The previous Annual Compliance visit was completed on December 09, 2024, and prior to today’s visit, the eighteen-month compliance history score was 100%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a 5-Star License issued October 10, 2024. The license was based on the program earning Cognia accreditation. The information on record with DCDEE was verified as correct by Amy Elliot, Lead Teacher. Current Inspections are as follows: Sanitation Classification of Superior was completed on February 12, 2025. The most recent fire inspection was completed on September 05, 2025. A. Elliot, Lead Teacher, and a substitute teacher, were present while I observed the indoor and outdoor areas. Upon my arrival, sixteen (16) children were present and were participating in free-choice activities. Groups of children were observed playing with magnetic blocks at a table, browsing books in the library area, block building in the block area, putting puzzles together in a carpeted area, pretending to cook and pretending to shop at a farmer’s market in the dramatic play area. Children cleaned up the toys and materials before transitioning to the outdoor learning environment. The outdoor learning environment was monitored for playground safety requirements. During outdoor play, children were observed playing on stationary equipment, playing in sand with shovels and buckets, and running around the outdoor space. When the group transitioned back into the classroom, they lined up to wash their hands and sat in a carpeted area for a whole group activity. The lead teacher read a book to the children while the substitute teacher cleaned and sanitized tables to prepare for lunch. Routine care and handwashing practices were observed. Lunch consisted of chicken patty sandwich on whole wheat bun, carrots, banana, ketchup, and milk. Two staff files were reviewed. One staff member did not complete the required Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete CPR and First Aid training within 90 days of employment. One staff member did not complete CPR training prior to her CPR certification's expiration date. Two children’s records were reviewed, and all documentation was observed in compliance with applicable child care rules. I verified compliance by reviewing the physical files as well as the off-site verification forms for staff, children, and transportation. Program files were reviewed. The most recent fire drill was completed on October 31, 2025. The most recent shelter-in-place drill was completed on October 16, 2025. The most recent playground inspection was completed on October 15, 2025. NC Pre-K requirements were reviewed. The Creative Curriculum was implemented in the activity plan and the classroom activities. Teaching Strategies GOLD was used as a formative assessment tool. The Ages and Stages Questionnaires (ASQ-3) were used for developmental screenings and were placed in each child’s file. Hearing and Vision screenings were documented on each child’s health assessment form. Dental Screenings were placed in each child’s file. At the completion of the visit, the Visit Summary was reviewed with A. Elliot. The following violations were observed: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Knowledge of pediatric first aid, including pediatric CPR which addresses management of a blocked airway and rescue breathing, and the confidence to use these skills, are critically important to the outcome of an emergency situation. The lead teacher stated that they will register for an upcoming CPR and First Aid training that is scheduled on December 13, 2025 at the Partnership for Children of Cumberland County. Health and Safety Training Requirements A violation was cited today for a staff member not completing the required health and safety training courses within five years of the previous training. Teachers should be knowledgeable about infectious disease and immunizations because properly implemented health policies can reduce the spread of disease, not only among the children but also among staff members, family members, and the greater community. Knowledge of injury prevention measures in child care is essential to control known risks. Medication administration and knowledge about caring for children with special health care needs are essential to maintaining the health and safety of children with special health care needs. The lead teacher stated that she started taking the training but did not have a chance to finish all training topics. Upon my arrival, thirteen children were present and were participating in free-choice activities. Children were observed participating in a matching game, making structures with blocks and Legos, playing with toys trucks in the block center, making pizza and cupcakes in the dramatic-play center, and creating art with crayons, construction paper, stickers, markers, and dry-erase boards. Children had access to blocks, fine-motor, dramatic-play, science, literacy, and creative art materials. Children cleaned up the toys and materials before transitioning to handwashing and lunch time. Lunch consisted of cheese pizza, carrots, apple slices, and milk. Bathroom and handwashing routines were observed. Children were not observed playing outside during the visit, due to rainy weather. Two staff files were reviewed. Two staff members did not have documentation on file of completing all required health and safety training topics within the past five years. Two children’s records were reviewed, and all documentation was in compliance. I verified compliance by reviewing the physical files as well as the off-site verification forms for the staff, children, and transportation. Program files were reviewed. According to Child Care Rule .0304(b), each center shall be inspected annually by an Environmental Health Specialist and receive an approved or superior rating in accordance with applicable sanitation requirements adopted by the Commission for Public Health as described in 15A NCAC 18A .2800. I encouraged the lead teacher to remind the administrator to contact the Cumberland County Environmental Health Department at (910)433-3618 to schedule and obtain an updated sanitation inspection as soon as possible. Health and Safety Training A violation was cited today for staff members not having documentation of completing the required health and safety training topics within the past five years. According to Child Care Rule .1103(b), Health and safety training shall be completed as part of on-going training so that every five years, all of the topic areas set forth in 10A NCAC 09 .1102(b) will have been covered. Health and Safety topics include: prevention and control of infectious diseases, including immunization; administration of medication, with standards for parental consent; prevention of and response to emergencies due to food and allergic reactions; building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; emergency preparedness and response planning for emergencies resulting from a natural disaster, or a man-caused event; handling and storage of hazardous materials and the appropriate disposal of biocontaminants; precautions in transporting children, if applicable; prevention of shaken baby syndrome, abusive head trauma, and child maltreatment; CPR and First Aid training; recognizing and reporting child abuse, child neglect, and child maltreatment; and prevention of sudden infant death syndrome and use of safe sleeping practices. The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. These rule changes may or may not impact your facility. The multi-age group staff/child ratio changes support a previous rule change submitted by Early Head Start and the new rule for defining cooperative arrangements provides clarification to a previous change in statute. The previous Annual Compliance visit was completed on February 14, 2023, and prior to today’s visit, the eighteen-month compliance history score was 93% which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a 5-Star License issued January 02, 2020. The license was based on the program earning 7 points in education standards, 7 points in program standards, and one quality point by choosing to meet a programmatic option of reducing the group size by one in each group from the seven-point level as required in Rule .2818 (c). The information on record with DCDEE was verified as correct by Amy Elliot, Lead Teacher. Current Inspections are as follows: Sanitation Classification of Superior was completed on January 04, 2023. The most recent fire inspection was completed on September 08, 2023. A. Elliot, Lead Teacher, and Cynthia Quick, Teacher Assistant, were present while I observed the indoor and outdoor areas. Upon my arrival, thirteen children were present and were participating in free-choice activities. Children were observed participating in a matching game, making structures with blocks and Legos, playing with toys trucks in the block center, making pizza and cupcakes in the dramatic-play center, and creating art with crayons, construction paper, stickers, markers, and dry-erase boards. Children had access to blocks, fine-motor, dramatic-play, science, literacy, and creative art materials. Children cleaned up the toys and materials before transitioning to handwashing and lunch time. Lunch consisted of cheese pizza, carrots, apple slices, and milk. Bathroom and handwashing routines were observed. Children were not observed playing outside during the visit, due to rainy weather. Two staff files were reviewed. Two staff members did not have documentation on file of completing all required health and safety training topics within the past five years. Two children’s records were reviewed, and all documentation was in compliance. I verified compliance by reviewing the physical files as well as the off-site verification forms for the staff, children, and transportation. Program files were reviewed. A sanitation inspection has not been completed since January 04, 2023. NC Pre-K requirements were reviewed. The Creative Curriculum was being implemented in the activity plan and classroom activities. The Ages and Stages Questionnaires (ASQ-3) were used for developmental screenings and were placed in the children’s files. Hearing and Vision screenings were documented on the children’s health assessment form. Dental Screenings were placed in the children’s files. At the completion of the visit, the Visit Summary was reviewed with A. Elliot. The following violations were observed: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The operator did not schedule or obtain a sanitation inspection within the past twelve months. I encouraged the lead teacher to remind the administrator to contact the Cumberland County Environmental Health Department at (910)433-3618 to schedule and obtain an updated sanitation inspection as soon as possible. Health and Safety Training A violation was cited today for staff members not having documentation of completing the required health and safety training topics within the past five years. According to Child Care Rule .1103(b), Health and safety training shall be completed as part of on-going training so that every five years, all of the topic areas set forth in 10A NCAC 09 .1102(b) will have been covered. Health and Safety topics include: prevention and control of infectious diseases, including immunization; administration of medication, with standards for parental consent; prevention of and response to emergencies due to food and allergic reactions; building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; emergency preparedness and response planning for emergencies resulting from a natural disaster, or a man-caused event; handling and storage of hazardous materials and the appropriate disposal of biocontaminants; precautions in transporting children, if applicable; prevention of shaken baby syndrome, abusive head trauma, and child maltreatment; CPR and First Aid training; recognizing and reporting child abuse, child neglect, and child maltreatment; and prevention of sudden infant death syndrome and use of safe sleeping practices.
