    Name of Operation: BAL PERAZIM CHILD CARE CENTER Facility ID: 26001040 Consultant: RONA DAVIS Operation Type: Center Case Number: Visit Date: 8/21/2025 Number Present: 9 Completed Date: 8/21/2025 Age: From 0 To 4 Total Minutes: 210 Time In: 09:00 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to a Routine Unannounced visit. The previous Annual Compliance Visit was completed February 5, 2025, and prior to today’s visit, the eighteen-month compliance history score was 99%. Your program currently operates with a five-star license issued December 20, 2019. The License was based on the program earning six points in education standards, six points in program standards, and one quality point by having a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher, group leader over the last 12 months. The information on record with DCDEE was verified as incorrect by Sharon Hassell and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Approved completed May 12, 2025. The facility’s fire inspection was completed on 8/1/25. The facility is waiting for the fire inspector to email the child care fire inspection form. Tanisha Carver, Administrator, was present and left during the visit. Sharon Hassell, teacher assisted me during the visit. There were nine children in care. In space #3, I observed children sitting with their teachers. One child was looking at a book. Another child was participating in tummy time. The teacher squeezed a soft rattle and talked to the child about what she was doing. Another teacher talked to children as they played in the dramatic play area. They used pretend food and dishes. In space #2, children were observed participating in group time activities. They discussed the days of the week, month of the year, and numbers. Specifically, the number twenty-one. After group activities, children played in activity centers. They were given a five-minute warning before cleaning up. Next, they went outdoors and played soccer, basketball, and they ran. Once returning indoors, children washed their hands. Today’s lunch consisted of beef stew, rice, mixed vegetables, apples, and milk. At the completion of the visit, the Visit Summary was reviewed with S. Hassell. The following violations were observed: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One new employee did not have a medical report on file. 10A NCAC 09 .0701(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff member did not have a signed acknowledgement form on file regarding they received the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy. The staff member received, reviewed, and signed an acknowledgement form during the visit. .0608(d)(1-4) The violation must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on September 4, 2025, describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. Box 1218 Raeford, NC 28376 Technical Assistant/Discussions/Reminders: A violation was cited for one new staff member not having a medical report on file. You thought the employees’ Tuberculosis form was a physical. Refer to child care rule 10A NCAC 09 .0701(a) Shaken Baby Syndrome and Head Trauma Policy: A violation was cited for not having an acknowledgement form on file for one staff member. Remember to review and have new employees sign an acknowledgement form and place in their file before caring for children under five years of age. Refer to child care rule NCAC 09 .0608(d)(1-4). This was corrected during the visit. During today’s visit you were given the following DCDEE resources: Orientation Form Staff File Checklist. Observation Suggestions: Fostering Children’s thinking Skills: Children’s thinking skills can be promoted throughout the school day in many classroom activities including centers, whole group instruction, meal and snack time, as well as transition. This is important when teachers are intentionally thinking about finding opportunities to do this at different times during the day, and not just during a structured instructional activity. Create opportunities for children to solve problems, experiment, and figure out how things work. • When children are building towers with blocks, the teacher asks, “What do you think will happen if we add more blocks? Why do you think that?” The teacher then asks children to add more blocks and talk about what happens. This allows children to experiment and learn more from the activity. Make learning meaningful and connected to children’s lives. • When going outside for a spring nature walk, a child notices flowers blooming. The teacher says, “Tell me about what you see coming out of the ground. Why are the flowers coming out now? Why didn’t we see them last month? Have you seen flowers like this at home or somewhere else?” This helps children deepen their understanding of their world. Use how and why questions that help children think about ideas. • When reading a story, the teacher asks questions, such as: “How do you think that made her feel? Why do you think that?” to help children think more deeply about the book. Link understanding with something previously learned. • Children are in a center time and playing store, buying and selling various items. The teacher reminds the children about their visit to a store the day before and connects what they saw and did with the current activity. Predict what may happen next. • When reading a book, the teacher asks questions, such as: “What do you think will happen next?” to help children with their prediction skills. Brainstorm new ideas make something in a new way. • When coloring, the teacher asks children to draw pictures of animals that lay eggs. After the children are done with drawing, the teacher and children sit together and make a list of all the animals they came up with. Reminders: T. C.’s CBC letter expires 12/16/25. The process for submitting the required information to the CBC Unit can begin within 6 months before the qualifying letter expires. The previous Annual Compliance Visit was completed February 22, 2024, and prior to today’s visit, the eighteen-month compliance history score was 95%. Your program currently operates with a five-star license issued December 20, 2019. The License was based on the program earning six points in education standards, six points in program standards, and one quality point by having a combined turnover rate of 20% or less for the administrator, program coordinator, lead teacher, teacher, group leader over the last 12 months. The information on record with DCDEE was verified as incorrect by Tanisha Carver and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed May 24, 2024. Fire Classification of Approved completed July 29, 2024. The fire inspection was received during today’s visit. Tanisha Carver, Administrator, was present while I observed the indoor and outdoor areas. Upon my arrival, there were twenty-three children in care. Children in space #1, were observed playing in activity centers. They used various developmental age-appropriate materials. I observed children using the water and sand table. Children washed their hands before and after use. In dramatic play, children were sitting on a couch feeding their dolls. One child had a car seat. In the math center, children were using magnets. Other children participated in drawing pictures using paper, markers, and crayons. One teacher was observed communicating in Spanish with a child. Children in space #2, were observed playing outdoors. They kicked a soccer ball and went on a bear hunt. Their teacher asked questions like, “What do we do if we see a bear?” Children in space #3, were singing and dancing to songs like the Itsy-Bitsy Spider with their teacher. I also observed children sitting, walking, and using soft materials. Children used tambourines and rattle shakers while singing and dancing. Today’s lunch consisted of chicken wings, dirty rice, cabbage, oranges, and milk. At the completion of the visit, the Visit Summary was reviewed with T. Carver. The following violation was observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current fire inspection was not submitted to DCDEE within one week of the inspection. A current fire inspection dated July 29, 2024 was received during the visit. 10A NCAC 09 .0304(a) The violation was corrected during the visit. Therefore, a compliance letter is not required. Box 1218 Raeford, NC 28376 Technical Assistant/Discussions/Reminders: Observation Suggestion: Continue to model positive and respectful ways of engaging children, by standing close to them, reflecting their emotions, and by responding to their attempts to communicate both verbally and nonverbally. Respectful language was used, such as addressing each child by their name and moments were taken to check-in with each child during activities to see how they were doing and what their needs are. Mailing Fire Inspection: A violation being cited regarding not mailing the facility’s current fire inspection to DCDEE within one week of the inspection. Refer to child care rule 10A NCAC 09 .0304(a). Consider mailing the fire inspection to DCDEE immediately after the inspection. During today’s visit, we discussed the New Accreditation Pathway Licensure. I forward an email regarding the Quality Rating Improvement System (QRIS) required by State Law 2023-40 including the three pathways to include the Accreditation Licensure.
