The previous Annual Compliance Visit was completed August 06, 2024, and prior to today’s visit, the eighteen-month compliance history score was 92%. This program currently operates with a Five-Star License issued March 04, 2022. The License was based on the program earning seven points in education standards, six points in program standards, and one quality point by choosing to meet a programmatic option for: Has reduced staff/child ratios by one in each group from the seven-point level as required in .2818(c). The information on record with DCDEE was verified as correct by Ms. Zekiera Howard, Lead Teacher, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Provisional was completed July 25, 2025. A violation was observed for the sanitation report. Fire Classification was completed May 28, 2025. A violation was observed for fire inspection. A copy of the inspection report was received during the visit. Ms. Artis, Administrator, was present and accompanied me while observing the indoor and outdoor areas. Upon arrival, the children were sitting on the carpet with their caregiver. The caregiver talked to the children, and the children were able to respond to the caregiver’s questions. After group time, the children transitioned to free play in the classroom. The caregiver talked with the children and pretended to call for a fire truck. The children played with small insects, large trucks, blocks, dramatic play toys, and pretend play. When finished with free play, the caregiver played a “clean up” song and the children cleaned up their toys and transitioned to the carpet to dance. Toileting and handwashing routines were observed. In addition to other violations observed, a violation was observed for ABCMS staff roster. Lunch served was chicken nuggets, French fries, oranges, bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Howard. The following violations were observed: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The center has not received an approved or superior rating sanitation inspection. 10A NCAC 09 .0304(b) 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current fire inspection is dated for April 28, 2025, and the fire inspection was not received within one week of the inspection visit. On July 18, 2025, the fire inspection was confirmed to be completed on April 28, 2025, however, the fire inspection was not completed on a form provided by the Division. Prior to the completion of the visit, the fire inspection was completed on the form provided by the division. 10A NCAC 09 .0304(a) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the requirements for the ABCMS provider portal. The operator was unable to provide a staff roster. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on August 11, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. Today you stated that you have added some of your staff, but this is not current. Reminder: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. You must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. Fire Inspection: An annual Approved Fire Inspection is required annually and validates the building, premises, and programs are following current Fire Codes; all of which attest to a safe environment for children. During today’s visit, I received a copy of the fire inspection completed on the Fire Marshal’s Office Inspection Report. The fire inspection was dated for April 28, 2025. The inspection was not completed on the form provided by the Division, today, you contacted the Fire Marshall to obtain a copy of the Inspection Report. The Rule states the inspection must be on the form provided by the Division and mailed or emailed to the Consultant within seven (7) days. Please refer to Child Care Rules 10A NCAC 09 .0304 (a), for more information regarding this rule. Moodle: Moodle Trainings are available on our website regarding rule changes and other additional trainings. Upon arrival, the children were dancing on the carpet and singing. After singing and dancing, the teacher talked to the children on the carpet. The children were given a piece of paper and crayons. The children were instructed to draw body parts of a ladybug. The children were able to guess that it was a ladybug, when instructed to draw spots. Toileting and handwashing routines were observed. Lunch served was peanut butter, jelly, carrots, bananas, wheat bread, and milk. A violation was cited because the menu was not posted. In addition, a violation was cited for the fire inspection and the sanitation inspection, the inspections were not completed within one year. All violations cited were corrected during today’s visit. At the completion of the visit, the Visit Summary was reviewed with Ms. Howard. After finishing their snack, the children washed their hands. During handwashing, the children counted to twenty. At the completion of the visit, the Visit Summary was reviewed with Ms. Howard. The following violation was observed: Violation Number Comment Rule 9999 A violation was found for which there is no item number. The two sinks used by children, one in the bathroom and one in the classroom, were not tempered between 80 degrees Fahrenheit and 110 degrees Fahrenheit. The temperature for both sinks were measuring below 80 degrees and cold to touch. The staff member was able to adjust the water temperatures for both sinks and the water temperatures were measuring 83 degrees Fahrenheit and warm to touch for both sinks. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Facility Specific TA/Consultation/Reminders: Requirements for handwashing sinks: A violation was cited today because both sinks used by children were cold to touch during handwashing. The water temperature was measured and the temperature was below 80. The staff member was able to adjust the water temperature using the water meter. This was corrected. Be reminded that children are required to wash their hands in water tempered between 80 degrees to 110 degrees. This will help eliminate and decrease the spread of germs. Observations and Technical Assistance: During the observation today, the teacher talked to the children about the different things that start with the letter “B”. The children were able to help call out names that start with the letter “B”. The teacher talked to the children about the sounds and talked about the different things in the classroom that start with the letter “B”. The teacher can expand on this activity by providing a magazine that contains children appropriate photos. The children can use child size scissors and practice cutting pictures out that start with the letter “B”. The children can also create a collage of things that start with the letter ‘’B” and glue them on a piece of construction paper of their choice. This will help the children identify things that start with the letter “B” by showing a visual demonstration. In addition, the teacher can provide opportunities for the children to feel textures. The children can be provided a variety of sensory activities and the main objective would be to find things that start with the letter “B”.
