The previous annual compliance visit was completed June 4, 2025, and prior to today’s visit, the eighteen-month compliance history score was 87%, which is above the required 75%. This program currently operates with a Notice of Compliance issued August 13, 2021. The information on record with DCDEE was verified as correct by Traica Geter, Administrator, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed October 14, 2025 Fire Classification of Approve for Daytime only completed April 14, 2026. Upon my arrival at 9:05 a.m., the entry door to the facility was propped open so I entered without assistance from staff. Two (2) staff and fifteen (15) children, aged ten months to five years old, were present in the fellowship hall, where meal service occurs. I introduced myself to the staff and asked if Tracia Geter, Administrator, was present. The staff stated that T. Geter was not present; however, they would contact her by telephone. Since the children were sitting and waiting for the next activity, I asked the teachers if they were getting ready to take the children back to their assigned classrooms. One of the teachers assisted the children with personal care and hand-washing routines. The teacher took a group of six (6) children to the bathroom, while the other teacher was left in the fellowship hall with nine (9) children, ten months to 3 years of age. T. Geter arrived at the facility at 9:30 a.m. At that time, the children were transitioned to their assigned classrooms, and T. Geter was counted in staff/child ratio in Space #3. In addition, T. Geter, who was counted in meeting the staff to child ratios, concurrently performed food preparation and other duties that were not direct child care responsibilities. When the children returned to the classrooms. I monitored each space and completed enrollment. Two additional children arrived during the visit in space 1a/b. Lunch served was hot dogs, mac and cheese, peas, peaches, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Geter. The following violations were observed. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In the space where children are served meals. There were two teachers grouped with fifteen children 10 months to 5 years of age. This was corrected when the administrator arrived to the facility and the children were transitioned to their classrooms. There were fifteen children ages 10 months to 5 years of age grouped together in the space where they receive their meals. This was corrected when the teachers transitioned the children back to their classrooms. .0713(a)(1) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. While monitoring the staff and training worksheet it was confirmed that the EMC Plan was not reviewed with six staff annually. 10A NCAC 09 .0802(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The administrator who was counted into staff/child ratio had to preform food preparation duties. This was corrected when an additional staff was called into work. .0713(a)(8) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff did not complete the Shaken Baby Syndrome and Abusive Head Trauma Policy prior to providing care to children. Hire date was 8/21/2025. This was corrected on 1/6/2026 when the Shaken Baby Syndrome and Abusive Head Trauma policy was reviewed and sign and place in to the staff file. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm June 10, 2026, on you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail how and when the violations were corrected. I introduced myself to the staff and asked if Tracia Geter, Administrator, was present. The staff stated that T. Geter was not present; however, they would contact her by telephone. Since the children were sitting and waiting for the next activity, I asked the teachers if they were getting ready to take the children back to their assigned classrooms. One of the teachers assisted the children with personal care and hand-washing routines. The teacher took a group of six (6) children to the bathroom, while the other teacher was left in the fellowship hall with nine (9) children, ten months to 3 years of age. T. Geter arrived at the facility at 9:30 a.m. At that time, the children were transitioned to their assigned classrooms, and T. Geter was counted in staff/child ratio in Space #3. In addition, T. Geter, who was counted in meeting the staff to child ratios, concurrently performed food preparation and other duties that were not direct child care responsibilities. When the children returned to the classrooms. I monitored each space and completed enrollment. Two additional children arrived during the visit in space 1a/b. Lunch served was hot dogs, mac and cheese, peas, peaches, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Geter. The following violations were observed. While monitoring the staff and training worksheet it was confirmed that the EMC Plan was not reviewed with six staff annually. 10A NCAC 09 .0802(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The teacher would ask the children about the colors they see. She would say the color and would try to get the children to repeat after her. Space #3 – The children were observed sitting at child size tables drawing different things that they would like to have for Christmas. The teacher would talk to them about their drawing and write what they said for them so that they could give it to the parents at the end of the day. The also were able to decorate a small Christmas tree while singing songs. The children also played with sort animals and small blocks. Space #3 – The children were observed sitting at child size tables drawing different things that they would like to have for Christmas. The teacher would talk to them about their drawing and write what they said for them so that they could give it to the parents at the end of the day. The also were able to decorate a small Christmas tree while singing songs. The children also played with sort animals and small blocks. Lunch served today was chicken nuggets, carrots, pineapples, sliced bread, and milk. At the completion of the visit, the Visit Summary was reviewed with T. Geter. The provider received an new inspection on 2/22/2024, and corrected during today's visit. GS 110-91 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff has not completed First Aid within 90 days of hire.
