The previous Annual Compliance Visit was completed April 03, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 96% which is above the required 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued June 22, 2022. The License was based on the program earning four points in education standards, five points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Kimberly Fox, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed May 01, 2025. Fire Classification of Approved completed June 09, 2025. Ms. Fox assisted me while observing the indoor and outdoor areas. In Space #5, children were playing in the following activities: puzzles, manipulatives, blocks, table games, and dramatic play. In Space #6, there was infant child holding their bottle, there was one child sleeping in their crib, one teacher was holding an infant, one child was pushing a push toy, and the other children were observed on the carpet exploring their space. In Space #8, the children were sitting at the table and talking to their teacher. The teacher talked about biblical stories and the children repeated what the teacher was saying. After the lesson, the teacher pointed to each color and the child responded by naming the color. Lunch served was cheeseburgers, carrots, celery, applesauce, and milk. Violations were observed for the following: activity plan, attendance, Criminal Background Check Requirements, storage of hazardous items, storage of medication, tb test requirements, and staff medical statements. Program files were reviewed. The most recent fire drill was completed on March 30, 2026. The three-month drill (Shelter-In-Place) was completed on January 20, 2026. The most recent playground inspection was completed on March 31, 2026. The emergency preparedness plan was dated April 21, 2025. At the completion of the visit, the Visit Summary was reviewed with Ms. Fox. The following violations were observed: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in all classrooms. This included Space #1, #3, #4, #5, #6, #7, and #8. This was corrected when the administrated printed all activity plans and posted in the classroom GS 110-91(12); .0508(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #7, there was a container of Clorox wipes stored in an unlocked cabinet, less than 5 feet from the door. This was corrected when the staff membered moved the Clorox wipes to a higher shelf, greater than 5 feet from the floor. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In Space #7, there was a container of Zarabee's chest rub stored in a cabinet, greater than 5 feet from the floor. This was corrected when the staff member removed the medication outside of the classroom, and placed in a locked storage. 15A NCAC 18A .2820(d) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The medical report was not completed for one staff member prior to the first day of work. The staff member was hired on January 5, 2026. This was corrected. We talked about making sure that each staff member is aware that attendance will need to be maintained daily, at the time of arrival. If a staff member does not have their attendance form, the staff will need to reach out to management, to ensure that the attendance is completed at arrival. We discussed suggestions for record keeping. Refer to Child Care Rule .0302(d)(4) for more information about requirements for Daily records. Activity Plan: A violation was cited because there was not a current activity plan posted. Activity plans will need to be completed monthly and posted for easy reference. Today, you stated that you have transitioned to a new approved curriculum, and you have not printed the activity plans. I suggested it is fine as you are navigating through this new transition, to continue completing on your previous activity plan form, until, your staff is trained and ready for the new curriculum. Once you have the activity plan, the staff members will need to be trained on how to access and complete the activity plan. There is one staff member that does not have a qualified CBC on file with DCDEE. The staff member will need to complete the requirements for an updated CBC. Today, we discussed the process, and I provided some places to complete the CBC requirements. The employee will have two weeks to complete the requirements. If there is not a qualified CBC on file after two weeks, the employee is not permitted to work without a qualified CBC. Medical Statement/TB Test or TB Screening: During today’s visit, there was one staff member that did not have the required medical statement and TB test screening/test. This is required to be received prior to starting employment. This is important to make sure that each member of staff hired, has been cleared by a medical practitioner prior to starting. This is important in maintaining a safe and healthy environment. Refer to 10A NCAC 09 .0701 Health Standards for Child Care Providers, Substitute Providers, Volunteers, and Uncompensated for information regarding medical statements, health questionnaires, and emergency information. You stated that you are wanting to go through with the Pathway 2 option for Classroom and Instructional Quality Pathway. I walked you through the application process on the DCDEE website and where to locate the required forms for Pathway 2. Observations/Technical Assistance/Transitions: Today, I provided some resources that can assist when transitioning. It is helpful to let children know in advance before making transitions. Provide reminders five minutes before changing an activity. Allow opportunities for “job roles” for some of the children that may struggle with transitions. In addition, sometimes, providing a visual schedule helps create a smooth transition. In the toddler classroom, the children were preparing to go outdoors. The previous Annual Compliance Visit was completed September 14, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 94% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). During today’s visit, I observed the indoor and outdoor areas unaccompanied by a staff member. Ms. Fox arrived later in the visit to assist me. In space # 3, Preschool-aged children participated in indoor and outdoor free-choice play. In Space #6, infants participated in tummy time while others who were mobile explored the space. One caregiver interacted with the infants on the floor while the other monitored a child eating in a highchair. In Space #4, children sat on the carpet, listening to the teacher read a book about fighting. The teacher asked the children specific open-ended questions about some of the illustrations in the book. Items stored in plastic were found stored under 5 feet accessible to children under three years old in spaces #6, #7, and #8. Teachers and support staff helped take diapers out of the plastic sleeves, gloves, and wipes were stored under the changing table that had a lock or placed 5 feet up from the reach of children. While outdoors, children chased each other, rode tricycles, and walked around the playground. Teachers were positioned on opposite sides of the playground where they could monitor the children best. Handwashing, diapering, and bathroom routines were observed today. Lunch served was sweet and sour chicken, peas, brown rice, watermelon slices, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Fox. The following violations were observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division.. The operator failed to schedule a fire inspection prior to the expiration date which was July 6, 2024. The most current fire inspection for the facility was completed on July 18, 2024 and sent to the Child Care consultant on July 20, 2024. 10A NCAC 09 .0304(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Accessible to children in space #6, box of plastic gloves, and two plastic packages of wipes were stored on the diaper changing table. In spaces #6, #7, #8, plastic bags containing diapers and plastic packages of wipes were stored in the children's cubbies. Items were corrected by removing diapers from plastic sleeves and placing plastic containers of wipes 5 feet from the reach of children. .0604(q) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation was corrected during the visit. No compliance letter is due. Facility Specific TA/Consultation/Reminders: Fire Inspection: A violation was cited today for fire inspection. The previous fire inspection was dated for July 6, 2023, and the current fire inspection was completed July 18, 2024. It is imperative to make sure to contact the Fire Marshal to find out the status on your required fire inspection. An annual Approved Fire Inspection is required annually and validates the building, premises, and program are following current Fire Codes; all of which attest a safe environment for children. Fire Inspections will need to be completed within 12 months of the center’s previous fire inspection. In addition, the Rule states the inspection must be on the form provided by the Division and mailed or emailed to the Consultant within seven (7) days. Please refer to Child Care Rules 10A NCAC 09 .0304 (a), for more information regarding this rule. Choking Hazards: A violation was cited during today’s visit for items that were potential choking hazards accessible to children under three years of age. I recommended teachers remove all materials accessible to children that could be a choking hazard in spaces used for children under three years of age. I discussed with administrators to have staff check their classrooms for safety hazards prior to caring for children for the day to ensure no choking hazards are accessible to children. Additional information regarding plastic bags, toys, toy parts small enough to be swallowed, and materials that can be torn apart can be found in NC Child Care rule 10A NCAC .0604 (q) Observation Suggestion: I encouraged teachers to continue to model positive and respectful ways of engaging children, by standing close to them, reflecting their emotions, and by responding to their attempts to communicate both verbally and nonverbally. Respectful language was used, such as addressing each child by their name and moments were taken to check-in with each child during activities to see how they were doing and what their needs are. The previous Annual Compliance Visit was completed September 14, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 94% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Four-Star License issued June 22, 2022. The License was based on the program earning four points in education standards, five points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Kimberly Fox, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed January 22, 2024. Fire Classification of Approved completed July 18, 2024. During today’s visit, I observed the indoor and outdoor areas unaccompanied by a staff member. Ms. Fox arrived later in the visit to assist me. In space # 3, Preschool-aged children participated in indoor and outdoor free-choice play. In Space #6, infants participated in tummy time while others who were mobile explored the space. One caregiver interacted with the infants on the floor while the other monitored a child eating in a highchair. In Space #4, children sat on the carpet, listening to the teacher read a book about fighting. The teacher asked the children specific open-ended questions about some of the illustrations in the book. Items stored in plastic were found stored under 5 feet accessible to children under three years old in spaces #6, #7, and #8. Teachers and support staff helped take diapers out of the plastic sleeves, gloves, and wipes were stored under the changing table that had a lock or placed 5 feet up from the reach of children. While outdoors, children chased each other, rode tricycles, and walked around the playground. Teachers were positioned on opposite sides of the playground where they could monitor the children best. Handwashing, diapering, and bathroom routines were observed today. Lunch served was sweet and sour chicken, peas, brown rice, watermelon slices, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Fox. The following violations were observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division.. The operator failed to schedule a fire inspection prior to the expiration date which was July 6, 2024. The most current fire inspection for the facility was completed on July 18, 2024 and sent to the Child Care consultant on July 20, 2024. 10A NCAC 09 .0304(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Accessible to children in space #6, box of plastic gloves, and two plastic packages of wipes were stored on the diaper changing table. In spaces #6, #7, #8, plastic bags containing diapers and plastic packages of wipes were stored in the children's cubbies. Items were corrected by removing diapers from plastic sleeves and placing plastic containers of wipes 5 feet from the reach of children. .0604(q) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation was corrected during the visit. No compliance letter is due. Facility Specific TA/Consultation/Reminders: Fire Inspection: A violation was cited today for fire inspection. The previous fire inspection was dated for July 6, 2023, and the current fire inspection was completed July 18, 2024. It is imperative to make sure to contact the Fire Marshal to find out the status on your required fire inspection. An annual Approved Fire Inspection is required annually and validates the building, premises, and program are following current Fire Codes; all of which attest a safe environment for children. Fire Inspections will need to be completed within 12 months of the center’s previous fire inspection. In addition, the Rule states the inspection must be on the form provided by the Division and mailed or emailed to the Consultant within seven (7) days. Please refer to Child Care Rules 10A NCAC 09 .0304 (a), for more information regarding this rule. Choking Hazards: A violation was cited during today’s visit for items that were potential choking hazards accessible to children under three years of age. I recommended teachers remove all materials accessible to children that could be a choking hazard in spaces used for children under three years of age. I discussed with administrators to have staff check their classrooms for safety hazards prior to caring for children for the day to ensure no choking hazards are accessible to children. Additional information regarding plastic bags, toys, toy parts small enough to be swallowed, and materials that can be torn apart can be found in NC Child Care rule 10A NCAC .0604 (q) Observation Suggestion: I encouraged teachers to continue to model positive and respectful ways of engaging children, by standing close to them, reflecting their emotions, and by responding to their attempts to communicate both verbally and nonverbally. Respectful language was used, such as addressing each child by their name and moments were taken to check-in with each child during activities to see how they were doing and what their needs are. An infant was being held for bottle feeding. One caregiver was changing one infant’s diaper. Handwashing routines were observed. In Space #7, the children created individualized art projects using red paint, built with blocks, and used musical instruments. While outdoors, children chased each other, rode cars, and were walking around the playground. Lunch served was chicken alfredo, broccoli, orange slices, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Fox. The following violations were observed: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. Staff records were not available for seven (7) new staff hired after April 2023.
