The previous Annual Compliance Visit was completed October 2, 2024, and prior to today’s visit, the eighteen-month compliance history score was 97% above the 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four- Star License issued November 21, 2023. The License was based on the program earning 7 points in education standards, 2 points in program standards, and one quality point by choosing to meet a programmatic option by having infant capacity restricted to no more than two children under age one. The information on record with DCDEE was verified as correct by Rhonda Cooper. Inspections are as follows: Sanitation Classification of Superior completed May 7, 2025. R. Cooper, Owner/Operator, was present and accompanied me while observing the indoor and outdoor areas. One preschool-aged child arrived at the facility when school dismissed. The operator greeted the family and assisted the child with completing personal care routines and hand washing. The operator served snack which consisted of whole grain Cheerios and milk. The child washed their hands and played in activity centers. The child was observed manipulating colored magnetic blocks and built towers and bridges. The operator and child discussed and identified the colors of each block as they played on the carpeted floor. A checklist was used to note the requirements monitored during the visit. The Verification of Required Information for Operator and Additional Caregivers and Statements of Responsibility forms were completed during today’s visit. One child’s file was monitored. The most recent fire drill, monthly outdoor inspection, and the quarterly lockdown drill were all completed September 2, 2025. At the completion of the visit, the Visit Summary was reviewed with R. Cooper. The following violation was observed: Violation Number Comment Rule 916 Operator did not maintain a record of on-going training in which he/she has participated. The on-going training document for the operator was not updated with most recent completed trainings. Cooper read the book “Good Night Gorilla” For lunch, children were served whole wheat spaghetti with meat sauce, corn, applesauce, and milk. I observed the operator and staff discussing the healthy meal the children were eating. A checklist was used to note the requirements I monitored today. The Verification of Required Information for Operator and Additional Caregivers and Statements of Responsibility forms were completed during today’s visit. One child’s record was monitored. The most recent fire drill was completed September 19, 2024 and the most recent shelter-in-place drill was completed July 30, 2024.
