D. Houghton, Administrator, assisted me with the visit. Your program currently operates with a Notice of Compliance (NOC) issued 3/25/2025. Please contact me if you are ever interested in becoming fully licensed to earn a star rating. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six percent as of June 5, 2026. The NC Secretary of State website was reviewed on June 5, 2026, and Saint John Community Christian Church was listed as current-active. The Notice of Compliance was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants and toddlers played on the floor with toys. They were cared for on an individual basis. Sleep checks were completed, recorded, and maintained as required. The children in Space 3 were finishing lunch which was meatballs, mashed potatoes, pears, wheat bread, and milk. The children in Space 5 played on the playground. The fenced outdoor space had portable gross motor toys, sand play and opportunities for dramatic play. Shade was provided by a wooden and metal canopy. When they returned indoors, they washed hands to prepare for lunch. Except for infants, children slept on linen-covered cots. The teachers interacted with the children, meeting their developmental needs. Files for a new staff person were reviewed. Since the last visit, a sanitation inspection was completed on March 5, 2026. I observed and documented the following three (3) violations during today’s visit. Two were corrected during the visit. Violation Number Comment Rule 209 Children used space that was not approved. Twelve (12) children were cared for in a space that had not been approved for use. GS 110-91(1)&(4-5) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 5, an aerosol can of Lysol Disinfectant spray was stored on a shelf in the bathroom, out of reach of children, but was not locked. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In Space 5, two (2) tubes of Equate Kids 50 Sunscreen were stored in a child's cubbie and were within reach of children. In Space 3, an aerosol can of Equate Kids 50 Sunscreen and an aerosol can of Off Insect Repellent were stored in a book bag that was out of reach but was not locked. 15A NCAC 18A .2820(d) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before June 23, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An unannounced visit will be made soon to verify correction of Item #209. You are still required to send a compliance letter. Technical Assistance With Documented Violations: Approved Space – Any space used by children must be approved prior to use to ensure health and safety. Today, twelve (12) children were being cared for in Space 5 which had not been approved for use. You informed me prior to your Annual Compliance visit on February 25, 2026, that you wanted to use the space previously used as the pastor’s office for a preschool classroom. I sent you a letter outlining the steps to have the space approved, including building, fire, and sanitation inspections and a visit to measure the new space and see it set up for use by children. I received your Fire Inspection, completed February 7, 2026. At your Annual Compliance visit, you provided me a building inspection, completed January 23, 2026, approving the new space with a bathroom for fifteen (15) children. I measured the space at your Annual Compliance visit and informed you that it could hold thirteen (13) children at 25 square feet but told you to contact me once you received the sanitation inspection and had it set up for use. I received the Sanitation Inspection, completed March 5, 2026. The Environmental Health Inspector approved the space pending removal of the carpet, new flooring and new paint. I informed you to let me know when these renovations were completed and the classroom was set up with furniture, toys, and posted documents, and I would schedule a visit to approve the classroom. During a phone call on March 11, 2026, you informed me that you probably would not be completing the renovations until the summer and wanted to have the classroom ready for the new school year. Again, I told you to contact me when it was ready for me to observe and approve. See information below about approval of Space 5. Storage of Hazardous Items (Aerosol and Medication)- Improper storge of hazardous items and medications results in millions of accidental poisonings in children annually. In Space 5, a can of aerosol Lysol Disinfectant spray was stored out of reach of children on a high shelf in the bathroom but was not locked. In the same classroom, two (2) tubes of Equate Kids 50 Sunscreen were stored in a child’s cubby and were not stored out of reach of children. In Space 3, an aerosol can of Equate Kids 50 Sunscreen and an aerosol can of Off Insect Repellent were stored in a book bag that was out of reach but was not locked. You stated that the teacher probably forgot to lock the aerosols and a parent had brought in the sunscreen because that is the brand you are using with all children in the center. You immediately corrected these violations by locking the Lysol Spray, placing the sunscreen out of reach and locking the aerosol sunscreen and insect repellent. This item has been corrected. *Plug in Air-fresheners - Plug-in air fresheners can potentially pose serious risks to young children, including chemical poisoning from touching or swallowing fragrant liquids. They could potentially release chemicals, which may cause asthma attacks, severe skin irritation, and headaches. Low-lying outlets make them accessible, creating burn and poisoning hazards. New Space: You are looking to have a new space approved for child care. I have received the following inspections: Building Inspection, dated January 23, 2026, approving the new child care space and bathroom (15 children) and break room (for staff use only). Fire Inspection, dated January 30, 2026 Sanitation Inspection, dated March 5, 2026, approving the new space with the stipulation that carpet be removed and flooring replaced and walls painted. During your Annual Compliance visit on February 25, 2026, I measured the space. It has a capacity of 13 children. Today, I observed the space. It was set up with furniture, toys, and learning materials for use by preschool and young school-age children. You removed the carpet and replaced it with laminate flooring. The walls were freshly painted, and new blinds were installed on the windows. Required documents were posted where they would be visible to families. A small alcove provides a space for cubbies and cot storage. The space has a bathroom with a toilet and sink. The only storage permitted in a bathroom is for toileting and diapering supplies. Do not use the bathroom to store extra furniture such as chairs, tables, cots, or toys. We discussed how to adequately supervise bathroom use. Because the bathroom is part of the classroom, if children are able to use the bathroom and wash their hands with no assistance, they may ask to use the bathroom and use it without your direct supervision as long as you can hear them and you are aware that they are in the bathroom. Limit the bathroom room to one child at a time. If children need assistance with toileting or are still being diapered, you will need to request assistance if only one teacher is assigned to the classroom, so that one teacher can maintain supervision of the classroom and a second staff member can assist in the bathroom or take the child to a room with a diaper station. Space Capacity at 25 square feet Space 1 (currently used for Infants) – 8 children Space 2 (currently used for Toddlers) – 16 children Space 3 (currently used for Two’s) – 15 children Space 4 (approved for School-age) – 22 children Space 5 (currently used for Three’s – Five’s) – 13 children - NEW Total = 74 children Total Based on Building Inspection June 29, 2024 – 50 children January 23, 2024 – 15 children Total = 65 children Outdoor Capacity = 35 children at 75 square feet Based on this information, I will be changing your capacity from 50 to 65 children; however, be cognizant of the space capacity for each individual space so that you do not exceed the classroom capacities or the overall permit capacity. No more than 35 children may be on the playground at one time. Infants may not be on the playground with children, two years of age and older. One-year olds may not be on the playground with children three years of age and older. Reminders: Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Three-year water testing was completed 10/16/24 and is due again by 10/16/27. ABCMS – Your staff roster on the ABCMS Portal is current with all staff listed. Staff Worksheets – We reviewed your staff worksheet. I highlighted items that are coming due in the next year. *Health and Safety Training and Five (5) Year Cycle Tracking Requirements – Review Row 17 on Staff Worksheet for initial and five-year H&S Training renewal dates. Because there was confusion about your participation with subsidy, staff working with you at your Annual Compliance visit on February 25, 2026, have until February 25, 2027, to complete Health and Safety Training. Moving forward, new staff need to complete Recognizing and Responding to Suspicions of Child Maltreatment and CPR/First Aid within three months of hire and the rest of the training modules within twelve months. Staff working in the infant classroom have two (2) months to complete ITS/SIDS training but may not be left alone in the infant classroom until they have completed ITS/SIDS. Centers Operating with a Notice of Compliance – Church-sponsored centers that operate with a Notice of Compliance and do not accept subsidy are exempt from rules related to new staff orientation, playground safety training, Health & Safety Training, and Recognizing and Responding to Suspicions of Child Maltreatment. They are required to maintain CPR/First Aid training and ITS/SIDS training (if licensed for infant care). If they elect to receive subsidy, subsidy rules require them to meet child care rules related to new staff orientation (10A NCAC 09 .1101), Health & Safety Training, Recognizing and Responding to Suspicions of Child Maltreatment, and playground safety training, along with CPR/FA and ITS/SIDS (10A NCAC 09 .1102). Bottle warmers and any appliance with a heating element must be inaccessible to children and at least five feet or higher. Be careful when using bottle warmers even when out of reach to prevent them from falling over with hot water. The program’s compliance history was ninety-six percent as of June 5, 2026. The NC Secretary of State website was reviewed on June 5, 2026, and Saint John Community Christian Church was listed as current-active. The Notice of Compliance was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants and toddlers played on the floor with toys. They were cared for on an individual basis. Sleep checks were completed, recorded, and maintained as required. The children in Space 3 were finishing lunch which was meatballs, mashed potatoes, pears, wheat bread, and milk. The children in Space 5 played on the playground. The fenced outdoor space had portable gross motor toys, sand play and opportunities for dramatic play. Shade was provided by a wooden and metal canopy. When they returned indoors, they washed hands to prepare for lunch. Except for infants, children slept on linen-covered cots. The teachers interacted with the children, meeting their developmental needs. Files for a new staff person were reviewed. Since the last visit, a sanitation inspection was completed on March 5, 2026. At your Annual Compliance visit, you provided me a building inspection, completed January 23, 2026, approving the new space with a bathroom for fifteen (15) children. I measured the space at your Annual Compliance visit and informed you that it could hold thirteen (13) children at 25 square feet but told you to contact me once you received the sanitation inspection and had it set up for use. I received the Sanitation Inspection, completed March 5, 2026. The Environmental Health Inspector approved the space pending removal of the carpet, new flooring and new paint. During a phone call on March 11, 2026, you informed me that you probably would not be completing the renovations until the summer and wanted to have the classroom ready for the new school year. In Space 5, a can of aerosol Lysol Disinfectant spray was stored out of reach of children on a high shelf in the bathroom but was not locked. In the same classroom, two (2) tubes of Equate Kids 50 Sunscreen were stored in a child’s cubby and were not stored out of reach of children. In Space 3, an aerosol can of Equate Kids 50 Sunscreen and an aerosol can of Off Insect Repellent were stored in a book bag that was out of reach but was not locked. You stated that the teacher probably forgot to lock the aerosols and a parent had brought in the sunscreen because that is the brand you are using with all children in the center. New Space: You are looking to have a new space approved for child care. I have received the following inspections: Building Inspection, dated January 23, 2026, approving the new child care space and bathroom (15 children) and break room (for staff use only). Fire Inspection, dated January 30, 2026 Sanitation Inspection, dated March 5, 2026, approving the new space with the stipulation that carpet be removed and flooring replaced and walls painted. During your Annual Compliance visit on February 25, 2026, I measured the space. It has a capacity of 13 children. Today, I observed the space. It was set up with furniture, toys, and learning materials for use by preschool and young school-age children. You removed the carpet and replaced it with laminate flooring. The walls were freshly painted, and new blinds were installed on the windows. Required documents were posted where they would be visible to families. A small alcove provides a space for cubbies and cot storage. The space has a bathroom with a toilet and sink. The only storage permitted in a bathroom is for toileting and diapering supplies. Do not use the bathroom to store extra furniture such as chairs, tables, cots, or toys. We discussed how to adequately supervise bathroom use. Because the bathroom is part of the classroom, if children are able to use the bathroom and wash their hands with no assistance, they may ask to use the bathroom and use it without your direct supervision as long as you can hear them and you are aware that they are in the bathroom. Limit the bathroom room to one child at a time. If children need assistance with toileting or are still being diapered, you will need to request assistance if only one teacher is assigned to the classroom, so that one teacher can maintain supervision of the classroom and a second staff member can assist in the bathroom or take the child to a room with a diaper station. In Space 3, an aerosol can of Equate Kids 50 Sunscreen and an aerosol can of Off Insect Repellent were stored in a book bag that was out of reach but was not locked. You stated that the teacher probably forgot to lock the aerosols and a parent had brought in the sunscreen because that is the brand you are using with all children in the center. You immediately corrected these violations by locking the Lysol Spray, placing the sunscreen out of reach and locking the aerosol sunscreen and insect repellent. This item has been corrected. *Plug in Air-fresheners - Plug-in air fresheners can potentially pose serious risks to young children, including chemical poisoning from touching or swallowing fragrant liquids. They could potentially release chemicals, which may cause asthma attacks, severe skin irritation, and headaches. Low-lying outlets make them accessible, creating burn and poisoning hazards. New Space: You are looking to have a new space approved for child care. I have received the following inspections: Building Inspection, dated January 23, 2026, approving the new child care space and bathroom (15 children) and break room (for staff use only). Fire Inspection, dated January 30, 2026 Sanitation Inspection, dated March 5, 2026, approving the new space with the stipulation that carpet be removed and flooring replaced and walls painted. During your Annual Compliance visit on February 25, 2026, I measured the space. It has a capacity of 13 children. Today, I observed the space. It was set up with furniture, toys, and learning materials for use by preschool and young school-age children. You removed the carpet and replaced it with laminate flooring. The walls were freshly painted, and new blinds were installed on the windows. Required documents were posted where they would be visible to families. A small alcove provides a space for cubbies and cot storage. The space has a bathroom with a toilet and sink. The only storage permitted in a bathroom is for toileting and diapering supplies. Do not use the bathroom to store extra furniture such as chairs, tables, cots, or toys. We discussed how to adequately supervise bathroom use. Because the bathroom is part of the classroom, if children are able to use the bathroom and wash their hands with no assistance, they may ask to use the bathroom and use it without your direct supervision as long as you can hear them and you are aware that they are in the bathroom. Limit the bathroom room to one child at a time. If children need assistance with toileting or are still being diapered, you will need to request assistance if only one teacher is assigned to the classroom, so that one teacher can maintain supervision of the classroom and a second staff member can assist in the bathroom or take the child to a room with a diaper station. No more than 35 children may be on the playground at one time. Infants may not be on the playground with children, two years of age and older. One-year olds may not be on the playground with children three years of age and older. They ate in their classrooms and napped on linen-covered mats. Lunch was observed and consisted of meatballs, corn, mixed fruits, whole grain bread, and milk. Medication permission forms for OTC, prescription, and emergency medication have an administration log as part of the form. This must be completed any time the medication is given. Medication permission forms are valid for a defined length of time and must be updated, if needed. D. Houghton, Administrator, assisted me with the visit. Your program currently operates with a Notice of Compliance (NOC) issued 3/25/2025. Please contact me if you are ever interested in becoming fully licensed to earn a star rating. This is your first annual compliance visit. The sanitation inspection was completed 4/1/25 with a “Superior” classification. The last fire inspection was conducted 1/30/26 with a Satisfactory rating and approval for daytime care only. Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Three-year water testing was completed 10/16/24 and is due again by 10/16/28. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 2/20/2026. This facility is owned by Saint John Community Christian Church. The non-profit corporation was listed as current- active on the NC Secretary of State website. License and contact information were current. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. I monitored nine staff records, three children’s records and all program records. Children in the center were participating in routines and developmentally appropriate activities. Infants and toddlers were cared for on their own schedules. Documentation of safe sleep checks was available and feeding plans were posted. Children went outdoors in the afternoon after it warmed up. They ate in their classrooms and napped on linen-covered mats. Lunch was observed and consisted of meatballs, corn, mixed fruits, whole grain bread, and milk. I observed and documented the following violations during today’s visit. Two of the three violations were corrected during the visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. A medication permission form was not available for an AuviQ for a child in Space 1. A medical action plan was available. The staff had not administered the medication. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. The prescription box was not available for an AuviQ used by a child in Space 1. .0803(2)(a) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. Any violations documented may impact the compliance history score. Two of the three violations were corrected during the visit. The third violation must be corrected immediately. On or before 3/11/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the remaining violation was corrected. Over-the-Counter medication also needs to be accompanied by the original packaging so that you have all information and instructions related to the OTC medication. The prescription box was not available for an AuviQ used by a child in Space 1. You were not aware that this is required. You contacted the parent, and she brought in the prescription box. This violation has been corrected. Medication Permission Forms - All medications (prescription, OTC, non-medical topical ointment, lotion, or cream) for children enrolled at the center must have a current, completed medication permission form to ensure that staff are administering medication according to the parents’ or physician’s instructions even if there is a Medical Action Plan. A child in Space 1 had a Medical Action Plan for an Auvi-Q but did not have a completed and signed medication permission form. The administrator stated that the parent had not returned the permission form, but she immediately contacted the parent who came to the center to complete it. The medication permission forms provide additional information that could be critical to the administration of the medication and the care of the child. Medication permission forms for OTC, prescription, and emergency medication have an administration log as part of the form. This must be completed any time the medication is given. Medication permission forms are valid for a defined length of time and must be updated, if needed. Medications can be authorized for the following time frames: 12 months – topical non-prescription, non-medication creams and lotions like sunscreen or hand lotion (February 25, 2026 – February 25, 2027) 6 months – medications for chronic illness or emergency medications like Epi-pens, inhalers, anti-seizure medication, Benadryl (must be locked) (February 25, 2026 - August 25, 2026) 30 days – medications for acute issues like tooth pain like Children’s Tylenol (February 25, 2026 – March 25, 2026) *Each medication must have its own medication permission form.
