The previous Annual Compliance Visit was completed September 12, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 99% This program currently operates with a Four-Star License issued October 17, 2019. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: Has obtained an Infant/Toddler Certificate. The information on record with DCDEE was verified as correct by Ms. Gold Omogbehin, operator and the owner’s corporation status was verified on the Secretary of State’s website as current and active. Current Inspections are as follows: Sanitation Classification of Superior completed May 14, 2025. Ms. Gold was present and accompanied me while observing the indoor areas. Upon arrival, there was quiet music playing. Two children were sleeping on their cots. The children were provided with a sheet over the cot, and a blanket. There was one infant that was asleep in their crib, and there was one infant that was sitting on the carpet with a toy. Diapering procedures were observed. Snack served was animal crackers and juice. Violations were documented for emergency medical care plan, naptime procedures, and incident report. There are allegations of violations of child care requirements, including: • Supervision • General Safety for Naptime Procedures • Emergency Medical Care (EMC) Plan Procedures • Children’s Records The concerns were discussed with Ms. Gold and to inform her of the purpose of my visit. Interviews were conducted with the operator, Ms. Gold. Regarding the concerns for children are not adequately supervised, observations and interviews were conducted. Observations of the teacher and child interactions were observed. During today’s visit, there were four (4) children present. All children were adequately supervised. There were two children sleeping on their cots. There were two infants observed playing on the carpet, sitting in the infant seat, and the caregiver held the bottles for the infants during feeding. Soft toys were available for the children. Regarding the naptime procedures, observations and interviews were conducted. Upon arrival, there were two children sleeping on their cots, and two infants observed sleeping in their cribs. When the infants woke up, additional activities were made available for the infants. One of the infants was placed on the carpet to play with soft toys, and one infant was placed in an infant seat. The caregiver rotated each infant to serve the infants their bottles. The caregiver held the bottles for the infants. During the interviews, it was stated that a child was allowed to sleep on the carpet. Ms. Gold stated that following the injury, the child was crying, and the child cried until the child fell asleep on the carpet. The child stayed sleeping until their parent arrived to pick up the child. After falling asleep, Ms. Gold stated the child slept for approximately seven (7) minutes. Regarding the concerns for EMC Requirements, observations and interviews were conducted. The emergency medical care (EMC) plan was reviewed, and the required information on the form was available for review. The EMC plan is located on the wall. The EMC plan stated that Ms. Gold would administer First Aid with injuries. During the interview, Ms. Gold stated that the child hit her head on the shelf, and the child’s injury was observed, and there was swelling. The caregiver did not administer first aid. Ms. Gold stated that the child cried on the carpet, until she fell asleep. An incident report was not completed. The parents were not notified. Based on the observations and interview information, the allegation for supervision is unsubstantiated. Based on the observations and interview information, the allegations for general safety for naptime procedures, EMC plan, and children’s records are substantiated. Violation Number Comment Rule 928 Operator did not follow the emergency procedures outlined in the emergency medical care plan. It was confirmed during interviews with Ms. Gold, that there was one child that hit their head on a shelf, and first aid was not administered. In addition, it was stated that a child had swelling on one part of the forehead. .1713(1-4) 2052 A copy of the incident report was not given to the parent. During interviews, it was confirmed that the child had swelling on the forehead, and an incident report was not completed, and/or given to the parent. .1721(e)(3) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. It was confirmed during interviews, that there was one child that became tired after an injury, and the child was allowed to sleep on the carpet, until the parent arrived to receive the child. 10A NCAC 09 .1718(a)(2) At the completion of the visit, the Visit Summary was reviewed with Ms. Gold. The following violations were observed: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on May 05, 2026, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. The operator stated that she communicates with the families if something is going on with their child. It is also recommended to extend an invitation for the parent to stop by at their convenience to check on their child. This allows an opportunity to build a trusting relationship with the parent. In addition, when contact is made with the parent, it is recommended to inform the parent of the child’s behavior after the injury. This may give the parent comfort knowing that their child is feeling better and has resumed activities or if the child may need the parent to examine their child. In addition, we talked about nurturing the child if an injury or incident occurs, if the child is still upset, this is an indication that maybe the parent may need to be called as this could result in a more serious injury such as, concussion. The previous Annual Compliance Visit was completed September 12, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 99% This program currently operates with a Four-Star License issued October 17, 2019. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: Has obtained an Infant/Toddler Certificate. The information on record with DCDEE was verified as correct by Ms. Gold Omogbehin, operator and the owner’s corporation status was verified on the Secretary of State’s website as current and active. Current Inspections are as follows: Sanitation Classification of Superior completed May 14, 2025. Ms. Gold was present and accompanied me while observing the indoor areas. Upon arrival, there was quiet music playing. Two children were sleeping on their cots. The children were provided with a sheet over the cot, and a blanket. There was one infant that was asleep in their crib, and there was one infant that was sitting on the carpet with a toy. Diapering procedures were observed. Snack served was animal crackers and juice. Violations were documented for emergency medical care plan, naptime procedures, and incident report. There are allegations of violations of child care requirements, including: • Supervision • General Safety for Naptime Procedures • Emergency Medical Care (EMC) Plan Procedures • Children’s Records The concerns were discussed with Ms. Gold and to inform her of the purpose of my visit. Interviews were conducted with the operator, Ms. Gold. Regarding the concerns for children are not adequately supervised, observations and interviews were conducted. Observations of the teacher and child interactions were observed. During today’s visit, there were four (4) children present. All children were adequately supervised. There were two children sleeping on their cots. There were two infants observed playing on the carpet, sitting in the infant seat, and the caregiver held the bottles for the infants during feeding. Soft toys were available for the children. Regarding the naptime procedures, observations and interviews were conducted. Upon arrival, there were two children sleeping on their cots, and two infants observed sleeping in their cribs. When the infants woke up, additional activities were made available for the infants. One of the infants was placed on the carpet to play with soft toys, and one infant was placed in an infant seat. The caregiver rotated each infant to serve the infants their bottles. The caregiver held the bottles for the infants. During the interviews, it was stated that a child was allowed to sleep on the carpet. Ms. Gold stated that following the injury, the child was crying, and the child cried until the child fell asleep on the carpet. The child stayed sleeping until their parent arrived to pick up the child. After falling asleep, Ms. Gold stated the child slept for approximately seven (7) minutes. Regarding the concerns for EMC Requirements, observations and interviews were conducted. The emergency medical care (EMC) plan was reviewed, and the required information on the form was available for review. The EMC plan is located on the wall. The EMC plan stated that Ms. Gold would administer First Aid with injuries. During the interview, Ms. Gold stated that the child hit her head on the shelf, and the child’s injury was observed, and there was swelling. The caregiver did not administer first aid. Ms. Gold stated that the child cried on the carpet, until she fell asleep. An incident report was not completed. The parents were not notified. Based on the observations and interview information, the allegation for supervision is unsubstantiated. Based on the observations and interview information, the allegations for general safety for naptime procedures, EMC plan, and children’s records are substantiated. Violation Number Comment Rule 928 Operator did not follow the emergency procedures outlined in the emergency medical care plan. It was confirmed during interviews with Ms. Gold, that there was one child that hit their head on a shelf, and first aid was not administered. In addition, it was stated that a child had swelling on one part of the forehead. .1713(1-4) 2052 A copy of the incident report was not given to the parent. During interviews, it was confirmed that the child had swelling on the forehead, and an incident report was not completed, and/or given to the parent. .1721(e)(3) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. It was confirmed during interviews, that there was one child that became tired after an injury, and the child was allowed to sleep on the carpet, until the parent arrived to receive the child. 10A NCAC 09 .1718(a)(2) At the completion of the visit, the Visit Summary was reviewed with Ms. Gold. The following violations were observed: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on May 05, 2026, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. For the most recent incident, the operator stated that everything happened so “fast” that she did not think about doing anything further. The operator stated that the child had just fell asleep and the parent arrived to pick up the child. The operator stated that she communicates with the families if something is going on with their child. Cots/Mats: A violation was cited today because the child was allowed to sleep on the carpet instead of allowing the child to sleep on a cots/mats. During today’s visit, two children were placed on their cot, and two children were observed sleeping in their crib. Emergency Medical Care Plan: A violation was cited today for Emergency Medical Care Plan (EMC) not being followed. The EMC is required to be posted. In the event of an emergency, the EMC is a tool that can be used as a reference. When changes are made to the emergency medical care plan, the date should reflect the most recent change to the emergency medical care plan. I would need to verify that the staff member has a qualifying criminal background check on file. As of today, you stated by telephone that you will be closing your center location in Raeford. I also informed you that you will need to contact all of your parents to pick up there children. Today you stated that you plan to arrive by 4 pm. Criminal Background Check (CBC) Gladys Manye, Substitute Staff Member, does not have a qualified CBC on file with DCDEE. Ms. Manye stated that she started employment on April 28, 2025. Ms. Manye must not be on the premises and may return to work once qualified CBC is on file. She was asked to leave the premises at this time. I was able to verify that Ms. Manye had begun the process, however, a qualifying letter is not active and a determination had not been made as of 5/2/2025. Ms. Manye was the only caregiver present at the facility.The CBC database was checked today, and the status is not available. Ms. Manye may return once he received his qualification letter. Refer to Child Care Rule 10A NCAC 09 .1729, for more information regarding this rule requirement. Temperature of FCCH: During today’s visit, the temperature felt very warm. Today you stated by telephone that you have an AC unit that you used. During today’s visit the staff member did not know about the AC unit and the staff member turned on the fan. Remember that the Family Child Care Home will need to be well ventilated. I was unable to determine the temperature in the facility because there was not a thermostat. I recommend having a device that will notify you of the temperature. You stated that when you turn the AC on it will be set at a set temperature. Once the staff member turned the AC on it was set on 74 and the unit was running. Staff Files: A violation was cited today, because a staff file was not available for review. Prior to employment, it is best practice to ensure the Staff File Checklist is completed. The items required are notated on the form and it also list timeframes for other required documents. This helps to determine if all items are on file and available for review. The previous Annual Compliance Visit was September 25, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 96% This program currently operates with a Four-Star License issued October 17, 2019. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: Has obtained an Infant/Toddler Certificate. The information on record with DCDEE was verified as correct by Ms. Gladys Manye, substitute, and the owner’s corporation status was verified on the Secretary of State’s website as current and active. Current Inspections are as follows: Sanitation Classification of Superior completed September 12, 2024 Ms. Manye was present and accompanied me while observing the indoor areas. I observed two children sleeping on their cots and one child was playing on their tablet, and one child was awake on their cot. A violation was observed for screen time, temperature, and staff records, Criminal Background Check. Quiet music was playing. The caregiver and the children cleaned their things, completed toileting and handwashing, and then transitioned to the table for snack. The snack served was animal crackers and milk. A checklist was used to note the requirements I monitored today. The Statements of Responsibility form was left with you today. This form is completed at every visit, however, Ms. Gold Omogbehin was not available to complete. At the completion of the visit, the Visit Summary was reviewed with Ms. Manye. The following violations were observed: Violation Number Comment Rule 410 Copy of the required information for an individual was not on file in the home available for review. The required information for a substitute employed on April 28, 2025, was not available for review. .1702(b)(1-9) 918 Prior to employment, a Criminal Background Check was not completed. Prior to employment, a Criminal Background Check was not completed for Gladys Manye. The date of employment was April 28, 2025. G.S. 110-90.2; .2703(a) 1820 The temperature in a room where infants aged 12 months or younger where sleeping exceeded 75 degrees. The temperature in the room exceeded 75 degrees. This was corrected during the visit on 5/12/2025. Upon arrival, the temperature in the room did not exceed 75 degrees. .1724(a)(6) 1831 A valid qualification letter was not on file and available for review at the facility. A valid qualification letter was not on file for Gladys Manye. Screen time was provided for more than thirty minutes. There was one child that was using the tablet for (45) forty minutes. The child was provided a different activity when I informed the staff member of the Child Care Requirement for screen time. Your current enrollment shows that you have six (6) children enrolled. During your last annual compliance visit, you completed the Operator’s Statement, and you signed your initials, agreeing to the statements. I have also left a copy for you to review. Be reminded of the following, but not limited to: • I understand that as the licensee, I will be the person on-site providing care to children. (FCCH Only) • I understand that if I choose to have another individual in my home during operating hours, that individual must meet all applicable qualifications in the North Carolina Child Care Rules and Law, must have a valid criminal history qualification letter, and must be identified in my written plan of care. In addition to our phone conversation, we were able to confirm that you are currently working at your other facility. I informed you that you will need to have a staff member with a qualifying letter and since, at this time, you do not have another staff member with a qualifying letter, I asked you which facility will you be closing until you get a qualified. You will need to be present at the facility or a qualified staff member. I also requested for you to provide me your staff members with details that you plan to work in your family child care home. I would need to verify that the staff member has a qualifying criminal background check on file. As of today, you stated by telephone that you will be closing your center location in Raeford. I also informed you that you will need to contact all of your parents to pick up there children. Today you stated that you plan to arrive by 4 pm. Criminal Background Check (CBC) Gladys Manye, Substitute Staff Member, does not have a qualified CBC on file with DCDEE. Ms. Manye stated that she started employment on April 28, 2025. She worked on April 28, 2025, May 1, 2025, May 2, 2025. Ms. Manye is not permitted to work without a qualified CBC. Ms. Manye must not be on the premises and may return to work once qualified CBC is on file. She was asked to leave the premises at this time. I was able to verify that Ms. Manye had begun the process, however, a qualifying letter is not active and a determination had not been made as of 5/2/2025. Ms. Manye was the only caregiver present at the facility.The CBC database was checked today, and the status is not available. Ms. Manye may return once he received his qualification letter. Refer to Child Care Rule 10A NCAC 09 .1729, for more information regarding this rule requirement. Temperature of FCCH: During today’s visit, the temperature felt very warm. Today you stated by telephone that you have an AC unit that you used. During today’s visit the staff member did not know about the AC unit and the staff member turned on the fan. Remember that the Family Child Care Home will need to be well ventilated. I was unable to determine the temperature in the facility because there was not a thermostat. I recommend having a device that will notify you of the temperature. You stated that when you turn the AC on it will be set at a set temperature. Once the staff member turned the AC on it was set on 74 and the unit was running. Staff Files: A violation was cited today, because a staff file was not available for review. Prior to employment, it is best practice to ensure the Staff File Checklist is completed. The items required are notated on the form and it also list timeframes for other required documents. This helps to determine if all items are on file and available for review. The previous Annual Compliance Visit was completed January 10, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 99% This program currently operates with a Four-Star License issued October 17, 2019. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: Has obtained an Infant/Toddler Certificate. The information on record with DCDEE was verified as correct by Ms. Gold Omogbehin, operator and the owner’s corporation status was verified on the Secretary of State’s website as current and active. Current Inspections are as follows: Sanitation Classification of Superior completed July 07, 2023. Ms. Gold was present and accompanied me while observing the indoor and outdoor areas. I observed children in the playroom of the home, playing on the carpet with age-appropriate materials. The children were drawing on the chalk table. Free choice materials were available. The caregiver talked to the children and helped direct the children to new activities. The diaper changes were observed. Handwashing routines were observed. Violations were cited today for missing forms from the children’s files. Lunch served was turkey, lettuce, bananas, bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Omogbehin. The following violations were observed: Violation Number Comment Rule 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. The discipline policy was not signed and dated for seven out of nine files reviewed. G.S.110-91(10); .1727(a)&(b) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. The Summary of the NC Child Care Law signed statement was not on file for five of the nine files reviewed. GS 110-102 1828 The Family Child Care Home operator did not discuss the operational policies with parents on or before the child's first day of attendance. The operational policies did not have a signed statement confirming the policies were discussed for five out of nine files reviewed. 10A NCAC 09. 1715(b) 2032 The signed statement regarding receipt of the shaken baby syndrome and abusive head trauma policy did not contain the required information. The shaken baby syndrome and abusive head trauma policy was not signed for five out of five files reviewed. .1726(b)(1-6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on June 28, 2023, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. We reviewed the FCCER-S book and I suggested reaching out to select agencies to obtain an updated copy of the FCCER-S book. The Partnership for Children is also a resource that can be used to help with obtaining an updated copy of the FCCER-S book. We reviewed the website together and talked about the webinar and other resources. The website also provides other forms that can be used to complete your self-study. There is not a required specific form or format that is required. You do have the flexibility to select and create your self-study. Be reminded to make sure to document your efforts during the 3-month process. I provided recommendations for your focus areas for your self-study to include teacher/child interactions, room arrangement, and health and safety topics. At this time, you stated that you are not interested in having the scales assessment for your Family Child Care Home. If you decide that you would like to have the FCCER-S Scales completed, please notify me so I can provide additional resources for you.
