You, Beth Darby and Beth O’Kelly, Lead Teachers, assisted me with the visit. The QRIS Pathway to the Stars Pathway #3: Accreditation and Head Start packet was reviewed with Ryan Etheridge, Administrator. Your program currently operates with a five-star license effective April 20, 2022. You have earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space, and enhance ratios minus one (1). The last annual compliance visit was conducted on January 22, 2026. The sanitation inspection was completed December 3, 2025, with a “Superior” classification and six (6) demerits. The last fire inspection was conducted on August 26, 2025, and your facility was approved for daytime care only. The most recent monthly fire drill was conducted on March 24, 2026, at 10:30am and recorded. The most recent quarterly shelter-in-place drill was conducted on March 16, 2026, at 8:35am and recorded. The facilities EPR Plan was reviewed on June 6, 2025. The most recent monthly playground inspection was completed March 6, 2026, by Beth Darby. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent (100%) percent as of April 21, 2026. Classrooms operating hours are from 7:30am to 2:30pm. The environment rating scale score received on March 30, 2022, was 6.0. I visited both the indoor and outdoor spaces. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance with child care rules. Children throughout the facility were participating in the following activities and/or routines. In Space #1, I observed children and staff during hand washing, bathroom routines, offered water, group time, teacher directed activities, transitioning to free play, and lunch. In Space #2, I observed children and staff during indoor free play, bathroom routines, hand washing, group time, and lunch. The caregivers were interacting and meeting the developmental needs for each of the children in care. Lunch time was observed and children were served either a cheeseburger on a bun with a lettuce and tomato slice, or pepperoni and cheese hot pocket, tatar tots, whole red apple or peaches, and milk. A limited monitoring of staff files was conducted for the purpose of reviewing CPR/First Aid and special training. Supervision, staff/child ratio, storage of hazardous products, storage of medication, adequate/approved space, license posted, permit restrictions and program records were also monitored. You provided me with applicable program and staff records for review. Pathway #3 Packet for Rated License: During today's visit, I reviewed with you, Mr. Etheridge, Administrator, the documentation submitted for Pathway #3: Accreditation and Head Start from the Cleveland County Schools Office of School Readiness. The documents that were reviewed with you included the Application for Assessment for a Rated License for Centers, the Classroom Staff to Child Ratio for Accreditation or Head Start form, Pathway #3-Request to Use Accreditation and Head Start Licensure Pathway to Earn a Star Rated License form, and the Office of Head Start verification documentation Award Letter. I reviewed with you the QRIS Pathway to the Stars Pathways information, documents, and requirements during the facility’s last annual compliance monitoring visit conducted on January 22, 2026, Rated License Information: The facility’s application for re-assessment was received from the Cleveland County Schools’ Office of School Readiness on March 16, 2026, choosing the Accreditation and Head Start Pathway #3 option. Your new 5 Star License is based on the following: The license is based on the Head Start educational and programmatic requirements that are comparable or exceed NC QRIS requirements. Your program meets the Head Start Ratio. A Classroom Staff to Child Ratio for Accreditation or Head Start form was completed and I received this document on March 16, 2026. This form is to be posted on each classroom space. The Request to use Accreditation and Head Start Licensure Pathway to Earn a Star Rated License form was submitted on March 16, 2026. An award letter for the current budget period of July 2, 2025, through June 30, 2026, was provided on March 16, 2026. I reviewed and completed with you, Ryan Etheridge, Administrator the Pathway 3 Accreditation and Head Start Star Level Assessment for Child Care Centers document during today’s visit. The program met the requirements for a 5 Star License. A new license will be mailed directly to you and must be posted upon receipt. At that time, please return the old license to me as it remains the property of the State of North Carolina. The following violation was observed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In Space #1, on April 2, 2026, seven (7) children’s departure times were not documented and were not available for review. 10A NCAC 09 .0302(d)(4) Technical Assistance for the violations cited during today’s visit. Documenting Arrival and Departure Times: -All children must be signed in and out each day including the time of the child’s arrival and departure. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. I also suggested to add checking the arrival and departure records as part of the classroom closing out the day procedures. Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before May 6, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Lead water testing, lead-based paint testing, and asbestos testing: We discussed the test results that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your facility. The lead water testing results were completed on April 17, 2024. April 21, 2026, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had no results available for review for lead-based paint. The Clean Classrooms for Carolina Kids™ team determined that Fallston Elementary was exempt from asbestos inspection requirements. The facility provided the appropriate documentation of a prior asbestos inspection/reinspection that meets the requirements of 10A NCAC 41C .1003. No additional action is required by the facility. You, Beth Darby and Beth O’Kelly, Lead Teachers assisted me with the visit. Your program currently operates with a five-star license effective April 20, 2022. You have earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space, and enhance ratios minus one (1). The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six percent (96%) as of November 1, 2023. The Secretary of State website was checked on November 3, 2023, and your business Cleveland County School was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, Tiffani Sims, your child care consultant, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, story time, and snack time. Inspections/Drills: The most recent sanitation inspection for your facility was conducted on December 8, 2022. A superior sanitation classification was issued with two (2) demerits noted on the grade card. The most recent fire inspection for your facility was conducted on September 25, 2023. The most recent fire drill was conducted on October 31, 2023, at 9:11am and recorded. The most recent shelter-in-place was conducted on September 20, 2023, at 10:15am and recorded. The most recent playground safety inspection was conducted on October 11, 2023. You stated two (2) new staff have been hired since your most recent annual compliance visit conducted on February 23, 2023. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violation(s) of child care requirements were observed and documented during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The most recent annual fire inspection documentation on file for review was completed on September 25, 2023. The facility’s annual fire inspection was due by September 13, 2023. 10A NCAC 09 .0304(a) Technical Assistance regarding the violations observed during today’s visit: -Fire Inspections Reports: As a reminder an approved fire inspection report is required to be completed either on or before the date of the last fire inspection. I suggested contacting the Fire Marshall’s office at least four to six weeks prior to when the fire inspection expires. The child care rule reference is 10A NCAC 09 .0304 On-going Requirements for a License (a). Compliance History: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. The period for background check qualification renewals was revised from three years to five years. 2. The provisional qualification period was reduced to forty-five days.
