The previous Annual Compliance Visit was completed May 2, 2024, and prior to today’s visit, the eighteen-month compliance history score was 94%, which exceeds requirements as outlined in North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four (4) Star License issued June 11, 2021. License was based on the program earning seven (7) points in education standards, two (2) points in program standards, and one quality point by choosing to meet a programmatic option: Infant capacity restricted to no more than two children under one year of age. The information on record with DCDEE was verified as correct by Carole Lewis and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed January 16, 2025. An updated wall placard was posted however the operator stated a copy of the sanitation report was not provided to her during the inspection visit. An email was sent to the local environmental health agency requesting a copy of the report. Carole Lewis, Owner/Operator, was present and accompanied me while observing the indoor and outdoor areas. Two preschool-aged children were present in the child care area of the home including the operators’ preschool-aged grandchild. I observed the operator and children discuss Earth Day and complete an art activity. Children used paper coffee filters glued to construction paper and colored them with blue and green markers making it look like earth. The operator encouraged children to complete hand washing routines before transitioning to free choice activities in interest centers. Children cooked pretend meals with foods and cooking utensils in dramatic play and stacked Lego blocks in the manipulative area. Positive interactions and rich conversations occurred throughout the visit. The operator was observed assisting the child with cleaning up the area and preparing for outdoor play. Once outdoors, children noticed a duck from the neighborhood pond walking in the yard with her ducklings. The operator and children watched from afar, counting the number of ducklings. While outdoors, the rain started and the operator transitioned children indoors and begin preparing for mealtime. Toileting and hand washing occurred. For lunch, children were served beans and franks, sliced wheat bread, rice, fresh strawberries, and milk. A checklist was used to note the requirements monitored during today’s visit. The Verification of Required Information for Operator and Additional Caregivers and Statements of Responsibility forms were completed during today’s visit. The most recent fire drill was completed on April 22, 2025 and the most recent lockdown emergency drill was conducted on February 2, 2025. The monthly outdoor inspection was conducted on April 18, 2025. At the completion of the visit, the Visit Summary was reviewed with C. Lewis. Children cooked pretend meals with foods and cooking utensils in dramatic play and stacked Lego blocks in the manipulative area. Positive interactions and rich conversations occurred throughout the visit. The operator was observed assisting the child with cleaning up the area and preparing for outdoor play. Once outdoors, children noticed a duck from the neighborhood pond walking in the yard with her ducklings. The operator and children watched from afar, counting the number of ducklings. While outdoors, the rain started and the operator transitioned children indoors and begin preparing for mealtime. Toileting and hand washing occurred. For lunch, children were served beans and franks, sliced wheat bread, rice, fresh strawberries, and milk. A checklist was used to note the requirements monitored during today’s visit. For art, the children drew and created a treasure chest using crayons, markers, and construction paper. Staff transitioned children to the outdoor play area. Children played in sand using toy shovels, buckets, and molds. A wooden stationary structure with sliding boards, a stand-alone swing set, tricycles, a scooter, a resin playhouse, and sand/water table were available to children. Upon return indoors, children completed hand washing routines and were offered cool water to drink. Children were observed playing in interest centers with age appropriate materials such as magnifying glasses, toy farm animals, alphabet blocks, and framed puzzles. Children completed hand washing routines in preparation for mealtime. Lunch, prepared by the operator, consisted of turkey burger on wheat bun, garden salad with ranch dressing, baked French fries, fresh blueberries with whipped cream, and milk. Children were observed resting on individual cots with clean linen as soft instrumental music played. A checklist was used to note the requirements I monitored today. The following violations were observed: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Accessible to preschool children in care, an aerosol can of room deodorizer spray was stored on top of the operator's desk. Accessible to children, a bouquet of latex balloons were stored on the floor in the child care space. The violation was corrected when the operator removed the balloons and placed them in locked storage. .1719(a)(19) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. We also discussed the following strategies for achieving and maintaining compliance: review the resource with household members and additional caregivers; remind them to check behind each other; and periodically check the home throughout the day to ensure all hazardous items are locked. Recognizing and Responding to Child Maltreatment: A violation was cited during today’s visit regarding Recognizing and Responding to Suspicions of Child Maltreatment training. As a reminder, administrators and all staff must complete the required training within 90 days of employment. I recommend administrators require new staff to complete the training requirement during new staff orientation to ensure it has been completed in a timely manner. The previous Annual Compliance Visit was completed May 10, 2023, and prior to today’s visit, the eighteen-month compliance history score was 95%, which exceeds requirements as outlined in North Carolina General Statute 110-90(4)(c-d). This program currently operates with a four (4) Star License issued June 11, 2021. License was based on the program earning seven (7) points in education standards, two (2) points in program standards, and one quality point by choosing to meet a programmatic option: Infant capacity restricted to no more than two children under one year of age. The information on record with DCDEE was verified as correct by Carole Lewis and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed February 13, 2024. Carole Lewis, Owner/Operator, was present and accompanied me while observing the indoor and outdoor areas. Anaya Lewis, additional caregiver was present and assisted with the visit. Four preschool-aged children were present during today’s visit. Upon arrival, I observed the operator assisting children with toileting training and washing their hands in the bathroom. A. Lewis let the children in circle time activities by reciting the morning prayer, children’s daily affirmations, and reciting days of the week in Spanish and English. I observed the operator and children read Bird’s Berry Bounty and discuss finding the berries throughout the book. For art, the children drew and created a treasure chest using crayons, markers, and construction paper. Staff transitioned children to the outdoor play area. Children played in sand using toy shovels, buckets, and molds. A wooden stationary structure with sliding boards, a stand-alone swing set, tricycles, a scooter, a resin playhouse, and sand/water table were available to children. Upon return indoors, children completed hand washing routines and were offered cool water to drink. Children were observed playing in interest centers with age appropriate materials such as magnifying glasses, toy farm animals, alphabet blocks, and framed puzzles. Children completed hand washing routines in preparation for mealtime. Lunch, prepared by the operator, consisted of turkey burger on wheat bun, garden salad with ranch dressing, baked French fries, fresh blueberries with whipped cream, and milk. Children were observed resting on individual cots with clean linen as soft instrumental music played. A checklist was used to note the requirements I monitored today. The Verification of Required Information for Operator and Additional Caregivers and Statements of Responsibility forms were completed during today’s visit. Children records were monitored today. The most recent fire drill was completed on April 24, 2024 and the most recent lockdown emergency drill was conducted on March 20, 2024. The monthly playground inspection was conducted on April 22, 2024. At the completion of the visit, the Visit Summary was reviewed with C. Lewis. The following violations were observed: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Accessible to preschool children in care, an aerosol can of room deodorizer spray was stored on top of the operator's desk. The violation was corrected when the operator removed the aerosol can and placed it in locked storage. .1719 (a)(7) 1875 The EPR Plan did not include the location of the Ready to Go File and/or required information. The EPR Plan on file was not updated annually. The Ready to Go File did not contain applications of children currently enrolled. .1714(d)(10) 1924 Balloons were accessible to children. Accessible to children, a bouquet of latex balloons were stored on the floor in the child care space. The violation was corrected when the operator removed the balloons and placed them in locked storage. .1719(a)(19) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. Recognizing and Restoring to Suspicions of Child Maltreatment training was not completed by the operator and additional caregiver within the required time frame. .1703(a)(5) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. Health and Safety trainings were not completed by the operator and the additional caregiver as part of on-going training within five years. .1703(d)(2) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on May 16, 2024, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. We also discussed the following strategies for achieving and maintaining compliance: review the resource with household members and additional caregivers; remind them to check behind each other; and periodically check the home throughout the day to ensure all hazardous items are locked. Recognizing and Responding to Child Maltreatment: A violation was cited during today’s visit regarding Recognizing and Responding to Suspicions of Child Maltreatment training. As a reminder, administrators and all staff must complete the required training within 90 days of employment. I recommend administrators require new staff to complete the training requirement during new staff orientation to ensure it has been completed in a timely manner.
