Name of Operation: BEN MARTIN ELEMENTARY Facility ID: 26002166 Consultant: RONA DAVIS Operation Type: Center Case Number: Visit Date: 2/25/2025 Number Present: 15 Completed Date: 2/25/2025 Age: From 4 To 5 Total Minutes: 225 Time In: 09:00 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements and the NC Pre-K program using Child Care Rules Section .3000 pertinent to an annual compliance visit. The previous Annual Compliance Visit was completed March 5, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 99%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-star license, issued October 10, 2024 by earning the Cognia Accreditation. The information on record with DCDEE was verified as correct by Kim Burns. Current Inspections are as follows: Sanitation Classification of superior completed March 6, 2024. Fire Classification of approved completed November 8, 2024 Kim Burns, Lead Teacher, was present and assisted me while I observed the indoor and outdoor areas. Upon my arrival, there were fifteen (15) children in care. They were observed on the playground; running, climbing, sliding, bouncing on large balls with a handle. The children participated in a game of shark with their teacher. A child was the shark. She ran to tag another child and they turned into the shark. Once inside, the children placed their jackets in their cubbies and washed their hands. They proceeded to activity centers (blocks, dramatic play, art). Children used playdough and created pictures using bingo dot stampers. I observed toileting, handwashing, staff/child interactions, sanitation, clean-up, group time, and lunch. Today’s lunch consisted of chicken sandwiches (bun), green beans, fresh orange, and milk. Ten percent of children and staff files were checked during the visit. NC Pre-K children files were included in a random selection. NC Pre-K Administrator: Corine Warren NC Pre-K (space #D7) Teacher: Kimberly Burns TA: Willette Smith At the completion of the visit, the Visit Summary was reviewed with Ms. Smith, due to Ms. Burns attending a meeting The following violation was observed: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member's first aid certification training expired 1/7/25. .1102(c) The violation must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on March 11, 2025 describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If I do not receive the compliance letter by the due date, an additional visit will be conducted to verify violations have been corrected. Obtaining the required certifications/trainings is important because it helps staff develop the skills and knowledge necessary to keep children in care safe and healthy. Refer to child care rule 10A NCAC 09 .1102(d) regarding staff requirements. You stated that Mr. Brost is scheduling a first aid training. The previous Annual Compliance Visit was completed March 30, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 99%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-star license, issued December 23, 2019, earning seven (7) points in education standards, six (6) points in program standards, and one quality (1) point by having reduced staff/child ratios by one in each group from the seven point level as required in .2818(c). The information on record with DCDEE was verified as correct by Kim Burns. Current Inspections are as follows: Sanitation Classification of superior completed October 4, 2022. Fire Classification of approved completed September 25, 2023. Kim Burns, Lead Teacher, was present and assisted me while I observed the indoor and outdoor areas. Upon my arrival, there were twelve (12) children in care. I observed children lining up for outdoors. Once outdoors, the children were observed running, climbing, sliding, playing with balls, hoola hoops, and other outdoor materials. Upon returning indoors, children washed their hands and went to activity centers (water, sand, manipulatives, blocks, dramatic play, used playdough, and participated in an art activity). I observed toileting, sanitation, lunch, and rest time. Lunch consisted of chicken tenders, waffles, corn, chilled strawberries, and milk. Positive communication was used between the children and teachers. For example, they discussed the letter J, which was the first letter of a child’s name. The teacher asked the child, “What else begins with the letter J?” and used scaffolding by giving the child clues. She asked, “What do we spread on toast?” The child said, “Jelly”. The teacher expanded on the answer by asking, “What type of jelly do you like?” Ten percent of children and staff files were checked during the visit. NC Pre-K children files were included in a random selection. The following violation was observed: Violation Number Comment Rule 899 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility did not have a current sanitation inspection. The last annual sanitation inspection was 10/4/22. GS 110-91 The violation must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on March 19, 2024 describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. The facility’s last sanitation inspection was dated 10/4/22. Contact Environmental Health and inform them of the facility’s violation and that you must have an inspection by 3/19/24. You stated that Environmental health has been contacted several times regarding an inspection. DCDEE WORKS Two staff member's information was checked in WORKS during today's visit. W.S. was enrolled with no information submitted. C. W. was not enrolled in WORKS. Please ensure that two staff members (W.S. and C.W.) enroll and/or update information in the DCDEE WORKS system using their individual NCIDs. Staff members will need to create a DCDEE WORKS account, upload required certificates, and submit their updated educational transcripts by mail or electronically.
