The previous Annual Compliance Visit was completed October 07, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 96%. This program currently operates with a Three-Star License issued December 19, 2019. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program he information on record with DCDEE was verified as correct by Ms. Jayla Melvin, Lead Teacher, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Approved completed July 11, 2025. Fire Classification of Approved completed November 17, 2024. Ms. Melvin was present and accompanied me while observing the indoor and outdoor areas. In Space #1, the children observed sitting at the table. The caregiver was showing cards, and the children were identifying the colors. After identifying the colors, the caregiver played a “I spy” game with the children. In Space #3, one child was asleep in their crib, and two children were walking around exploring their space. Lunch served today was hotdogs, string cheese, baked beans, oranges, wheat bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Melvin. The following violation was observed: Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. In Space #2, the refrigerator did not maintain a temperature of 45 degrees F. 15A NCAC 18A .2806(j)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00pm on September 19, 2025, you must submit a written, dated, and signed statement to me. I provided technical assistance on the importance of making sure your EPR plan is printed and available for review. In addition, the EPR Plan should be in the same location as the Ready to Go file. We talked about accessing on the website the checklist that will give detailed information on the requirements for the Ready to Go file. EPR plans will need to be updated yearly. If no changes are needed, make sure to just print the cover page and a date will generate. The EPR plan is helpful to have to help assist with emergencies as an easy reference. Refer to Child Care Rules GS 110-91 (9) and 10A NCAC 09 .0607, for more information regarding this rule requirement. Shelter-In Place or Lockdown Drill Log: Shelter-in-place drills will need to be completed every three months. We discussed placing dates on the calendar for upcoming due dates for drills to ensure that drills are conducted as indicated in Rule 10A NCAC 09 .1719(a)(16) &.1721(e)(7). During today’s visit, you were able to complete a lock down drill. The information was recorded on the log. Choking Hazards/Plastic/Rubber/Wipes: A violation was not observed today however, technical assistance was provided today for plastic, gloves, and wipes. A violation could be warranted if the following items continue to be accessible to children: We talked about how plastic could be a choking hazard. It is best to remove the plastic and store it in an alternate place. If items are stored in plastic, it is important to make sure items are removed out of the plastic. I suggest using bins for storage or installing a shelf/cabinet. For the diapers, the diapers should be removed from the package and placed on the shelf or another location. Both wipes and gloves will need to be stored greater than 5 feet. The staff members will need to ensure that these items are out of the reach of children. The diaper packaging and gloves could be a potential choking hazard if consumed and the wipes have chemicals that could be harmful to children if consumed. It is best to remove the plastic and store it in an alternate place. If items are stored in plastic, it is important to make sure items are removed out of the plastic. I suggest using bins for storage or installing a shelf/cabinet. For the diapers, the diapers should be removed from the package and placed on the shelf or another location. Both wipes and gloves will need to be stored greater than 5 feet. The staff members will need to ensure that these items are out of the reach of children. The diaper packaging and gloves could be a potential choking hazard if consumed and the wipes have chemicals that could be harmful to children if consumed. Screen Time: We discussed keeping a log to document screen times or using activity plans to document screen times. We discussed requirements when providing screen time for children. Children under the age of three years old are prohibited from screen time. If screen time is offered to children, the following would need to occur: • not permitted for children under the age of three • no more than 30 minutes per day and no more than a total of two and a half hours per week • screen time will need to be offered to stimulate a developmental domain in accordance with the North Carolina Foundations for Early Learning and Development as referenced in Rule .0508 We discussed options for documenting screen times. If you decide to use a cumulative log, this will need to be completed when screen time occurs. If you decide to use an activity plan, it will need to be documented and available for review. Transportation Requirements: The transportation requirements were not monitored during today’s visit due to the vehicle currently not on the premises. Review the website below that also offers Facts and Questions, information for transportation vehicles, and child safety information http://www.buckleupnc.org/occupant-restraint-laws/seat-belt-law-summary/. When a child safety seat is required, it is important to ensure that the restraint seat is used according to the manufacturing requirements. Be reminded, car seats and booster seats both have expiration dates. This is important in maintaining compliance for the safety of children. In each transportation vehicle, it is required to include a photograph of each child using transportation. On the emergency contact form including a photograph helps identify each child being transported. In addition, first aid kits are required. I have provided some information, but not limited to the items reviewed during today’s visit. Please review the Child Care Rules 10A NCAC 09 .1003 to stay updated on transportation requirements. Outdoor Learning Environment: The fencing may be near the time of replacement. The storage shed will need to be replaced. There is a post that is hanging from the top. This was out of the reach of children and not accessible. In addition, make sure the loose fill is the required amount for your equipment. The staff members can use rakes to bring the sand to the required amount. It may be best practice to add more sand to the area to ensure the children are safe from fall zones. We discussed placing dates on the calendar for upcoming due dates for drills to ensure that drills are conducted as indicated in Rule 10A NCAC 09 .0302(d)(5), (d)(8). Screen Time: We discussed keeping a log to document screen times or using activity plans to document screen times. We discussed requirements when providing screen time for children. Children under the age of three years old are prohibited from screen time. If screen time is offered to children, the following would need to occur: • not permitted for children under the age of three • no more than 30 minutes per day and no more than a total of two and a half hours per week • screen time will need to be offered to stimulate a developmental domain in accordance with the North Carolina Foundations for Early Learning and Development as referenced in Rule .0508 We discussed options for documenting screen times. If you decide to use a cumulative log, this will need to be completed when screen time occurs. If you decide to use an activity plan, it will need to be documented and available for review. Transportation Requirements: The transportation requirements were not monitored during today’s visit due to the vehicle currently not on the premises. When a child safety seat is required, it is important to ensure that the restraint seat is used according to the manufacturing requirements. In each transportation vehicle, it is required to include a photograph of each child using transportation. On the emergency contact form including a photograph helps identify each child being transported. In addition, first aid kits are required. I have provided some information, but not limited to the items reviewed during today’s visit. Please review the Child Care Rules 10A NCAC 09 .1003 to stay updated on transportation requirements. Outdoor Learning Environment: The fencing may be near the time of replacement. The storage shed will need to be replaced. There is a post that is hanging from the top. This was out of the reach of children and not accessible. In addition, make sure the loose fill is the required amount for your equipment. The staff members can use rakes to bring the sand to the required amount. It may be best practice to add more sand to the area to ensure the children are safe from fall zones. The previous Annual Compliance Visit was completed December 07, 2022, and prior to today’s visit, the eighteen-month compliance history score was at 81%. This program currently operates with a Three-Star License issued December 19, 2019. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program he information on record with DCDEE was verified as correct by Ms. Jayla Melvin, Lead Teacher, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Approved completed January 31, 2023. Fire Classification of Approved completed November 08, 2023. Ms. Melvin was present and accompanied me while observing the indoor and outdoor areas. In Space #2, five children were sitting on the carpet watching television. The teacher transitioned the children to the table for a table activity. A violation was cited for screen time because there were children present under the age of three. In addition, two other violation were cited today. In Space #3, there was one infant sleeping in their crib and there were two infants sitting in infant seats. In Space #4, the children were on the carpet playing with manipulatives and trucks. Lunch served today was roast beef, cheese, fries, oranges, bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Melvin. The following violations were observed: Violation Number Comment Rule 544 Screen time was offered to children under three years of age. In Space #2, screen time was offered to two (2) children under three years of age. The ages of the children were two (2) years old. The television was turned off and the children were provided another activity. .0510(f) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Photographs were not attached for five of the eight children that receive transportation.
