The previous Annual Compliance Visit was completed February 24, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 96%. This program currently operates with a Four -Star License issued February 27, 2020. The License was based on the program earning two points in education standards, seven points in program standards, and one quality point by choosing to meet a programmatic option for: Has reduced group size by one in each group from the seven-point level as required in Rule .2818(c) The information on record with DCDEE was verified as correct by Ms. Amanda Weaver, Lead Teacher and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed September 04, 2025. Fire Classification of Approved completed March 06, 2025. Ms. Weaver was present and accompanied me while observing the indoor areas. Inside the children played in the dramatic play area pretending to cook, holding the babies, stacking blocks, playing with animals, coloring, manipulatives, puzzles, and using the magnetic squares. After playing indoors, the children cleaned up their toys and transitioned to the next activity. The children completed toileting and handwashing routines. The children put on their jackets and transitioned outdoors to play. Lunch served was teriyaki dumplings, seasoned broccoli, carrots with dressing, chilling peaches, and milk. A violation was observed today for storage of hazardous items. The most recent fire drill was completed January 09, 2026, and the shelter-in-place drill was completed October 17, 2025. The outdoor inspection form was completed January 16, 2026. At the completion of the visit, the Visit Summary was reviewed with Ms. Weaver. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #1, there were two bottles of dish soap and a bottle of glycerin stored on a shelf inside of a cabinet, less than 5 feet from the floor. In addition, there was one container of Clorox wipes stored on a shelf inside of a different cabinet. This was corrected by being place of top of a cabinet that was five feet from the floor. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today were corrected during today’s visit. A compliance letter is not required. Facility Specific TA/Consultation/Reminders: Storage of Hazardous Items A violation was observed today for Storage of Hazardous Items. Store items that are labeled “keep out of reach of children” and have no other warning can be stored a shelf or in an unlocked cabinet. The shelf or cabinet must be mounted at least five feet above the floor. There was one container of Clorox wipes stored on a shelf less than 5 feet from the floor. In another cabinet, there were two bottles of dish soap and glycerin stored on a shelf less than five feet from the floor. It is important to store these items out of reach for children to prevent consumption of poison/harmful chemicals. There is a handout for Hazardous Items Storage for Child Care. The Children were observed playing on the large play equipment and with sand toys. After outdoor play the children transitioned back to the classroom where the participated in handwashing routines and free choice play. The children then transitioned to free choice play where they were observed playing with blocks, dress-up clothes, reading books, and free choice art materials. The breakfast served today was cheese sticks, graham crackers, juice, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Weaver. The following violations were observed. While monitoring four children files two of the files did not have a dental screening within 30 days of after children entered into the classroom. .3005 (a)(5) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. One staff member was eating cheese stick pretzels during group time. this was not a specified meal time for children. This was corrected by teacher putting the bag of pretzels away in her personal bag. .0901(i) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The children transitioned to centers and were observed: coloring, connecting foam letters, building with blocks, connecting shapes, and dressing up in dramatic play. Toileting and handwashing routines were observed. Lunch served was chicken sandwich, mashed potatoes, mixed green salad, pears, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Goodwin. The following violations were observed: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Space #1, there was one outlet that did not contain a safety plug. This was corrected and the outlet was covered with a safety plug. 10A NCAC 09 .0604(c) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There was one container of pain reliever stored on a cabinet less than 5 feet from the floor. This was correct by storing the medication in a locket cabinet. 15A NCAC 18A .2820(d) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The Emergency Information Form was not on file for (R.M. date of use in classroom 9/13/2023). This was corrected. The staff member completed the Emergency Information Form and the copy is now on file. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. Prior to employment, a Criminal Background Check was not completed for Rebecca Masters, date of use in the classroom 9/13/2023. The children were engaged in the activities that were selected. The children also talked to their peers. When expanding on activities, it is important to provide books and encourage the use of literature. It is important to make sure that reading is communicated to the children, as exciting and fun. In addition, add books that focus on children’s interest. Talk to the children about the books and provide an activity that correlates with the book. The teacher can also provide paper bags with materials and allow children to create puppets to reassemble a character in the book. Electrical Outlets: A violation was cited today for electrical outlets. This was corrected during today’s visit. Today, we talked about different strategies to ensure that the facility is operating in compliance with Child Care Rules. Before the children arrive, it is best practice to conduct a walk-though of the classroom to make sure that all electrical outlets are covered, in addition, make sure the area used by children is safe from all hazards and potential hazards. Refer to Child Care Rule: 10A NCAC 09 .0604 for more information regarding this rule requirement. Storage of Medication: The violation for storage of medication was cited today. In Space #1, there was one container of pain reliever stored on a cabinet, less than 5 feet from the floor. This was corrected during today’s visit. Properly storing medication is important because if in reach of children, may present a poison hazard. I shared that additional information regarding the storage of medication can be found in Sanitation Rule .2028(d) and provided you with a copy of the “Hazardous Items Storage for Child Care” resource from our website. We also discussed the following strategies for achieving and maintaining compliance: review the resource with staff at your next staff meeting; have staff practice administering medicine and return to locked storage before completing the administration record (practice helps create and reinforce a routine); remind each team of staff to check behind the other; and/or periodically visit classrooms throughout the day and check medication boxes to ensure they are locked. Staff Files: A violation was cited today, because there were a variety of required items missing from one staff file. Prior to employment, it is best practice to ensure the Staff File Checklist is completed. The items required are notated on the form and it also lists timeframes for other required documents. This helps to determine if all items are on file and available for review. In addition, make sure that you have the most recent Staff File Checklist for Centers. The Division of Child Development and Early Education (DCDEE) website provides updated forms and resources. A blank copy of the Staff File Checklist can be located by visiting https://ncchildcare.ncdhhs.gov, The form can be found by selecting Provider tab then select Provider Documents and Forms. Please refer to Child Care Rule 10A NCAC 09 .0701, 10A NCAC 09 .0704, and10A NCAC 09 .2408 for more information regarding this rule requirement. Criminal Background Check (CBC): A violation was cited for Criminal Background Check (CBC). Ms. Rebecca Masters, Teacher, does not have a qualified CBC on file with DCDEE. Ms. Masters will need to complete the requirements for an updated CBC. If there is not a qualified CBC on file after two weeks, the employee is not permitted to work without a qualified CBC. I did advise in the future, to complete the requirements in advance to ensure the qualification letter is received prior to the expiration date. The previous Annual Compliance Visit was completed March 13, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 95%. This program currently operates with a Four -Star License issued February 27, 2020. The License was based on the program earning two points in education standards, seven points in program standards, and one quality point by choosing to meet a programmatic option for: Has reduced group size by one in each group from the seven point level as required in Rule .2818(c) The information on record with DCDEE was verified as correct by Ms. Margarat Goodwin, Teacher and the owner’s corporation status was verified on the Secretary of State’s website as current-active. The children transitioned to centers and were observed: coloring, connecting foam letters, building with blocks, connecting shapes, and dressing up in dramatic play. Toileting and handwashing routines were observed. Lunch served was chicken sandwich, mashed potatoes, mixed green salad, pears, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Goodwin.
