In classroom space #1 children rested on linen covered mats. The lights were dimmed and a story played as the children rested. After rest time, the children were observed eating snack, snack included cheese-its and apple juice. The following violations documented during the 2/19/26 visit were monitored for compliance during this visit: 106- Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last documented fire drill at the facility was conducted on 8/6/24, a fire inspection was not scheduled or obtained within 12 months. This is a violation of a requirement in 10A NCAC 09 .0304(a). As of today’s visit, a fire inspection has not been completed at the facility. You stated that the county requested proof that facility was zoned as a child care facility.You contacted the town of Farmville to get the original zoning documents and a letter stating the facility was grandfathered in meeting zoning requirements. You stated you received the zoning information from the town of Farmville on 3/9/26 and submitted the information to Pitt County on 3/11/26. You shared that the information is still pending and therefore you have not been able to pay for the inspection. You also shared that you had spoken with customer service and they stated it usually clears and an inspection is done within two days. I encouraged you to contact the fire marshal, you called during the visit and left a message for the marshal. You shared with me an email received during the visit from county requesting the facilities floor plan and building code summary. As stated in the 2/19/26 visit summary, all violations must be corrected immediately and a compliance letter verifying this must be received by the established due date. I received a compliance letter from you on 3/5/26. Today, however, a repeated violation regarding fire inspection was documented. In your compliance letter 3/5/26 you stated to avoid another violation in this area you will note on an annual calendar in advice not to allow the fire inspection expire and on an updated compliance letter received on 3/10/26 you stated the Town of Farmville emailed you 3/9/26 with zoning information. You could now upload this to the inspection portal. After they review the documents I was told they can come out within two business days. As of today, a fire inspection has not been completed at the facility. Repeat violations demonstrate a pattern of noncompliance and may result in an administrative action per Child Care Rule 10A NCAC 09 .2204(6). You will be notified in writing of any action taken. V. Fields- Rodgers, administrator assisted me with the visit. Your program currently operates with a four- star license, issued 5/1/2023 earning 5 points in the education component, 5 points in the program standards component(meeting enhanced ratio and space). Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 3/6/25. The sanitation inspection was completed 9/9/25 with an “Approved” classification. The last fire inspection was conducted 8/6/24 and your facility was approved for daytime care only. The NC Secretary of State website was reviewed on 2/19/26 and Stephanie’s Grace Childcare Center LLC was listed as current-active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. I visited indoor and outdoor spaces. You were present and caring for four children in classroom space #1. The children were observed during outdoor free play. The children ran as they played, rode on scooters and played in the portable play house. Following outdoor play the children engage in an art activity. Lunch was observed and included tortilla chips, cheese, salsa, cucumbers, fruit cocktail and milk. The following violations were documented. GS 110-102 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. In classroom space #1 a cell phone played children's songs and learning videos, the screen was in view of children two years of age. .0510(d)(2)(A-C) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The administrator did not have a current health questionnaire on file. .0701(a) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. One child enrolled on 8/22/2g had an immunization record on file dated 1/16/2026. 10A NCAC 09 .0302(d)(2) 1757 A valid qualification letter was not on file and available to review at the facility. The administrators qualification letter expired on 1/12/26 and new letter was not on file. G.S. 110-90.2(b) & (d) & .2703(e) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Total safeguarding is achieved through a multiplicity of regulatory programs and agencies including sanitation and fire inspectors. Fire inspections should be completed annually and within 12 months of the previous inspection. The previous inspection was completed on 8/6/24 a current inspection was not on file. You were aware the fire inspection had not been completed and stated that you needed to get the inspection. Submit a current fire inspection with your compliance letter. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. Within in 30 days of enrollment a child must have a immunization record on file. One child enrolled on 8/22/25 had an immunization record on file date 1/16/26. The child’s file also did not include parent acknowledgement of receipt of the facilities operational policies, parent participation plan, summary of NC Child Care Laws, or the smoking and tobacco policy. Review children’s files often and use the Center children’s checklist to ensure you have all required documents within the specified timeframe. Criminal Qualifying Letter: Qualification letters expire five years from the date on the letter and paperwork can be sent in up to six months in advance of the expiration date in order to obtain a new letter. A tracking system should be put into place to help you remind staff to submit paperwork before a qualification letter expires. Staff V. Fields-Rodgers, criminal qualifying letter expired on 1/12/26. You were aware your qualification letter expired in January. We discussed that you will have fifteen days (March 6, 2026) to receive an updated qualifying letter, or you will not be able to return to work until the letter is received. Please submit the qualifying letter to me upon receipt as proof of correction with your compliance letter. Screen time: Screen time should only be offered to children over three years of age and used to meet a developmental goal. Screen time must be documented on a cumulative log or activity plan. In classroom space #1 a cell phone play children’s songs and videos, in view of a child two years of age. We discussed placement of the cell phone to ensure the screen is not visible or use a speaker to play music. Additional Comments: NCID: Did you know that if you do not login on any DCDEE platforms (e.g., Moodle, WORKS, CBC) for a period of 12 months, your account will be archived? An archived account cannot be reinstated. You will need to create a new one and then email all platforms (e.g., Moodle, WORKS, CBC) to merge accounts. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from eight to 14 characters. When you change your current password after Feb. 23, however, you will be required to create a new one that must: • Be a minimum of 14 characters. • Consist of at least one numeric character, at least one uppercase letter, at least one lowercase letter, and at least one special character such as #, &, * etc. • Passwords shall not contain number or character substitutes to create dictionary words (e.g., d33psl33p for deepsleep). North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet ofproperty of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. The last annual compliance visit was conducted 3/6/25. The sanitation inspection was completed 9/9/25 with an “Approved” classification. The last fire inspection was conducted 8/6/24 and your facility was approved for daytime care only. The NC Secretary of State website was reviewed on 2/19/26 and Stephanie’s Grace Childcare Center LLC was listed as current-active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. You were present and caring for four children in classroom space #1. The children were observed during outdoor free play. The children ran as they played, rode on scooters and played in the portable play house. Following outdoor play the children engage in an art activity. Michelle O'Kelley, Child Care Consultant accompanied me during today's visit. V. Fields- Rodgers, administrator assisted me with the visit. Your program currently operates with a four- star license, issued 5/1/2023 earning 5 points in the education component, 5 points in the program standards component(meeting enhanced ratio and space). Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 4/25/2023. The sanitation inspection was completed 3/13/24 with a “Superior” classification. The last fire inspection was conducted 7/20/23 and your facility was approved for daytime care only. The NC Secretary of State website was reviewed on 4/10/23 and Stephanie’s Grace Childcare Center LLC was listed as current-active status. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. I visited indoor and outdoor spaces. A child was being cared for in classroom space #1. The child was observed drawing on paper with markers, you sat with the child and engaged in conversation with the child. Todays lunch menu peanut butter and jelly sandwich, yogurt, celery and juice.
