The last annual compliance visit was completed December 10, 2025, and prior to today’s visit, the eighteen-month compliance history score was 95%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued December 12, 2025. The License was based on the program earning six (6) points in education standards, two (2) points in program standards, and one (1) quality point by having the infant capacity restricted to no more than two children under one year of age. The information on record with DCDEE was verified as correct by Tabatha Pass. Current Inspections are as follows: Sanitation Classification of superior completed October 9, 2025. I was greeted by Tabatha Pass, Operator. Four children were present in care. The children were observed engaging in puzzle activities. One child counted to ten with Ms. Tabatha, identified his left and right hands, and recognized the word “left,” reciting the letters aloud. I observed toileting, diaper changing, handwashing, sanitation practices, lunch, and rest time. Lunch consisted of sausage, Macaroni and cheese, cauliflower, applesauce, and milk. A four month old child was observed being held and fed with a bottle. During transitions to the restroom and while lunch was being prepared, the children played in a ball pit and engaged in positive communication with Ms. Tabatha. During a group activity, the phrase “Down by the pond, what do I see?” was read aloud. The children shared their ideas about the phrase (frog), made frog sounds, spelled the word “frog,” and discussed how frogs hop. During lunch they discussed what they were eating. At the completion of the visit, the Visit Summary was reviewed with T. Pass. The following violation was observed: Violation Number Comment Rule 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The operator did not have a current CPR training certification on file. Operator has a North Carolina Family Child Care Home Credential equivalency 60/12, at least 18 hours ECE/CD, and more than 4 years EC experience. • The Gee Whiz curriculum and Learning Everyday embedded formative assessments are used with children. • Formative assessments for children, annual educational opportunity with families, coaching and training opportunities for the operator will be reviewed during the next Annual Compliance visit. Based on the information provided, the facility will receive a Five-star license for the Classroom & Instructional Quality Pathway. The completed packet will be submitted for review and processing. Children were observed playing with various developmentally age-appropriate materials like colored shape puzzles, and a playhouse. We observed diaper changing, sanitation, toileting, hand washing, lunch, and rest time. Today’s lunch consisted of Vegetable soup with green beans, carrots, and peas, apple sauce, crackers, and milk. Children were observed saying their colors, numbers, alphabets, and emotions using charts that were posted on the wall in the hallway leading to the bathroom. At the completion of the visit, the Visit Summary was reviewed with T. Pass. The following violation was observed: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not create an ABCMS Roster for the facility. G.S. 110-90.2 & .2703(r) Child Care programs are always expected to achieve and maintain compliance. The operator has requested to increase the facility’s capacity to 10 children. QRIS Modernization Pathway: During today’s visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation you will be pursuing the Classroom and Instructional Quality Pathway 2. ABCMS – CBC Provider Portal Reminder: The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff and house hold members sixteen years of age and older are noted on the roster. I was greeted by Sierra McCain, substitute. Tabatha Pass, Owner, was present and assisted me while I was observing the indoor and outdoor areas. Upon my arrival, there were two (2) children in care. Children were observed sitting on a child-sized couch. Next, they played with a table sized beads maze, sliding various colors of beads on the maze. A child was observed, looking out of a picture window in the child care area. I observed diaper changing, hand washing, sanitation, and lunch. Lunch consisted of spaghetti, broccoli, cauliflower, bread, and milk. Positive communication was used. Ms. Pass named the items on a child’s plate. The child repeated the items and said ummm good. Ms. Pass said delicious and said the word in Spanish. When the child finished his broccoli, she said, “excellent”. At the completion of the visit, the Visit Summary was reviewed with T. Pass. The following violation was observed: Violation Number Comment Rule 519 Individuals who provide care for less than 5 hours a week, during planned absences of the operator, had not completed first aid and CPR as described in Rule .1702(b)(2). A substitute working during planned absences did not have a current First Aid Certification on file. .1729(b) Child Care programs are always expected to achieve and maintain compliance. This program currently operates with a Three-Star License issued July 9, 2024. The information on record with DCDEE was verified as correct by Tabatha Pass. Current Inspections are as follows: Sanitation Classification of superior completed November 15, 2024. During an Annual Compliance visit on December 17, 2024, violations were cited for the operator not completing Recognizing and Responding to Suspicions of Child Maltreatment and Health and Safety Trainings in a required timeframe. A compliance letter was not received by the due date, December 31, 2024. Therefore, a visit was conducted to verify violations Item #2019 and #2022 were corrected. Tabatha Pass, Owner, was present and assisted me while I verified violations were corrected. Upon my arrival, there were three (3) children in care. Ms. Tabitha and the children were having any conversations. For example, Ms. Tabitha named the children present and children pointed and tried repeating after Ms. Pass. Children played with babies. Ms. Pass gave children ideas like, laying the baby on the sofa and reading the babies a book. Children were observed crawling, walking, climbing, they used materials like airplanes, stuffed animals, books, a large empty cardboard box, bouncy horse/turtle and musical toys. Ms. Pass read a story to the children and asked them to tell her about different foods. Lunch will consist of ground beef pizza, mixed vegetables, apple sauce, and milk. Violations item #2019 and #2022 were verified not corrected. Violations were cited again during today’s visit. At the completion of the visit, the Visit Summary was reviewed with T. Pass. The previous temporary time period visit was completed February 13, 2024, and prior to today’s visit, the eighteen-month compliance history score was 100%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a temporary license effective, January 8, 2024 to July 8, 2024. The information on record with DCDEE was verified as correct by Tabatha Pass. It is an important part of licensure. Review children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed the importance of reviewing all children’s files to ensure accuracy with completion of all required areas of the child's application. You should not sign off on the document until all areas of the form have been addressed. We reviewed and discussed the resource from the DCDEE website entitled, “Children’s File Checklist FCCH,” to include the items required to be on file, the due date, and the date received/completed. I recommended that T. Pass use the checklist to monitor the information required to be maintained on file for each child’s record and to ensure all required information is on file by the due date. A preschool-age child enrolled on March 26, 2024, did not have the required information for Day 1 on file at the FCCH. Rated License Assessment: As discussed on February 13, 2024, the FCCH must transition to a 3-5 Star Rated License to continue to be eligible to receive subsidized child care assistance after six (6) months. An application for a star rated license was received today. T. Pass did not request to have the FCCERS-R conducted as part of the rated license assessment. Program Standards: We discussed completing a three (3) month self-study/assessment using the FCCERS-R on February 13, 2024. T. Pass is on track to complete the three-month self-study / assessment on or before July 1, 2024. As a reminder, to earn two points in program standards, you must complete a three-month self-study/assessment using the FCCERS-R. Education Standards: T. Pass stated she has not requested and submitted her official transcripts to the Workforce Education Unit for evaluation. T. Pass shall request a copy of her official college transcript from Guilford Technical Community College, Fayetteville Technical Community College, and Fayetteville State University, and submit them by mail or electronically to the Workforce Education Unit. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance Specific to Today’s Visit: Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed the importance of reviewing all children’s files to ensure accuracy with completion of all required areas of the child's application. You should not sign off on the document until all areas of the form have been addressed. We reviewed and discussed the resource from the DCDEE website entitled, “Children’s File Checklist FCCH,” to include the items required to be on file, the due date, and the date received/completed. I recommended that T. Pass use the checklist to monitor the information required to be maintained on file for each child’s record and to ensure all required information is on file by the due date. A preschool-age child enrolled on March 26, 2024, did not have the required information for Day 1 on file at the FCCH. Rated License Assessment: As discussed on February 13, 2024, the FCCH must transition to a 3-5 Star Rated License to continue to be eligible to receive subsidized child care assistance after six (6) months. An application for a star rated license was received today. T. Pass did not request to have the FCCERS-R conducted as part of the rated license assessment. Program Standards: We discussed completing a three (3) month self-study/assessment using the FCCERS-R on February 13, 2024. T. Pass is on track to complete the three-month self-study / assessment on or before July 1, 2024. As a reminder, to earn two points in program standards, you must complete a three-month self-study/assessment using the FCCERS-R. Education Standards: T. Pass stated she has not requested and submitted her official transcripts to the Workforce Education Unit for evaluation. T. Pass shall request a copy of her official college transcript from Guilford Technical Community College, Fayetteville Technical Community College, and Fayetteville State University, and submit them by mail or electronically to the Workforce Education Unit. Pass serves no more than two infants under the age of one. At the end of the temporary time period, points will be assigned, and a new license issued. Training: T. Pass shall complete the Emergency Preparedness and Response in Child Care training by January 8, 2025. T. Pass will have four (4) months from the date of completion to go online at https://rmp.nc.gov/portal/portal.aspx and complete the EPR Plan. As a reminder, you shall review the EPR Plan annually or when information in the plan changes to ensure all information is current. Additional caregivers and substitutes shall review the EPR Plan during orientation and/or on an annual basis with T. Pass. Documentation of the review shall be maintained on file. In Addition, T. Pass shall complete the CCDF health and safety trainings within twelve months of being licensed (January 8, 2025). The CCDF Health and Safety trainings are now available in the DCDEE Moodle Learning platform (https://www.dcdee.moodle.nc.gov/).
