Your program currently operates with a Four Star Family Child Care Home License issued December 6, 2023. Restrictions on the license include a first shift capacity of eight (8) children ages 0 through 12, a second shift capacity of eight (8) children ages 0 through 12, a third shift capacity of eight (8) children ages 0 through 12, fireplace/woodstove not used during operating hours, and no more than two (2) infants under 1 year of age . Restrictions were in compliance today. The last annual compliance visit was completed on February 18, 2025. The program's compliance history was reviewed with the operator. Prior to the visit, the facility’s eighteen-month compliance history was one hundred percent (100%). The Secretary of State website was reviewed on January 20, 2026 and Tabies Babies Day Care Center LLC was listed as current-active. You, O. FLoyd, owner/operator were present for the visit. There was one (1) child enrolled and one (1) child present. New substitutes/caregivers have not been hired since the last visit. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled; this program currently does not have four year old children enrolled. I observed the child arrive at your program; he washed his hands and then chose magnet tiles for free play. You discussed colors and building options and encouraged his persistence in building. An afternoon consisted of peanut butter crackers and milk. No infants are currently enrolled. The following violation(s) were documented. Violation Number Comment Rule 908 Health questionnaire was not completed annually. The most recent health questionnaire expired on March 5, 2025. .1703(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen (18)-month compliance history score of at least seventy-five (75) percent. Any violation(s) documented may impact the compliance history score. The violation was corrected during the visit, and a compliance letter is not due at this time. Reminders: You are due to review your EPR Plan this month. Reviewing the plan annually ensures that information is up-to-date and allows you to remain aware of your plans and procedures. You will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119.
