The previous Annual Compliance Visit was completed August 4, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 100% which is above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a three-Star License issued August 23, 2023. The License was based on the program earning six (6) points in education standards, two (2) points in program standards, and one quality point by choosing to meet a Programmatic option: with services no more than two infants under one year of age. The information on record with DCDEE was verified as correct by Tonya Davis, Operator. Current Inspections are as follows: Sanitation Classification of Superior completed 12/2/2025. Ms. Daivs was present while I observed the indoor and outdoor areas. Upon entering children were observed playing in the play area of the home. The children were participating in free choice play. Lunch today was fish sticks, potatoes, mandarin oranges, and milk. An observation was completed from 9:25a.m. to 9:45a.m., the children were observed participating in free choice play. The children were pretending to cook, clean, and play dress up in the housekeeping play area. The provider was feeding one infant child while one child sat near her playing with some grasping toys. At the completion of the visit, the Visit Summary was reviewed with Ms. Davis. The following violations were observed. Violation Number Comment Rule 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The additional caregiver, did not renew CPR course prior to expiration date 12/2025. See the additional caregivers verification form attached. .1703(a)(3) 1735 The daily schedule, activity plan, infant feeding plan, allergy posting, SIDS Sleep Chart/visual check were not maintained on file for a minimum of 30 days from the revision or replacement date. The operator did not complete the SIDS Sleep Chart/visual check for one infant that was enrolled on 1/7/2026. This was corrected during today's visit. A SIDS Sleep Chart/visual check was started today. .1721((f)(4)(A) 1945 Items that are prohibited to be an infants sleep space, eg. pillows, wedges, blankets, toys, bumper pads, quilts, loose bedding, towels, and wash cloths were present in the crib with a sleeping infant. The provider had a pillow and two grasping toys in the crib with the child while sitting on the pillow in the crib. This was corrected by the operator removing the pillows and toy from the crib. .1724(a )(3) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. (New QRIS), Education Standards, CQI Plan, Family and Community Engagement Standards. Ms. Davis stated that she has already begun to work on the requirements for pathway two and that she is working with Kathy Hoffler, Family Child Care Consultant, with the Partnership for Children, who has already begun to work with her. We reviewed her education in Works Unit. The plan is for Ms. Davis to complete all requirements and submit documents to me by February of request to be evaluated for a five-star license. During today’s visit, I discussed access and use of the NCFELD book in the development of activity planning as required by NC Child Care Rule 10A NCAC .0508 Activity Schedules and Plans. Your program have a copy of the publication. She talked to the children about what they were doing. The children were smiling and sharing and telling the provider about what they were doing. I encouraged the operator to keep having good interaction with the children, talking, playing, and singing with them. During today’s visit the Facility Application, Appendix A, Statement of Responsibilities was updated and the Legal Designee. At the completion of the visit, the Visit Summary was reviewed with Ms. Davis. The following violation was observed. The children were observed playing in the play area of the home. The children were playing in free choice activities. Lunch served today will be fish sticks, wheat bread, potatoes, pears, and milk. I completed an observation from 9:15a.m. to 9:35a.m.: the teachers were engaged with the Children. One of the teachers read the story “Brown, Brown, Bear” with the children and then talked about colors of the animals. The teacher asked each child to find the different color she requested from the different centers. The children enjoyed finding the colors and showing the teacher that they knew their colors. They got excited when she clapped for them and told them what a great job they were doing. The children also reviewed their shapes and participated in music and movement. At the completion of the visit, the Visit Summary was reviewed with Ms. Davis. The following violations were observed. Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Upon enter the facility the playground area was monitored. The outdoor play area where the children play there were three large holes that had been dug. The holes could be tripping hazards for children. .1719(a)(1)&(17) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility did not have a sanitation inspection completed within a twelve month period. prior to the inspection I received via email on 1/15/2024. the last inspection had been completed on 12/7/2022 GS 110-91 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. However, when providing feedback to children it should be descriptive rather than just saying “Good job” or “Thanks”, provide a brief description of the observed behavior. This feedback helps children know exactly what the behavior is that you would like to see repeated. For example, “Thanks for hanging up your coat all by yourself, Cameron. You sure are getting big.“ “Wow, Patrick, you just washed your hands all by yourself without Ms. Ellie or me even telling you to do it.” Tone of voice, facial expressions, being on a child’s level, and the timeliness of when the feedback is delivered are all variables that affect the spirit in which feedback is accepted. Upon enter the facility the playground area was monitored. The outdoor play area where the children play there were three large holes that had been dug. The holes could be tripping hazards for children. .1719(a)(1)&(17) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility did not have a sanitation inspection completed within a twelve month period. prior to the inspection I received via email on 1/15/2024. the last inspection had been completed on 12/7/2022 GS 110-91 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on February 19, 2024, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. However, when providing feedback to children it should be descriptive rather than just saying “Good job” or “Thanks”, provide a brief description of the observed behavior. This feedback helps children know exactly what the behavior is that you would like to see repeated. For example, “Thanks for hanging up your coat all by yourself, Cameron. You sure are getting big.“ “Wow, Patrick, you just washed your hands all by yourself without Ms. Ellie or me even telling you to do it.” Tone of voice, facial expressions, being on a child’s level, and the timeliness of when the feedback is delivered are all variables that affect the spirit in which feedback is accepted. This program currently operates with a three Star License issued March 24, 2020. The License was based on the program earning six points in education standards, two points in program standards, and one quality point by choosing to meet a programmatic option with serves no more than two infants under one year of age. The school-age child transitioned from playing on his tablet to playing with cars and trucks in the block area of the facility. The children were engaged in their playtime. Lunch served today will be ground beef meat with taco seasoning, tortilla’s, corn, pineapples, and milk. During today’s visit, while monitoring the operators file, Recognizing/Responding and Suspicions of Child Maltreatment had expired 6/30/2023. This training is part of the Health & Safety trainings and should be completed every five years. At the completion of the visit, the Visit Summary was reviewed with T. Davis. The following violation was observed.
