    Name of Operation: BENJAMIN MARTIN PRIME TIME Facility ID: 26001586 Consultant: RONA DAVIS Operation Type: Center Case Number: Visit Date: 10/13/2023 Number Present: 29 Completed Date: 10/13/2023 Age: From 5 To 10 Total Minutes: 150 Time In: 02:00 PM Time Out: 04:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a Routine Unannounced visit. The previous Annual Compliance Visit was completed May 11, 2023, and prior to today’s visit, the eighteen-month compliance history score was 88%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Three-Star License issued February 27, 2020. The License was based on the program earning four (4) points in education standards, two (2) points in program standards, and one (1) quality point by choosing to meet a programmatic option by providing staff benefits and Infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Sandra Bastian. Current Inspections are as follows: Sanitation Classification of superior completed October 4, 2022. Fire Classification of approved completed September 25, 2023. Sandra Bastian, Site-Director, was present and assisted me while observing the indoor area. The facility’s license has a restriction, “Playground does not meet childcare safety requirements”. Therefore, the playground was not monitored during the visit. Upon my arrival children were twenty-nine children in care. As children arrived, they were greeted by staff. They placed their personal items in individual bins. The area was shared with car riders. After attendance was taken, children lined up for outdoors. Children stopped so that they could use the restroom and wash hands. Some children got a drink of water from the water fountain. A variety of balls were carried outdoors. Some children stayed indoors. A child read a Sticky Situation card to other children, and the Site Director asked questions about the story. Next, the children washed their hands for snack. The children that were outdoors, retuned and washed their hands for snack. Snack consisted of vanilla crunch crackers and chilled 100% fruit juice. After snack, children cleaned their areas. The grouped that were inside went outdoors and took hoola-hoops with them. The children that came indoors, participated in the Sticky Situation activity. Today, was fun Friday. At the completion of the visit, the Visit Summary was reviewed with S. Bastian. The following violations were observed: Violation Number Comment Rule 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility's annual sanitation inspection was not current. The last sanitation inspection was dated 10/4/22. GS 110-91 The violation(s) must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on October 27, 2023, describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the deadline, an annual compliance follow-up visit will be conducted to verify that all violation(s) have been corrected. Contact Environmental Health at 910-433-3676. Inform them that you had a monitoring visit and you have until October 27, 2023 to receive a current inspection. Observation Inspection: Expand on the children experiences by offering verbal and non-verbal support when necessary. For example, when children are waiting, play a game, ask question, or sing/chant songs. Let the children share how their school day went. When children are waiting for long periods of time during transitions, typically some may have to be redirected or positive discipline is needed. Provide a balance of adult and child-directed activities where children can make guided decisions based on teacher’s clearly identified expectations (example, where to sit at a table).
