Upon my arrival, I was greeted by a staff member, K. Worriax. Ms. Worriax stated that the administrator, T. Clegg, was present. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. A total of thirty-six (36) children were present during today’s visit. I observed the children during free choice play and lunch. Today’s lunch was in compliance with the Meal Pattern requirements. The administrator submitted a compliance verification letter on June 6, 2025, stating the violation has been corrected. The following violation was monitored for compliance during today’s visit: Item #201-Nine (9) children were observed in space #2 and capacity was maintained for all other spaces. Therefore, an additional violation was not documented. The following violations were documented during today’s visit: Violation Number Comment Rule 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. Today’s menu did not include food substitutions prior to lunch being served. 10A NCAC 09 .0901(b) 544 Screen time was offered to children under three years of age. Mr. Sullivan and Ms. Thornton have not been qualified to work with children, and no valid qualification letter has been issued. Per North Carolina General Statue 110-90.2(b) a criminal record check shall be completed prior to working with children. In addition, a valid qualification letter must be on file and available for review at the facility as stated in Child Care Requirement .2703(e). I stated to the administrator that therapists without a valid qualification letter on file and available for review must not be left unsupervised with enrolled children. Additionally, I explained to the administrator that therapists have the option to complete a criminal background check and obtain a valid qualification letter, or they must be supervised at all times by a staff member. Please remember to submit a copy of the valid qualification letter as part of your corrective action letter if they choose to complete the criminal background check process. To keep young children engaged during transition times such as waiting for meals, caregivers may consider alternative activities such as reading books, singing nursery rhymes, or playing games like “I Spy.” Research shows that excessive screen time in young children may cause a delay in the development of social and communication skills. Upon my arrival, I was greeted by the administrator, T. Clegg. Ms. Clegg accompanied me as I completed a general walk-through of the facility which consisted of four (4) classrooms, the outdoor play areas, and the kitchen. A total of forty-four (44) children were present during the visit. The children were observed during outdoor play, naptime, and eating lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility is currently operating with a three-star rated license issued on August 15, 2019. The NC Secretary of State website was reviewed on June 3, 2025, and Leap of Faith Academy, LLC was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual fire inspection was completed on February 27, 2025, and the last annual sanitation inspection was completed on December 19, 2024. The most recent documented monthly fire drill was conducted on May 2, 2025, the most recent documented quarterly lockdown drill/shelter-in-place drill was conducted on March 10, 2025, and the most recent documented monthly playground inspection was completed on June 2, 2025. During today’s visit, a partial assessment of the child care requirements was conducted. I observed all required postings, and attendance logs. There are no infants enrolled in this program. I monitored program requirements, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, and the outdoor area & equipment. I also reviewed files for two (2) new staff members. The following violation was observed during today’s visit: Violation Number Comment Rule 201 A room was occupied by more children than the space allowed at 25-sq. ft. of floor space per child. During today’s visit, seventeen (17) children were present in space #4. The maximum space allowed for this classroom is fifteen (15) children. GS 110-91(6); .1401(f) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 92%. COMPLIANCE LETTER: All violations must be corrected immediately. Due to the nature of the violation cited during today’s visit, a follow-up visit will occur in the near future to ensure ongoing compliance has been maintained. TECHNICAL ASSISTANCE/CONSULTATION: During today’s visit, I observed seventeen (17) school-aged children present in space #4, which is licensed for a maximum capacity of fifteen (15) children. You stated that two (2) of the enrolled children attend on a part-time basis and share one enrollment slot. To remain in compliance with the licensed capacity for this space, it is recommended that you develop a written schedule clearly outlining the designated attendance times for these two part-time children to help prevent unintentional overlap. Additionally, since there are currently twenty-one (21) children enrolled in space #4, careful planning and monitoring are necessary to ensure that attendance does not exceed the approved capacity at any time. The Child Care Commission is tentatively scheduled to adopt the new rules for the Quality Rating and Improvement System (QRIS) Modernization, previously the star rated license assessment. Once the rules are adopted, the hold harmless for the rated license assessments will end and all programs will be required to transition. Before the new rules are adopted, please encourage all staff to create a WORKS account using their NCID credentials. The staff members should include administrators, lead teachers, teachers, program coordinators, and/or group leaders. Once the WORKS account is created, staff must submit their official transcripts. Providers without verifiable education may result in a reduction in stars at the facility. The annual compliance monitoring checklist for Child Care Centers was used to note requirements monitored today. Upon my arrival, I was greeted by a staff member, R. Hill. Ms. Hill stated the administrator, T. Cleggs, was present. I was unaccompanied as I completed a general walk-through of the facility which consisted of four (4) classrooms, the outdoor play areas, and the kitchen. A total of twenty-four (24) children were present during today’s visit. I observed the children participating in outdoor play and having lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a three-star rated license issued August 15, 2019. There are no infants enrolled in this program, so safe sleep logs were not monitored. I monitored program requirements, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, and the outdoor area & equipment. I also reviewed staff files, and a sample of the children’s records. This facility does not provide transportation. G.S. 110-102 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A mop bucket containing Lysol floor cleaner was stored in the bathroom that the children use. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. An employee with a hire date of 10-2-23 did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. An employee with a hire date of 10-2-23 did not complete the recognizing and responding to suspicious of child maltreatment training until 04-04-24. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. The staff and training worksheet form is accessible to you on the Division of Child Development and Early Education (DCDEE) website at https://ncchildcare.ncdhhs.gov/. To access the form, you must click on the “Provider tab” and scroll down to the “Provider forms and documents” section, where you will be able to find the form titled Staff and Training Worksheet. This form is available in a PDF format where you can type in it and save it to your computer for edits in the future. MONTHLY/QUARTERLY DRILLS: As a reminder, fire drills are required to be conducted monthly, and a shelter-in-place or lockdown drill must be conducted at least every three (3) months. The annual compliance monitoring checklist for Child Care Centers was used to note requirements monitored today. Upon my arrival, I was greeted by a staff member, R. Hill. Ms. Hill stated the administrator, T. Cleggs, was present. I was unaccompanied as I completed a general walk-through of the facility which consisted of four (4) classrooms, the outdoor play areas, and the kitchen. A total of twenty-four (24) children were present during today’s visit. I observed the children participating in outdoor play and having lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a three-star rated license issued August 15, 2019. The NC Secretary of State website was reviewed on December 17, 2024, and Leap of Faith Academy, LLC was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was completed January 8, 2024. The most recent annual fire inspection was completed on March 12, 2024. The most recent annual sanitation inspection was completed on February 15, 2024. The last documented monthly fire drill was conducted on December 5, 2024, the last documented quarterly lockdown drill/shelter-in-place drill was conducted on September 3, 2024, and the last documented monthly playground inspection was completed on December 2, 2024. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings and attendance logs. There are no infants enrolled in this program, so safe sleep logs were not monitored. I monitored program requirements, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, and the outdoor area & equipment. I also reviewed staff files, and a sample of the children’s records. This facility does not provide transportation. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 115 A summary of the NC Child Care Law was not posted in a prominent place in the center. A copy of the summary of Child Care Law was not posted. G.S. 110-102 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A mop bucket containing Lysol floor cleaner was stored in the bathroom that the children use. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. An employee with a hire date of 10-2-23 did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. An employee with a hire date of 10-2-23 did not complete the recognizing and responding to suspicious of child maltreatment training until 04-04-24. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. An employee with a hire date of 10-2-23 did not complete all the health and safety training until 12-1-24. .1102(a) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 95%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By December 31, 2024, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. Therefore, it is important to update your staff and training worksheets as you hire/dismiss employees and complete ongoing training, because it will help you document the center’s compliance with child requirements. The staff and training worksheet form is accessible to you on the Division of Child Development and Early Education (DCDEE) website at https://ncchildcare.ncdhhs.gov/. To access the form, you must click on the “Provider tab” and scroll down to the “Provider forms and documents” section, where you will be able to find the form titled Staff and Training Worksheet. This form is available in a PDF format where you can type in it and save it to your computer for edits in the future. MONTHLY/QUARTERLY DRILLS: As a reminder, fire drills are required to be conducted monthly, and a shelter-in-place or lockdown drill must be conducted at least every three (3) months. ANNUAL LICENSE FEES: Invoices have been emailed to the facility addresses on file as of November 30, 2024. Online payments are due by December 31, 2024. In accordance with North Carolina General Statute § 110-90(1a), licensed child care facilities are assessed an annual license fee. Upon my arrival, I was greeted by the administrator T. Clegg. A checklist was used to note the requirements I monitored today. I was accompanied by Ms. Clegg as I completed a general walk-through of the indoor and outdoor areas, which consisted of four (4) classrooms, the kitchen, and the outdoor play environment. The children were observed during free choice activities, outdoor play, and teacher directed activities. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a three-star rated license issued August 15, 2019. ** Annual Inspections: The last sanitation inspection was completed September 11, 2023, and received a ‘Superior’ classification with 11 demerits. The last fire inspection was completed on March 29, 2023. The last documented fire drill was conducted on January 5, 2024, the last documented lockdown drill was completed on December 15, 2023, and the last documented playground inspection was completed on January 3, 2024. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, and attendance logs. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. The three (3) violations cited during today’s visit were corrected. Therefore, a compliance letter is not needed. Prior to today’s visit, the 18 month compliance history for this facility was 99%.
