The previous Annual Compliance Visit was completed July 22, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 94%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four - Star License issued October 9, 2023. The License was based on the program earning seven points within education standards, two points in program standards, and one quality point by choosing to meet an education option with the child care administrator who has at least 10years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Doris Jones and the owner’s corporation status was verified on the Secretary of State’s website as current and active. Current Inspections are as follows: Sanitation Classification of Superior completed 4/9/2026. Fire Classification of Approved Daytime Only completed 2/6/2026. Ms. Jones was present during the observation of both the indoor and outdoor areas. Upon my arrival at 9:10 a.m., the facility appeared to be closed. I contacted Ms. Jones by telephone to inform her that I was on-site to conduct the annual compliance visit. Ms. Jones stated that she was returning to the facility and would arrive within approximately 10 minutes. She also reported that the enrolled children are not scheduled to begin attendance until July 2, 2026. At the completion of the visit, the Visit Summary was reviewed with Ms. Jones. The following violations were observed. Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The administrator did not complete the ITS‑SIDS training on or before the expiration date of the previous certification. The previous training expired on May 26, 2026. .1102(f) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was not updated annually and did not have current information. .0607(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I contacted Ms. Jones via telephone to let her know I was at the facility to complete the annual compliance visit. Ms. Jones stated that she would be at the facility within 10 minis. She was at Daine’s Child Care and Learning Center Inc. Ms. Jones and I discussed enrollment at the facility, and she stated that she has not had any children enrolled since the week of May 12-16, 2025. All children that were enrolled at this facility are now enrolled at Daine’s Child Care and Learning Center Inc. We also discussed Ms. Jones’ concerns about safety in the area due to the homeless population as she stated in an email to me on 6/19/2025. She stated that she has been told by the sheriff department to contact them if any comes on the property. Ms. Jones stated that she is working on enrollment. At the completion of the visit, the Visit Summary was reviewed with Ms. Jones. The following violations were observed. Violation Number Comment Rule 604 Lavatories were not kept clean, in good repair and kept free of storage. The lavatories in the facility had not be cleaned. One toilet had a dark ring of build up in it. This was corrected by the operator going around and cleaning all four lavatories in the building. 15A NCAC 18A .2818(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two staff had not completed the Recognizing and Responding to Suspicion of Child Maltreatment training as part of the Health and Safety training every five years. .1103(b) 9999 A violation was found for which there is no item number. Foods items where kept free from spoilage, filth, or other contamination and was therefore, not safe for human consumption. In the kitchen, there was one box of Frosted Flakes with an expirations dated of 8/22/2024, one box of Fruity Pebbles with and expiration date of 9/7/2024. There were several cans of soups that were expired. This was corrected during the visit. The operator stated that she did not realize the food items were expired. Additionally, the foods were discarded during the visit and the operator stated she planned to check the refrigerator and foods stored weekly to ensure that foods are not expired. This a violation of Sanitation Rule 15A NCAC 18A .2804(a.) I encouraged the teacher to keep providing positive interaction with the children and to provide more conversation with the children. Self-talk and Parallel talk will both be necessary in providing a language enriching environment. Self-talk offers an opportunity to allow conversation to occur. For example, the teacher can talk out loud about what she/he is doing, seeing, hearing, or feeling. Parallel talk is important for the teacher to engage in conversations by talking about what the child is doing, seeing, hearing, or feeling. Reminders: Do you have children who bite? Do children struggle to focus during circle time? Do challenging behaviors cause difficult transitions? Do you spend all day managing behavior? Are you at your wits' end? The previous Annual Compliance Visit was completed July 22, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 94c% which is above 75%, required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four-Star License issued October 9, 2023. The License was based on the program earning seven points in education standards, two points in program standards, and one quality point by choosing to meet an educational option with the child care administrator who has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with the DCDEE was verified as correct by Doris Jones and the owner’s corporation status was verified on the Secretary of State’s website as current and active. Current Inspections are as follows: Sanitation Classification of Superior completed 8/12/2024. Fire Classification of Approved completed 1/15/2025. Ms. Jones, Operator, was present while I observed the indoor and outdoor area. Upon entering the facility, there were no children present. At 3:10p.m., the children arrived at the facility from school. The children entered the facility and participated in handwashing routines and had snacks. After the snack the children transitioned to free choice play. Today’s snack was lays veggie poppables and grape juice. An Observation was conducted from 3:10 p.m. to 3:25 p.m.: The children were observed participating in art activities, reading books, and playing with blocks. The teacher moved about the child care space assisting the children with their activities. At the completion of the visit, the Visit Summary was reviewed with Ms. Jones. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facility did not receive a fire inspection within a 12 month period 12/1/2023. This was corrected on 1/15/2025. 10A NCAC 09 .0304(a) 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. During the month of November one week of signing the arrival and departure was not completed. During the month of December and January three weeks of arrival and departure times were not completed. 10A NCAC 09 .0302(d)(4) 1301 Center did not maintain a record of daily attendance. During the months of October, November, December, and January the facility did not maintain accurate attendance records based on arrival and departure documents. GS 110-91(9) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on February 11, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. The staff should ensure that parents are documenting time and signing their children in/out when they are dropping off and picking up their children. I shared with Ms. Jones that there was a lot of positive interaction with the children. However there needs to be more conversations instead of directives going on during free choice play. I encouraged the teacher to keep providing positive interaction with the children and to provide more conversation with the children. Self-talk and Parallel talk will both be necessary in providing a language enriching environment. Self-talk offers an opportunity to allow conversation to occur. For example, the teacher can talk out loud about what she/he is doing, seeing, hearing, or feeling. Parallel talk is important for the teacher to engage in conversations by talking about what the child is doing, seeing, hearing, or feeling. Reminders: Do you have children who bite? Do children struggle to focus during circle time? Do challenging behaviors cause difficult transitions? Do you spend all day managing behavior? Are you at your wits' end? Two children arrived during the visit and Ms. Mckethan, administrator, arrived at 9:45 a.m. The facility is in the process of transferring children and getting subsidy vouchers changed over from Diane’s Childcare & Learning Center to Caydan’s Wiggles & Giggles Preschool & Learning Center. One child was taken back to Diane’s because her voucher had not been switched over to Caydan’s Wiggles & Giggles Preschool & Learning Center. It was stated by Ms. Mckethan that this was the child’s first day at the facility. One child file was not available for review. It has been missed place when the file was being transferred from Diane’s Child & Learning Center to this facility. There was a file for the sibling to the child that was present with no file. Lunch served today was chicken breast, baked beans, pineapples, sliced bread, and milk. During the annual compliance visit on 7/22/2024, you were cited 6 violations and during the visit one violation was corrected. You still have a compliance letter coming due on 8/5/2024, for violation #862, 1034, 1035, 1052, and 1824. At the completion of the visit, the Visit Summary was reviewed with Ms. Jones. However, the emergency information for children was not on file because transportation is not being provided at this time. Ms. Jones assured me that she would send documents before she transports any children. A follow-up visit will be conducted in the next two weeks to monitor for the children you have enrolled in the facility. Transportation was monitored during the visit. The teacher participated with the children. She encouraged them and praised them on how well they ran the race or when they won the game. The children were really enjoying the games. One staff file was monitored and one violation was cited. See attached staff and training worksheet. At the completion of the visit, the Visit Summary was reviewed with Ms. Jones. Snack server was cream cheese Danish and milk. The children then transition to handwashing routines and to free choice activities. The children were observed building a house out of cardboard box blocks and wood blocks. One child was observed cooking breakfast for the teacher. Lunch served today will be applesauce, turkey, wheat bread, turnip greens, and milk. During today’s visit the provider stated due to staffing issues she is not providing transportation at this time and would contact me if and when she decided to start it again. Transportation was not monitored during today's visit. At the completion of the visit, the Visit Summary was reviewed with Ms. Jones. The following violations were observed Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. GS 110-102 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. In four children's file the transportation form that was available for review was for another facility and did not have all the required information for the facility that would that would be transporting the children. .1003(i)(j) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Four children's file did not have anything in writing that the parents were notified of the smoking and tobacco policy. .0604(j) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on August 7, 2023, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information.
