W. Johnson, operator assisted me with today’s visit. Your program currently operates with a four-star license, issued 7/22/2024 earning 7 points in the education component, 2 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for infant capacity restricted to no more than two children under one year of age. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent as of 3/17/25. The NC Secretary of State website was reviewed on 3/17/25 and your family child care is not corporately owned. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. No children were present at the time of the visit. Three school age children were present earlier in the morning prior to going to school. The following violations were observed. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the ABCMS provider portal training or create the facility roster. G.S. 110-90.2 & .2703(r) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. A shelter-in-place or lockdown drill was not practiced every three months, the emergency drill log reflected drills conducted on 9/12/24 and 1/11/25. .1719(a )(16) & .1721(e )(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 9, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. A certificate will be issued upon successful completion of the test. Email the certificate and Powerform along with your full name, position and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Emergency Drills: You must conduct either a shelter-in-place or a lockdown drill every three months. You conducted drills on 9/12/24 and 1/11/25. A drill was not practiced every three months. Additional Comments: NCID: Did you know that if you do not login on any DCDEE platforms (e.g., Moodle, WORKS, CBC) for a period of 12 months, your account will be archived? As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from eight to 14 characters. Jennifer Linhardt, Licensing Supervisor accompanied me during today’s visit. W. Johnson, operator assisted me with the visit. Your program currently operates with a four-star license, issued 4/26/2019 earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under age one. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 8/8/23. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six percent as of 4/23/24. I visited indoor and outdoor spaces. You were caring for one child in the designated room for child care. When I arrived, I observed the breakfast, transition to bathroom routines and to the outdoor space. Breakfast included bacon, cheese grits, raisins and milk. The following violations were documented. Violation Number Comment Rule 1719 Operator did not maintain the acknowledgement and written parental permission in a child's record for as long as the child was enrolled at the home and/or a copy of each document was not maintained on file for review by Division representatives. A child enrolled on 11/1/2021 did not have a written plan of care on file. .1712(e)(6) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. According to the emergency drill log, shelter-in-place or lockdown drills were recorded 9/15/23 and 1/16/24. .1719(a )(16) & .1721(e )(7) 1933 Operator was not positioned in a manner that would maximize his/her ability to hear and see the preschool age children in care, at all times to render immediate assistance. While the child was sitting at a table in the child care space eating breakfast, you stepped across the hall to clean the bathroom. .1711(a)(1) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. In an unlocked cabinet below five feet in the bathroom, there was a bottle off Clorox spay. .1719(a)(6) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. We discussed that you must position yourself to see and hear the children in your care at all times. You repositioned yourself so that you were able to see the child in the child care room. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. When I brought this to your attention you place the Clorox spray in a locked bedroom. Emergency Drills: You must conduct either a shelter-in-place or a lockdown drill every three months. Consider marking your calendar as a reminder of when drills are due or set a reminder on your phone. Recognizing and Responding to Suspicions of Maltreatment: Child care operators are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. We discussed that the health and safety training is required every five years along with the health and safety trainings. You stated that you completed the five year renewal of the training, a copy of the training was not on file. Rated License Information: Your three-year re-assessment was due by April 2022. You submitted your application for re-assessment during today’s visit. Your new 4 Star License is based on the following: Education –The operator has the North Carolina Early Childhood Credential (NCECC), an associate degree in early childhood education and has 25 years of experience. Based on the information noted above, your program earned 7points in the Education component. Program Standards –A self-study was submitted on March 6, 2024. Your facility has operational policies, earning 2 points in the Program Standards component. Quality Point – You have chosen to meet Programmatic Option: Serving no more than two infants under one year of age, therefore your program earned 1 Quality Point. Total –7 + 2 + 1 = 10 = 4 Star License. A new license will be mailed directly to you and must be posted upon receipt. At that time, return the old license to me as it remains the property of the State of North Carolina. I visited indoor and outdoor spaces. You were caring for two children four years old. When I arrived, I observed you and the children transition to the outdoor space. The children climbed on a stationary structure and rode in little tykes’ cars during outdoor play. The lunch menu reflected fish sticks, pasta salad, cucumbers, corn, and milk. The following violations were documented.
