Name of Operation: GLADE CREEK ELEMENTARY PRESCHOOL Facility ID: 03000040 Consultant: KAREN YOUNG Operation Type: Center Case Number: Visit Date: 5/14/2024 Number Present: 14 Completed Date: 5/14/2024 Age: From 4 To 5 Total Minutes: 105 Time In: 09:45 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable childcare requirements for a routine unannounced visit. Paxton Wooten, lead teacher assisted me with the visit. Your program currently operates with a FIVE (5) Star license effective 6/11/18. Restrictions include 1st shift daytime care; meets enhanced space; meets enhanced ratios; meets enhanced ratios minus one. The program’s compliance history was 95 % prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, and children having individual time testing with the teacher. The teacher assistant was moving about the room engaging with children. No new staff were reported at this program. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 4/30/24 with trash being the only thing noted on the inspection form. Trash has been removed. A fire drill was recorded 5/10/24 evacuating ten (10) children and two (2) adults. A shelter in place drill for the facility was last recorded 3/6/24 at 9:30 am with twenty-two (22) seconds to shelter-in-place by principal, H. Bannock. The most recent fire inspection was on 3/13/24. I observed a sanitation inspection was last conducted on 4/23/24 earning a Superior classification with seven (7) demerits. You stated that you do provide transportation in the am & pm for NC Pre-K children. Ms. Wooten stated that all emergency forms for children who are transported on the bus are located on the bus the child rides. I observed the Requirements for Preschool children on Public School Buses. I observed the Preschool Bus Drivers Responsibilities and Procedures Orientation signed by Principal H. Brannock. I also observed Center Director Responsibilities and Procedures signed by Lead teacher, Paxton Wooten on 10/31/23 and I observed the Transportation Policies for Parents & the Public-School Off-Site Verification signed by H. Brannock ad H. Vogler, Assistant Superintendent on 10/31/23. The following violations were cited during today’s visit: Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The monthly fire drill entry on the Emergency Drill Log & Report was incomplete for the past two (2) months of April and May 2024. There was no entry for Time of drill nor Time required to evacuate. .0604(t); .0302(d)(5) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, 28, 2024. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. This visit was conducted by Karen Young, Childcare Consultant, with Paxton Wooten, Lead Teacher. Your program currently operates with a Five (5) Star license issued 6/11/18. Restrictions include 1st shift daytime care; meets enhanced space; meets enhanced ratios; meets enhanced ratios minus one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. On the outdoor environment, no violations were cited. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center play and cleaning up, preparing to go to the cafeteria and having lunch in the cafeteria. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on 1/10/23. A sanitation inspection was completed 8/29/23 with a Superior classification and eleven (11) demerits. The last fire inspection was conducted 4/4/23. Program records and required postings were monitored. A fire drill was conducted on 11/16/23 at 12:35 pm with one (1) minute, thirty-four (34) seconds to evacuate fourteen (14) children and two (2) adults by H. Brannock. A lockdown drill was documented on 11/16/23 at 9:45 am with forty-two (42) seconds to lockdown by D. Reeth. An outdoor inspection was documented on 11/30/23 by P. Wooten. The two (2) new staff members have not had the Emergency Preparedness Training so there was no documentation of the EPR training today. I have suggested they contact their local Partnership to request the training. I stated I will continue to look and let them know if I hear of an EPR training opportunity. Staff files and children’s records were monitored per DCDEE procedures. Two new staff files were monitored today. Both the lead teacher and teacher assistant are new employees at this school. There was one (1) violation cited in staff files. The teacher assistant had no file available for review today. Two (2) children’s files were monitored today. There was violation cited in the children’s files. One child had no documentation of Prevention of Shaken Baby Abusive Head Trauma. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. You stated that you do provide transportation in the am and pm for the NC Pre-K children. During today’s visit, I observed documentation of the Requirements for Preschool children on Public Scholl Buses, the School Bus Driver Responsibilities and Procedures Orientation signed by Principal, H. Brannock, the Center Director Responsibilities and Procedures signed by Principal and Lead Teacher, Paxton Wooten on 10/30/23, the Transportation Policies for Parents for each child being transported on 10/30/23, and the Public-School Off-Site Verification signed by H. Brannock and H. Vogler, Assistant Superintendent, on 10/31/23. The following violations were cited during today’s visit: Violation Number Comment Rule 1739 All records required were not available for review by a representative of the Division. The teacher assistant's staff file in the NC Pre-K classroom was not available for review today. .2318(1-8) 1812 The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The two (2) new staff in the NC Pre-K program have not had the EPR training in the last four months. .0607(c) 1907 A copy of the Prevention of Shaken Baby and Abusive Head Trauma policy was not given to or explained to parents of newly enrolled children up to five years of age on or before the first day the child receives care at the center. One child did not have documentation of the SBAHT policy in their file. .0608(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Thursday, December 28, 2023.
