The previous Annual Compliance Visit was completed November 21, 2023, and prior to today’s visit, the eighteen-month compliance history score was 96%, which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). Your program currently operates with a five-star license issued August 18, 2017. The License was based on the program earning seven points in education standards, seven points in program standards, and one quality point by having a combined turnover rate of 20% or less for administrator, program coordinator, lead teacher, and group leader over the last 12 months. The information on record with DCDEE was verified as correct by Earlene White. Current Inspections are as follows: Sanitation Classification of Superior completed July 29, 2025. A copy of the sanitation inspection was received during today’s visit. The information was put in the Regulatory system. Fire Classification of Approved completed January 2, 2025. Earlene White, Operator, was present while I observed the indoor and outdoor areas. Upon my arrival, there were three (3) children in care. The children were observed sitting on the carpet with their teacher playing with airplanes, people, and dinosaurs. There was plenty of back – in – forth exchanges. For example, a child was asked, “What are you feeding your baby?” Many questions were asked about what children were doing. Children’s answers were repeated. Another child was sitting with his teacher, writing his name. The teacher named the letters of the child’s name. I observed diaper changing, toileting, hand washing, sanitation, lunch, and rest time. Children participated in counting, animal and color recognition. They played with dramatic play materials like dolls, blankets, and pretend food. Another teacher asked a child about what she was cooking. The child said, “Pork Chops”. The teacher repeated with the child said and asked if her mommy cooked at home. Lunch consisted of hot dogs, green beans, pears, whole wheat bread, and milk. At the completion of the visit, the Visit Summary was reviewed with E. White. The following violations were observed. Violation Number Comment Rule 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The operator's CPR training certification expired on 9/16/25. .1102(d) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. I observed toileting, handwashing, sanitation, and preparation for rest time. Children slept on mats. The mats were covered with sheets and spaced at least eighteen inches apart. Lunch consisted of chicken bologna, pears, corn, whole wheat crackers, and milk. At the completion of the visit, the Handwritten Visit Summary was reviewed with E. White. Due to technical issues with my computer, the Visit Summary could not be completed. Therefore, once completed, a Visit Summary will be emailed and mailed to you. There was one violation observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Children went into the block area, dramatic play, and tabletop area. In blocks a child used Duplos and Cardboard boxes to build a tower. A teacher named the color of blocks the child used to build her tower. Children at tabletops used a tree house with animals (polar bear, lion, and leopard). In the dramatic play are, children pretended to cook with skillets, pots, plates, and pretend food like, pizza, taco, lettuce, tomato, and broccoli. Ms. Earlene asked a child about the number of skillets and pots used. Children were given praised and affirmation as they played and answer questions. A child and a teacher talked about liking to eat broccoli. I observed diaper changing, toileting, sanitation, and hand washing. Lunch will consist of chicken hot dogs, wheat buns, black beans, apple sauce, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. White. The following violations were observed. Violation Number Comment Rule 218 Outdoor play area did not provide an area that is shaded. Next, Ms. Earlene described and demonstrated going into the bathroom to wash hands. They counted to twenty in English as they pretended to wash their hands and counted to ten in Spanish pretending to rinse their hands. Children were told to choose and activity center. Children went into the block area, dramatic play, and tabletop area. In blocks a child used Duplos and Cardboard boxes to build a tower. A teacher named the color of blocks the child used to build her tower. Children at tabletops used a tree house with animals (polar bear, lion, and leopard). In the dramatic play are, children pretended to cook with skillets, pots, plates, and pretend food like, pizza, taco, lettuce, tomato, and broccoli. Ms. Earlene asked a child about the number of skillets and pots used. Children were given praised and affirmation as they played and answer questions. A child and a teacher talked about liking to eat broccoli. I observed diaper changing, toileting, sanitation, and hand washing. Lunch will consist of chicken hot dogs, wheat buns, black beans, apple sauce, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. White. The following violations were observed. Violation Number Comment Rule 218 Outdoor play area did not provide an area that is shaded. There is no shaded area on the outdoor playground. 10A NCAC 09 .1402(d) 807 A safe indoor and outdoor environment was not provided for the children. There were ants and ant holes on the middle outdoor play area with bikes and the outdoor pink playhouse. 10A NCAC 09 .0601(a) 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility does not have a current sanitation inspection. The last inspection was dated 12/14/22. GS 110-91 The violation must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 PM on May 22, 2024, describing how and when the violation was corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. A violation was cited for not having shade on the outdoor area playground. You stated that you will have your son put a red shade on the playground. Refer to child care rule 10A NCAC 09 .1402(d). Observation Suggestions: Continue to use visual supervision of all children by talking to each other and moving about the classroom. Continue to show awareness, reacting quickly, and solving problems in a comforting way. Continue to facilitate positive peer interaction, modeling positive social interaction, and explaining children’s actions, intentions, and feelings to other children. Continue to smile and laugh with children showing enthusiasm and affection. You pay attention to children and notice their cues and when they may have difficulties and need assistance. The children appear comfortable and seeks support and interacts with you. Continue to resolve any issues that they cannot resolve on their own.
