The previous Annual Compliance Visit was completed January 28, 2025, and prior to today’s visit, the eighteen-month compliance history score was 96%, which exceeds requirements by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Five (5) Star License issued May 6, 2022. The License was based on the program earning seven (7) points in education standards, six (6) points in program standards, and one (1) quality point by choosing to meet an educational option: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Cathy Scriven, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed September 10, 2025. Fire Classification of approved for daytime care completed September 25, 2025 Cathy Scriven, assistant director, accompanied me in observing the indoor and outdoor areas. Michelle White, Lead Teacher, and Sianne Vega, Teacher, were present. Preschool-aged children were observed playing in free choice in activity centers. Teachers conducted small group activities and worked with children individually. Children played with playdough on trays with tools, built structures with pipes and large blocks, sensory tables, and dramatic play materials such as pretend foods, cooking utensils, and cleaning tools. Children dressed up in costumes, hats, and dolls. In circle time, I observed children and a teacher sing the alphabet, count to 100, and identify and spell sight words. Staff were observed escorting children to the rest room located in the hallway. Children were encouraged to clean up and transition for whole group. Staff escorted children to bathrooms located in the hallway complete toileting routines and handwashing. In preparation for mealtime teachers cleaned and sanitized tables while children completed hand washing routines. Outdoor play was not monitored. A Nutrition Opt Out form was on file for each child enrolled. Children rested on individual cots on clean linen as soft music played. A checklist was used to note the requirements monitored during the visit. Staff and training worksheets were received by email on January 16, 2026 and updated during the visit. A staff file was monitored for one (1) new employee. A Children’s Record form was completed and two (2) files were monitored. The most recent fire drill was completed on December 16, 2025 and monthly outdoor inspection was completed on January 14, 2026. A lockdown emergency drill was completed on January 20, 2026. May 5, 2025. At the completion of the visit, the Visit Summary was reviewed with M. White. The following violations were observed: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member, as identified on the staff and training worksheet, did not complete and submit required forms to complete a criminal background check prior to the expiration date of January 6, 2025. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. The criminal background check qualification letter on file for one staff member, as identified on the staff and training worksheet, expired on January 6, 2025. G.S. 110-90.2(b) & (d) & .2703(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Additionally, by 5:00pm on February 3, 2026, you must submit a written, dated, and signed statement to me. I also recommend the operator contact the Cumberland County Partnership for Children for technical assistance with classroom environment. The facility currents uses Creative Curriculum. Formative Assessments were discussed and how to access the approved list on DCDEE website. The facility wishes to remain their 5 star license. Criminal Background Check: The violations for criminal background check were cited today. The most current CBC qualifying letter on file for staff member Sianne Jusino-Vega expired on January 6, 2025. I shared that additional information regarding the criminal background check process can be found in NC Child Care Rule .2700 and on our website at https://ncchildcare.ncdhhs.gov/Home/DCDEE-Sections/Criminal-Background-Check-Unit. A current NCID and updated fingerprints from local law enforcement will be required to complete the new online criminal background check process. Please be reminded, if a valid qualification letter is not obtained by 5pm on February 3, 2026, staff member, S. Jusino-Vega, may not work or be on the premises of the facility until a valid qualification letter is received. Staff member S. The previous Annual Compliance Visit was completed February 1, 2024, and prior to today’s visit, the eighteen-month compliance history score was 97%, which exceeds requirements by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Five (5) Star License issued May 6, 2022. The License was based on the program earning seven (7) points in education standards, six (6) points in program standards, and one (1) quality point by choosing to meet an educational option: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. Preschool-aged children were observed playing in free choice in activity centers. The children played with dramatic play materials such as pretend foods, cooking utensils, and cooking meals in their pretend restaurant. Teachers followed children’s lead as they pretended to take orders for their meals. In the block area, children built towers using unit and soft foam blocks and vehicles. I observed two children and a teacher put together facial expression puzzles and had discussion about each one. Children were encouraged to clean up and transition for whole group to complete toileting routines at bathrooms in the hallway and then outdoors for free play. In the outdoor play area, children played soccer with a variety of balls, large plastic blocks, and ran relay races with teachers. Upon return indoors, children prepared for mealtime. For classroom jobs, two children were “waitresses” and assisted teachers with passing out lunch boxes from the refrigerator. For lunch, children were served meals prepared by their individual families. A Nutrition Opt Out form was on file for each child enrolled. As children prepared for rest time, teachers reviewed animals that hibernate asking children to name them. Children called out a variety of animals such as snakes and bears. Children rested on individual cots with clean linen as soft instrumental jazz played. A checklist was used to note the requirements I monitored today. The Staff and Training Worksheet was received January 13, 2025 and updated during today’s visit. The Children’s Record worksheet was completed for two (2) children’s files monitored. The most recent fire drill was completed on December 19, 2024 and monthly outdoor inspection was completed on January 6, 2025. A shelter-in-place emergency drill was completed on January 6, 2025. At the completion of the visit, the Visit Summary was reviewed with Ms. Scriven. The following violation was observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Cans of Lysol aerosol spray, containers of disinfectant wipes, and a bottle of cleanser were accessible to children in an unlocked free-standing cabinet. The violation was corrected when the administrator removed the items and placed them in a lock cabinet. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The shelf or cabinet must be mounted at least five feet above the floor. In one classroom, cans of aerosol spray, disinfectant wipes, and cleansers were stored in an unlocked storage area. Another teacher was observed reviewing the days of the week by reading them in order and identifying them by sounds of their first letter. Children played in the sand table with buckets, shovels, and molds. One teacher was observed playing soccer with a child and other children rode tricycles around the playground. Upon return indoors, children completed toileting and hand washing routines at bathrooms in the hallway. Children were observed engaging in story time as K. Baker read “Froggy Gets Dressed”. For lunch, children were served meals prepared by their individual families. A Nutrition-Opt Out form was on file for each child enrolled. Children completed hand washing before sitting on the carpet reading individual books. Teachers assisted children with preparing for rest time on their individual cots. Soft music played in the background. A staff and training worksheet was received on December 4, 2023 and updated during today’s visit. Children were encouraged to raise their hands to answer questions and were given a chance to find someone they looked like in the story. “Which one do you think you look like? I see the one I look like!” Teachers escorted children outdoors for a nature walk since the playground was not accessible. Upon return indoors, children completed toileting and hand washing routines. For lunch, children were served meals prepared by their individual families. A Nutrition Opt Out form was on file for each child enrolled. The previous Annual Compliance Visit was completed February 8, 2023, and prior to today’s visit, the eighteen-month compliance history score was 93%, which exceeds requirements by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Five (5) Star License issued May 6, 2022. The License was based on the program earning seven (7) points in education standards, six (6) points in program standards, and one (1) quality point by choosing to meet an educational option: The child care administrator has at least 10 years of documented child care administration work experience in a licensed program. The information on record with DCDEE was verified as correct by Cathy Scriven, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed October 28, 2022. Fire Classification of approved for daytime care completed September 9, 2022. I was greeted by Keyahna Alford, teacher and Michelle White, lead teacher. Cathy Scriven, Assistant Director, was present and assisted with the visit. I observed the indoor area however the outdoor area was not available due to tree construction near the playground area. Preschool-aged children were observed playing in free choice in activity center. The children played with magnetic colored tiles, manipulated playdough, cooked pretend meals in dramatic play, and painted on construction paper with paint brushes and tempera paint. Children cleaned up their areas and teachers encouraged to gather on the carpet for morning circle time. The teacher lead children in repeating the morning affirmation and classroom rules before story time. The teacher and children discussed the parts of the book, the author, and illustrator before reading “All are Welcome”. Children were encouraged to raise their hands to answer questions and were given a chance to find someone they looked like in the story. “Which one do you think you look like? I see the one I look like!” Teachers escorted children outdoors for a nature walk since the playground was not accessible. Upon return indoors, children completed toileting and hand washing routines. For lunch, children were served meals prepared by their individual families. A Nutrition Opt Out form was on file for each child enrolled.
