The previous Annual Compliance Visit was completed November 12, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 92%. This program currently operates with a Four-Star License issued March 27, 2020. The License was based on the program earning seven points in education standards, four points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Chenell Horne, Lead Teacher. Current Inspections are as follows: Sanitation Classification of Superior completed March 2, 2026. Fire Classification of Approved completed July 25, 2025. Ms. Horne was present and accompanied me while observing the indoor and outdoor areas. Children independently engage in free choice play in their outdoor learning environment. In Space #1, there was one infant child present with one caregiver. The child was sitting on the caregiver’s lap with a basketball. The caregiver talked to the child about the ball. In Space #2, the children were playing with outdoor toys. The teacher was blowing the bubbles. After the children finished playing outdoors, the children washed their hands and completed toileting routines. Violations were observed for playground inspection and 3-month drills. Lunch served was beefaroni, corn, apples, bread, and milk. The most recent fire drill was completed on June 01, 2026, and the most recent 3-month shelter-in place drill and lockdown drill was completed April 02, 2026. The most recent playground check was completed for November 03, 2025, the lead teacher completed an additional playground check for today, June 3, 2026. At the completion of the visit, the Visit Summary was reviewed with Ms. Horne. The last monthly playground inspection was dated for November 3, 2025. This was corrected, the lead teacher completed the monthly playground inspection during today's visit. Shelter-In Place or Lockdown Drill Log: Shelter-in-place drills will need to be completed every three months. We discussed placing dates on the calendar for upcoming due dates for drills to ensure that drills are conducted as indicated in Rule 10A NCAC 09 .1719(a)(16) &.1721(e)(7). There was one staff member that talked to the children during an activity with bubbles. The children caught the bubbles and talked about the bubbles. There was one staff member sitting on the grass with an infant. The teacher talked to the child about the basketball that they were holding. The teacher also gave the child opportunity to play on the grass. There was one child that was upset, the teacher talked to the child, and redirected the child. There are books that can be printed out and utilized in your facility to help manage children’s behaviors. For example, Tucker Turtle Takes Time to Tuck and Think. There are some resources and training that are free of charge. The steps to access the resources are located on the main page, select For Teachers and Caregivers, and then you will see the option for the book to print. Once you print the book, the staff can utilize sheet protectors and use a binder. It may be best to print a cover page and add on the front of the binder. For additional accessories, provide a soft turtle to use as a prop. The staff members can place these items in the cozy area as a way for the children to transition when he or she is having a difficult moment. The book promotes strategies for helping young children manage behaviors when they are upset. QRIS: We talked about the QRIS process, and I reviewed the required forms to submit for the application for reassessment. The information on record with DCDEE was verified as correct by Ms. Arether Mills, Administrator. Current Inspections are as follows: Sanitation Classification of Superior completed August 18, 2025. Fire Classification of Approved completed July 25, 2025. A violation was observed for fire inspection. Ms. Mills was present and assisted me while observing the indoor and outdoor areas. Children independently engaged in free choice play of classroom materials. In Space #1, the children were sitting on the carpet with their caregiver, pretending to eat. The children were holding pretend food and talking to their teacher. In Space #2, the children were playing in the following activities: blocks, dramatic play, puzzles, and toys. Lunch served was pulled pork, mixed vegetables, rice, applesauce, bread, and milk. The most recent fire drill was completed on October 15, 2025, and the most recent shelter in place drill was completed August 02, 2025. The most current monthly outdoor inspection was completed on November 03, 2025. At the completion of the visit, the Visit Summary was reviewed with Ms. Mils. The following violation was observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was not completed within one year form the last. The last fire inspection was dated for July 1, 2024. This was corrected. The current fire inspection is dated for July 25, 2025. 10A NCAC 09 .0304(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A violation was observed for safe sleep policy and screen time. During today's visit, the caregiver was observing the child during sleeping. We talked about making sure that blankets are not used in the cribs. It may be possible that the infant can grab the blanket and cover their face. This could cause suffocation. In Addition, we talked about screen time is not allowed for children under three years of age. During today's visit, the screen time was playing with a children's show. The previous Annual Compliance Visit was completed February 21, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 98%. This program currently operates with a Four-Star License issued March 27, 2020. The License was based on the program earning seven points in education standards, four points in program standards, and one quality point by choosing to meet an education option for: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Ms. Arether Mills, Administrator. Current Inspections are as follows: Sanitation Classification of Superior completed January 18, 2024. Fire Classification of Approved completed July 05, 2023. Ms. Mills was present and assisted me while observing the indoor and outdoor areas. Children independently engaged in free choice play of classroom materials. In Space #1, one infant was lying on the floor playing with a musical shaker and the other children were sitting on the carpet with their teacher playing with books and shakers. A violation was cited today because cleaning supplies were stored on the counter, less than 5 feet from the floor. In addition, two violations were cited for staff records. In Space #2, the children were playing in the dramatic play area. The children dressed up in a police officer uniform and pretended to talk on the phone. Some of the children played with blocks. Lunch served was pork ribs, broccoli, oranges, bread, and milk. At the completion of the visit, the Visit Summary was reviewed with Ms. Mills. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #1, there was a container of Clorox wipes, one bottle of bleach cleaner, and one bottle of disinfectant, stored on a counter, less than, 5 feet from the floor. The administrator moved the items on a shelf greater than 5 feet from the floor. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The medical report was not completed prior to employment for (M.R., date of employment 02/06/2024). 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB test or screening was not completed prior to employment for (M.R., date of employment 02/06/2024). .0701(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on February 20, 2024, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Facility Specific TA/Consultation/Reminders : Storage of Hazardous Items A violation was cited today for Storage of Hazardous Items. Store items that are labeled “keep out of reach of children” and have no other warning can be stored on a shelf or in an unlocked cabinet. The shelf or cabinet must be mounted at least five feet above the floor. In one classroom, cleaning supplies were stored on the counter approximately 4 feet. Cleaning supplies will need to be stored greater than 5 feet. Staff Files: A violation was cited today, because the tb test and medical statement was missing from a new staff files. We discussed additional items needed. Prior to employment, it is best practice to ensure the Staff File Checklist is completed. The items required are notated on the form and it also list timeframes for other required documents. This helps to determine if all items are on file and available for review. In addition, make sure that you have the most recent Staff File Checklist for Centers. The Division of Child Development and Early Education (DCDEE) website provides updated forms and resources. A blank copy of the Staff File Checklist can be located by visiting https://ncchildcare.ncdhhs.gov, The form can be found by selecting Provider tab then select Provider Documents and Forms. Please refer to Child Care Rule 10A NCAC 09 .0701, 10A NCAC 09 .0704, and10A NCAC 09 .2408 for more information regarding this rule requirement. Storage of Supplies: Technical assistance was provided today for baby wipes, gloves, and diaper storage. We talked about the best areas to store these items. For the diapers, the diapers should be removed from the package and placed on the shelf or another location. Both wipes and gloves will need to be stored greater than 5 feet. The teachers will need to ensure that these items are out of the reach of children. The diaper packaging and gloves could be a potential choking hazard if consumed and the wipes have chemicals that could be harmful to children if consumed. Natural Outdoor Learning Initiative: The DCDEE has a new initiative – Natural Outdoor Learning Initiative. Through this initiative, we hope to: increase knowledge and confidence for everyone in developing and implementing best practices to support the creation, programming, and safe use of naturalized outdoor spaces in child care facilities.
